About the role:
* Primary care friendly Athena EMR. We provide training and 1:1 HIT assistance on first day of practice. (Dragon Dictation; templates, smart phrases, Up-to-Date access)
* 2 or more providers at each site
* You have 2 dedicated MAs and a receptionist - all are bilingual, plus you will have Language Line support provided. MAs are HIGHLY trained 3 weeks of boot camp initially and the model of care is that providers go in 1 time, with MAs having taken care of the rest.
* M-F work schedule; 8 hour shifts. Tuesday Friday 8:30am -5:30pm; Monday s our centers are open until 8pm; providers rotate covering the 11am 8pm shift.
* Telephone call only 1 week primary and 1 week of backup per year. Get 10-15 calls per week. Can go wherever you want as long as access to your phone.
* Each physician gets 3 exam rooms.
* Referral Center, once provider submits referral through patient chart, the referral staff will do pre-auth, schedule appt with patient, do follow up and pull chart notes from visit and add to patients chart.
* E-consults available for specialties that might take a while for patients to be seen - send documentation, labs, etc out and the consult will send recommendations back within 48 hours.
* Structured work flow, with 2 hours of charting/catch up planned each day, not including your uninterrupted lunch.

Position Summary
The Clinical Dietitian is responsible to the Clinical Nutrition Manager. He/she works across the institution to provide quality nutritional support services. The Clinical Dietitian works with the food service management and staff to coordinate delivery of the prescribed diet and meet the nutritional needs of the patient. Position Responsibilities
Standard Work Duties: Dietitian (PRN)
- Evaluate patients for nutritional status based on nursing screening criteria/consults/screening reports and within the scope of established dietetic practice standards, federal/state regulations and The Joint Commission standards with a monthly productivity level of 1.2., validated by weekly productivity logs and monthly chart review.
- Implement and monitor nutrition care plans and document according to standards of care criteria with reassessments completed within three to five days for moderate risk and within 48 hours for high risk patients, as validated by monthly chart review; collaborates with fellow team members and communicates nutrition care plan when transferred to other units, when other team members are covering or weekend coverage and validated by chart review with the dietitian.
- Collaborate with and communicate effectively with physicians and members of the multi-disciplinary team to integrate information which refines and contributes to the overall medical care of the patient, with specific emphasis on nutritional care, through daily rounds, as validated by the customer feedback process and monthly chart review.
- Develop, coordinate and provide nutrition education to patients, families, staff, and physicians to ensure that nutritional care is maintained across the continuum of care and into the home/recovery setting through one on one, small groups, classroom setting and community setting as evidenced by weekly productivity log and participation in community programs.
- Serves as a liaison between patient, staff and food service department by integrating nutrition information, observed food delivered, patient prescribed dietary plan, federal/state regulations and The Joint Commission standards to ensure that optimal patient outcomes are achieved as validated by daily meal rounds and tray checks completed.
- Clinical competence and continuous improvement so that optimum patient outcomes are achieved through completion of Nutritional Competency, completion of mandatory education and completion of 15 hours of continuing education annually as mandated for Florida license and validated by annual mandatory education form.
- Demonstrates commitment to the dietitian team by attendance of department staff meetings and flexibility to meet workload so that patients' nutritional care needs are met by willingness to change scheduled hours of work as needed, provide coverage in other areas as needed, provide weekend coverage as needed and provide assistance to other staff members as needed as validated by attendance noted in department minutes and work schedules.

The Radiology Technologist performs general x-ray procedures consistent with industry standards, regulations and Meridian policy, under the direction of the Physician.

Responsibilties:
A day in the life of a Radiology Technologist at Hackensack Meridian Health includes:
- Communication: Explains procedures to patients to inform and alleviate potential anxiety and to minimize need for additional exposure. Informs patients of follow up instructions as required. All information relayed to patients is clearly and accurately documented in the charts.
- Compliance: Consistently complies with CLIA, OSHA, and other state/government regulatory standards/ requirements and agencies. Refers regularly to the POL Policy Manual. Maintains records or logs as required by regulatory agencies.
- Customer Service: Greets patients and visitors in a prompt, courteous and helpful manner. Smiles, making patients/visitors feel welcome and important; makes direct eye contact; responds promptly to patients' needs; monitors radiology schedule or requests and works to ensure full patient satisfaction.
- Documentation: Reviews chart in advance of patient intake. Completes all required documentation according to standards established by the physician, in an accurate, legible and timely manner, and enters the date of service using MM/DD/YYYY format; Signs and dates all charts within a timely manner.
- Equipment Maintenance: Demonstrates ability to troubleshoot equipment. Maintains radiology equipment by performing scheduled cleanings/tests and documents results. Acts quickly and responsibly to ensure equipment is functioning properly.
- Financial Responsibilities: Performs x-rays in a manner that minimizes costly errors or need for utilization of additional supplies.. Ensures that equipment is serviced on time.
- Inventory Management: Monitors work area, supply inventory and equipment. Notifies Practice Manager, or designee, when supplies are low. Assists with unpacking, storing, and/or distribution of supplies upon delivery.
- Patient Satisfaction: Promotes patient satisfaction by providing excellent service by interacting in a friendly, helpful manner. Communicates potential patient satisfaction issues to the practice manager. Keeps informed regarding the practice's performance on patient satisfaction.
- Post Scan Procedures: Develops films of acceptable quality with minimum retakes. Completes proper forms and documents findings for physician review. Enters all required information into the database. Prints reports and distributes as per practice policy.
- Pre-Procedure Responsibilities: Obtains required demographic information from patient and verifies insurance coverage prior to initiating procedure. Completes pertinent patient history and accurately records data into medical record and/or radiology equipment.
- Radiology Procedures: Verifies the identity of the patient by asking the patient's full name and date of birth, which should be compared to the physician order. Assists the patient on and off the x-ray table as needed and assesses the immediate environment for potential safety concerns.
- May be involved in rooming patients including taking vitals and documentation in EPIC.
- Other duties and/or projects as assigned.
- Adheres to HMH Organizational competencies and standards of behavior.
Qualifications:
Education, Knowledge, Skills and Abilities Required:
- High School diploma, general equivalency diploma (GED), and/or GED equivalent programs.
- Completion of an AMA approved school of Radiologic Technology.
- BLS certification.
Licenses and Certifications Required:
- NJ Registered Radiological Technologist License.
- Basic Cardiac Life Support Certificate.
Licenses and Certifications Preferred:
- American Registry of Radiologic Technologists Certification.

GENERAL SUMMARY:
The Clinic LPN assists physicians with examinations and procedures to serve the medical needs of patients with various health conditions in a professional, confidential, and caring manner. Coordinates the flow of patients, phone calls, lab work, and nurse visits to facilitate quality patient care and customer service goals. PRINCIPAL JOB FUNCTIONS:
