Job Summary

As a provider in our Atlas Medical Team, you will be entrusted with delivering mobile on-site primary care services to our vulnerable patient population residing in assisted living and other senior communities. This role demands autonomous management of complex healthcare scenarios, including multiple co-morbid conditions. Your expertise will extend beyond medical treatment, as you educate patients and their families on self-care, disease management, and end-of-life care. Collaboration will be a daily aspect of your work, liaising closely with facility staff and Atlas Patient Care support teams. All activities will be guided by quality measures provided by Atlas, emphasizing our commitment to high standards of care.   • Must have at least one (1) year of primary care experience

Responsibilities

  • Perform initial orthopedic telehealth and in person visits including but not limited to diagnosis, ordering imaging, and referring patients for in person care when appropriate.
  • The in person visits will be at a PCP office (exact location TBD) performing orthopedic exams and injections.
  • Build trust with patients through empathy, knowledge, experience, and shared decision making
  • Communicate and collaborate with physical therapists, physicians, and other members of the Icon clinical team
  • Create the patient?s personalized treatment plan and along with your dedicated care coordinator, determine next steps and navigate the patient through the journey
  • Participate in Icon product enhancement through feedback on the current technology platform
