Position Title Ambulatory Nurse Manager Center for Advanced Medicine Medical Pavilion Position Summary / Career Interest: We are seeking an Ambulatory Program Manager to lead operations for a first of its kind program that provides coordinated care for patients with chronic conditions, including IBD, ILD, Cystic Fibrosis, and Pulmonary Hypertension.
This role oversees a large, fast‐paced department and ensures the smooth delivery of same‐day, team‐based visits where patients can see all required providers in a single appointment.

Carries out a plan of care specific to unit competencies for the target patient population.
Coordinates care delivery and education through collaboration with patient, family and all health care team providers to ensure safe, effective and quality patient care.
