What You’ll Do:
- Provide visits as assigned by the Clinical Services Manager, including the assessment, determination of eligibility, admissions processing, planning, implementation and evaluation phases of the nursing process. Assignments will vary depending on licensure.
- Obtain data and assess necessary information from patient physical, psychological, social and spiritual factors that may impact patient and family's needs and coordinate intervention by other members of the interdisciplinary team
- Initiate communication with the attending physician, interdisciplinary team and other agencies for the purpose of coordinating optimal care
- Provide visits to assess the patient's needs and update the plan of care
- Perform therapeutic, preventative and rehabilitative nursing procedures as directed by the attending physician
- Perform clinical pronouncement of death and provide support to family, as applicable to licensure.
- Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
Qualifications:
- Graduate from an accredited registered nursing program and licensed in the state
- At least one (1) year clinical practical nursing experience preferably in intensive care, rehabilitation or medical surgical nursing (preferred)
- Home health experience preferred
- Possess and maintain valid CPR certification.
- Must have reliable transportation, current driver's license and appropriate automobile insurance

Clinical and Operational DutiesGeneral Clinical
- Obtains health history and performs physical examinations.
- Diagnoses, treats, counsels, and coordinate care for each patient.
- Reviews and update problem list and longitudinal plan of care at every visit.
- Meets documentation standards and completes all progress notes within 24 hours following the patient visit.
- Fulfills on-call responsibilities in accordance with the schedule developed by ACCESS and responds to calls within 20 minutes.
- Prescribe medication in accordance with Illinois statute and professional practice guidelines.
- Performs all duties in conformity with the quality standards and clinical guideline established by ACCESS.
- Meets Meaningful Use standards which is defined as using certified EHR technology to improve quality, safety, efficiency, and reduce health disparities, engage patients and families in their health care, improve care coordination, improve population and public health; all the while maintaining privacy and security.
- Manages EHR In-Basket, transfers In-Basket responsibilities to colleagues when appropriate, and responds to messages (e.g. lab results, refills, patient calls, patient messages, co-sign orders, co-sign charts, and open charts) within 72 hours.
- Assures all clinical activities are accomplished according to acceptable guidelines for reimbursement.
Patient Support & Education
- Provides culturally competent care

- Ensures patients presenting with greater acuity than what is clinically appropriate within the health center are transferred appropriately.
- Meets intensity of service and/or severity of illness admission criteria.
PCMH Requirements
- Coordinates with health center staff at all levels to ensure patient needs are met.
- Participates in health center huddles and meets scheduled health center times and productivity standards.
- Assures patients are seen in a timely and clinically appropriate manner and involves health center employees including medical assistants and front desk staff in this process.
- Assures care teams properly carry out effective treatment, education and performs clinical tasks in accordance with acceptable policy and procedure.
Provider Development
- Participates in the orientation and training of staff as needed.
- Ensures care team meets internal certification requirements for patient education and the administration of medication.
- Participates in proficiency testing as required by ACCESS to ensure that proper standards of care are being maintained.
- May function as a preceptor in teaching programs including medical residents, medical students, advance practice nurse students as well as other allied health professions.
- Adheres to Health Resources and Services Administration (HRSA) and Federal Tort Claims Act (FTCA) regulations.

Requirements/Preferences
- Current Professional Licenses to practice as a PA in the State of Illinois required. Maintain CME requirements.
- Board Certification is required.
- Obtain and maintain a collaborative agreement upon beginning of employment at ACCESS. Complete Practice Reviews, as required by ACCESS policy.
- Obtain a DEA & Controlled Substance license upon beginning of employment at ACCESS.
- Two (2) years of working experience in the practice of primary care medicine in an outpatient setting preferred.
- Knowledge of applicable rules, regulations and standards relative to quality assurance, safety and infection control required
- Electronic Health Records competency; EPIC knowledge preferred.

The Associate Market Clinical Director will directly supervise, performance manage and train Clinical Directors within in his/her assigned market. The incumbent in this role is accountable for center performance objectives, P&L, growth, and culture. In addition to being accountable for the overall clinical outcomes of his/her assigned market, they will have a portion of their time allocated to direct patient care as a PCP and/or other clinical duties. The remainder of their time allocated to leadership responsibilities including Clinical Director performance, engagement, building a strong clinical-operations synergy and culture. The allocation of time is dependent on several factors including PCP capacity, market needs, size of centers, patient membership and Regional President direction.ESSENTIAL JOB DUTIES/RESPONSIBILITIES:
- Directs accountability for clinical outcomes and day-to-day management of Medical Directors in multiple centers within assigned market, with regular presence in the centers under direct management, and under the supervision of the Regional President.
- Ensures successful clinical operations and meeting/exceeding plan market earnings.
- Manages, mentors and coaches Medical Directors in his/her assigned market to deliver outstanding clinical outcomes.
- Oversees and facilitates talent development of the PCPs, NPs, and Medical Directors in his/her centers including, but not limited to leading facilitated practice (physician shadowing/coaching), conducting 1:1s with direct reports, executing leadership development plans and performance management.
- Cultivates a center-level physician culture that is fully aligned with and delivering on the ChenMed core model, core values & behaviors and service standards.
- Assists Clinical COE in training of new practitioners within the assigned centers.
- Participates in recruiting and interviewing PCP and specialist candidates.
- Partners with Clinical COE and Talent Acquisition to support clinical talent lifecycle to accelerate Clinical talent growth, including hiring of PCPs, NPs, and Medical Directors, managing, and mentoring physicians, role modeling exemplary clinical leadership.
- Establishes and supports the development and cultivation of successful relationships with payers, specialists, the community, and hospitals, among others, and driving a social media presence locally, as part of the core responsibilities for the role
- Monitors and supports overall market culture, responding with urgency to workplace concerns.
- Reviews/approves center-level referrals and provides back-up for market referral and delegated utilization authorizations.

The Associate Market Clinical Director will directly supervise, performance manage and train Clinical Directors within in his/her assigned market. The incumbent in this role is accountable for center performance objectives, P&L, growth, and culture. In addition to being accountable for the overall clinical outcomes of his/her assigned market, they will have a portion of their time allocated to direct patient care as a PCP and/or other clinical duties. The remainder of their time allocated to leadership responsibilities including Clinical Director performance, engagement, building a strong clinical-operations synergy and culture. - Directs accountability for clinical outcomes and day-to-day management of Medical Directors in multiple centers within assigned market, with regular presence in the centers under direct management, and under the supervision of the Regional President.

- Manages, mentors and coaches Medical Directors in his/her assigned market to deliver outstanding clinical outcomes.

- Participates in recruiting and interviewing PCP and specialist candidates.
- Partners with Clinical COE and Talent Acquisition to support clinical talent lifecycle to accelerate Clinical talent growth, including hiring of PCPs, NPs, and Medical Directors, managing, and mentoring physicians, role modeling exemplary clinical leadership.
- Establishes and supports the development and cultivation of successful relationships with payers, specialists, the community, and hospitals, among others, and driving a social media presence locally, as part of the core responsibilities for the role
- Monitors and supports overall market culture, responding with urgency to workplace concerns.
- Reviews/approves center-level referrals and provides back-up for market referral and delegated utilization authorizations.
