Overview

The staff Registered Nurse (RN) is a professional caregiver who assumes responsibility for a group of patients for a designated time frame and provides care to these patients via the utilization of the nursing process, nursing trends, technology, financial and human resources within the philosophy of the institution. The staff Registered Nurse (RN) is a professional caregiver who assumes responsibility for patients undergoing endoscopy procedures including prep, intra procedure and recovery phases for routine GI procedures of colonoscopy and upper endoscopy. The RN will provide direct patient care, rotating through the phases, and deliver moderate sedation under direction of physician. Works collaboratively with unit staff to provide care to outpatient populations and delegates to unlicensed personal as appropriate.

Position Highlights: NT6 Observation

• Number of beds on unit: 35
• Patient population (age, diagnosis, specifics about unit): The Observation Unit is a 35-bed unit that provides acute care for issues ranging from cardiac, to general surgery, to behavioral health. These patients receive holistic care that reflect the values and mission of Carle Foundation Hospital. Patients’ goal length of stay is 24-48 hours.
• Nurse to Patient ratio: 1:5-6
• Department culture and resources: The Observation unit is a fast-pace, team-oriented unit. We work closely with our physician partners and ancillary team members to achieve the best outcomes for our patients. We have a robust Partnership Committee that meets monthly. We have a unit-based educator, 2 nurse supervisors, charge nurses and a discharge nurse.
• Frequent nursing skills/equipment used: Foley/IV/NG insertions, Nursing prioritization, IV pumps, Lift equipment, SCDs, Telemetry monitoring, Patient connection

The Quality Outcomes Coordinator (QOC) works at the department-level, in collaboration with clinical staff and leaders to deliver effective, efficient, high-quality care. The QOC continuously evaluates care and documentation, identifies gaps in processes and facilities process improvement activities to improve outcomes. Implements evidence-based research into practice to ensure patients benefit from the latest innovations in the delivery of care. The QOC serves to increase the driving force and consistency of quality and safety at the unit level.

Overview

Oral Maxillofacial Surgery serves a diverse patient population, including trauma, head and neck cancer, cleft palate, biopsies, dental extractions, dental implants, pediatric and adult facial and jaw abnormalities. The procedural nurse performs assigned procedures and physical assessments. Evaluates and revises plan based on changes in patient status during procedural sedation. Administers moderate sedation in the presence of, and by the order of a provider. Assesses patient's health history, performs medication reconciliation, assesses compliance with prescribed plan of care. Performs telephone triage and assesses patient's problems or concerns by phone. Assesses patient's learning needs and performs education for pre-operative and post-operative care. $5,000 sign on bonus for external candidates with 1 year of nursing experience

$2,500 relocation bonus for over 50 miles OR $5,000 for over 100 miles

Responsible for the oversight, coordination, and management of the functional and financial outcomes during acute illness requiring hospitalization for patients of the Carle Foundation Hospital. Ensures patients receive proactive initial assessment of needs, ongoing evaluations, and initiation of discharge planning while facilitating a safe and timely transition from the acute care/hospital setting to an appropriate level of care outside the hospital. Utilizes the five components of case management: assessment, coordination, monitoring, implementation, and evaluation. Multidisciplinary Rounds are completed daily with the care team at the patient's bedside which assists the team for timely planning and collaboration.

Act as a liaison working with patient/family and physician to determine next level of careConducts case review presentations to educate peers on unique or challenging cases and scope of practice issues.Coordinates the transition from inpatient care to post-hospital care, working with pre- and post- hospital providers to ensure responsive and appropriate care is provided post-discharge.Documents plan of care and utilization issues in appropriate locations, including but not limited to: case management/utilization review software and the multidisciplinary plan of care document on all assigned patients.Evaluates effectiveness of plan of care to ensure the progression toward desired patient outcomes.Initiates intervention, both pre-hospital, in-hospital, and post-hospital, for patients and families identified from a proactive initial admission assessment, as well as through referrals from members of the health care team.Initiates timely referrals to other health care team members (quality improvement, risk manager, social workers, physicians, Home Services, etc.)Performs nursing activities of assessment, coordination, planning, monitoring, implementation, and evaluation. Essential Functions

Performs computerized axial tomography procedures and assigned invasive procedures by acquiring and processing patient exams in accordance with accreting bodies and/or manufacturer guidelines and as outlined in department protocols. This includes performing multiplanar reformats and/or 3D rendered images independently. Reviews cases with the radiologist when appropriate. Reviews labs and patient history to perform correct exam. Administers IV contrast, and oral/rectal contrast when appropriate. Recognizes and responds to any adverse reactions or allergic reactions.Sceens the patient for possibility of pregnancy and performs pregnancy testing if applicable.

Ensures patient, visitor and environmental safety. Reports safety events immediately including those that are a near miss.

Assists in instruction of students and new staff as necessary. May perform duties of CT Lead or Imaging Coordinator in his/her absence.

Meets productivity levels as established by department benchmarking. Assists other areas during downtimes so to maintain productivity.

May be required to perform duties of other modalities within the department to which licensers are held by the technologist/sonographer.

Complies with other job descriptions of cross-trained modalities also performed within the department.

Required secondary registries are not required for cross trained modalities at HRHC unless otherwise required by specific compliance, licensure, or accrediting bodies.

Department Specific Job Function

Performs computerized axial tomography procedures and assigned invasive procedures by acquiring and processing patient exams as outlined in department protocols including performing multi-planar reformats and/or 3D rendered images independently. Reviews case with the radiologist when appropriate.

Administers IV contrast syringes, and oral/rectal contrasts when appropriate and recognizes and responds to any adverse reactions or allergic reactions. Administers IV contrast material by means of existing IV line or by performing veni-puncture and placing IV line.

Responsible to review radiology orders for completeness and accuracy. Reviews patient history and supporting clinical data for medical necessity. Takes steps to acquire additional information if needed from the patient, ordering provider and/or Radiologist.

Demonstrates acceptable imaging quality standards by critiquing positioning, technical factors and alignment. Reviews patient data for accuracy, quality and completeness.

Transfers and verifies image data in PACS and provides necessary medical history and information for Radiologist interpretation. Utilizes the web server/EA viewer and email system to send exams for preliminary readings after hours.

Responsible for maintaining imaging equipment and workplace environment including maintaining supply inventories and orderliness of work area. Recognizes malfunctions and service needs and reports service needs in a timely manner.
