Position Overview

Aveanna Healthcare has grown tremendously during the past year and we are currently looking for a Physical Therapist (PT) to join our dynamic home health care team. In this position, you make it possible for patients to recuperate from illness or injury in the comfort of their own home

Full-Time Monday - Friday visits needed for the Omaha Metro.

Essential Job Functions:

- Perform initial and ongoing assessments to evaluate mobility, coordination, strength and current level of function.
- Responsible for administration, evaluation, and interpretation of tests and measurements to determine functional goals and outcomes.
- Provide a written evaluation and plan of care defining physical therapy needs, goals, progress and expected outcomes.
- Delivery of physical therapy services to comply with prescribed physician orders.
- Maintains plan of care by performing timely documentation of client performance, modifying goals and treatment as needed, and determine discharge care plans as appropriate.
- Responsible for discharge planning, including collaborating and educating the patient and his/her caretakers on home exercise plan to follow discharge.
- Participates in coordination of services with health care team, physicians, parent/guardians, nurses, etc.
- Documents according to policy and procedure and requires minimal supervision or instruction.
- Documents effectively resulting in no technical or clinical denials subsequent to review of documentation by payers
- Maintain effective communication between staff, healthcare team members and family. Serve as a resource consultant for clinical staff.
- Supervises other personnel (PTA, HHA) as applicable.
- Completes OASIS assessments where allowed by state professional practice and regulation.
- Follow policies and procedures and conforms to all applicable patient care standards, licensure regulations and state laws.
- Maintain effective communication between staff, healthcare team members and family. Serve as a resource consultant for clinical staff.
- Supervises other personnel (OTA, HHA) as applicable.
- Completes OASIS assessments where allowed by state professional practice and regulation.

Requirements:

- Degree in Physical Therapy from an accredited university/program.

Currently, we treat the following conditions: orthopedic, weakness, balance/falls/gait, cervical/back pain, general deconditioning, and other various conditions. Job Summary

Plans, organizes and delivers physical therapy programs that help clients/patients/residents recover and improve their mobility, relieve pain, increase strength and prevent deformities. Conducts examinations, evaluations, and interventions clients/patients/residents who are affected by injury or disease. Utilizes tools and techniques to administer physical therapy interventions following safeguards. Documents client's/patient's/resident's information for evaluation; establishes therapeutic plans and modifies them if necessary. Educates clients/patients/residents and their family members about treatment plans/schedules and self-exercises to be continued at home. Knowledge of and utilizes appropriate age-related treatments and evaluations relating to the needs of clients/patients/residents. May treat individuals of all ages with varying diagnosis and disorders. Will demonstrate documentation in accordance with departmental guidelines and meet all regulatory requirements. Exhibits flexibility with acceptance of variable work schedules/assignments. Participates in activities to promote the department, Good Samaritan Society and Sanford Health.   • Increase pallet recovery by educating and negotiating with NPD locations, while identifying and resolving misuse or black-market issues through close collaboration with other departments.
  • Support the development and optimization of Low Volume Recovery (LVR) routes, providing guidance and training to junior teammates on new processes.


Key Responsibilities May Include:

  • Operate a CHEP-owned box truck (or similar equipment) in full compliance with safety regulations and CHEP standards.
  • Execute efficient delivery and collection of pallets and other assets at designated locations
  • Document and report findings on site conditions and recovery challenges, ensuring accuracy and timely updates to internal systems.
  • Collaborate with team members to develop and implement improved processes, focusing on key performance areas within LVR operations.
