Shift: Tuesday - Saturday 3pm-11pm (Evenings)



We are seeking a Transitional Youth Behavioral Health Specialist to join our team. Your role will oversee clients in daily activities while teaching new skills, ensuring safety, and managing behaviors. As a member of the Pathways team, you will work with other team members and report to our Program Manager.



WHAT YOU WILL DO:




  • Responsible for direct supervision and safety of all clients
  • Responsible for proper behavior management of children through the use of behavior management techniques.
  • Build healthy relationships with clients and be a role model of healthy behaviors
  • Have a working knowledge of the child's treatment plan and objectives and structure activities to support that plan.
  • Responsible for creatively structuring activities for groups and conducting psycho-educational groups.
  • May be responsible for medication administration and other medical support.
  • Responsible for the review/writing of all required documentation.
  • Responsible for teaching, guiding, and assisting the client in daily life skills as appropriate for age.


WHAT YOU WILL BRING:



Our ideal candidate will have 2 years of relevant experience working with children and the following:

Role Description:

The purpose of a Behavioral Health Specialist at Oak Street Health is to provide care management services and to provide evidence-based short-term psychotherapy to our patients with behavioral health needs.  The Behavioral Health Specialist is a key resource for Care Teams across a region of Oak Street Health primary clinics. They are responsible for coordinating and supporting behavioral healthcare for our patients, in collaboration with Care Teams and a psychiatric consultant. The Behavioral Health Specialist will assess the needs of patients, make referrals to appropriate behavioral health resources and specialists, and provide short-term counseling and evidence-based treatments as indicated. This role has the potential option for remote work days based upon achievement and maintenance of program metrics.

Core Responsibilities:

  • Collaborate with the primary care teams to identify appropriate patients to refer to the Oak Street Behavioral Health Program
  • Screen and assess patients for common mental health and substance use disorders
  • Develop care plans and provide or facilitate referrals to Oak Street’s psychiatric consultant and/or external resources
  • Provide short-term counseling and evidence-based treatments
  • Coordinate care with Oak Street’s psychiatric consultant, including warm handoffs for telehealth visits and collaboration on care plans
  • Develop relationships with primary care teams and consistently communicate changes in behavioral health care plans
  • Accurate and timely documentation of patient encounters and counseling sessions in Oak Street’s electronic medical record

What are we looking for?

Required:

Our Cardiac Telemetry unit is unique in that our nurses take care of patients recovering from open heart surgeries, heart catheterization with stent placement, ablations, pace maker/AICD placement, cardiovascular surgeries and we are a comprehensive stoke/stemi facility.

What does growth look like

Working in the cardiac progressive care units is a great way to build your critical thinking and skills. Nurses from our departments grow into areas such as ICU, cardiac cath lab, EP lab, or leadership. The leaders of these departments work with you to ensure that they understand your goals and provide you with opportunities to learn more about positions you may be interested. We work collaboratively with other departments in the facility to ease any transition you are interested in making. Typically nurses are able to successfully progress to other positions or departments in 18 to 24 months. No matter what your goal is, your leader will take an active approach to help you achieve it.

- Graduation from an accredited School of Nursing
- Current state of Missouri RN licensure required.
