Are you an experienced Nurse Practitioner passionate about providing high-quality care to older adults? Join our team in the PACE (Program of All-Inclusive Care for the Elderly) program and play a pivotal role in managing the health and well-being of frail and elderly participants.
Key Responsibilities:
- Conduct health histories, physical exams, and participant teaching.
- Order and interpret lab tests, perform procedures, and document care in the EMR.
- Coordinate and manage participant care daily under the supervision of the attending physician.
- Collaborate with other healthcare providers to ensure comprehensive care, including making referrals and requesting consultations.
- Serve as a liaison between physicians, nurses, and other healthcare professionals, offering education and support.
- Participate in the PACE provider call schedule and contribute to care planning as a member of the Interdisciplinary Team (IDT).
- Provide care to participants in homes, assisted living facilities, nursing homes, and hospitals when necessary.
- Assist in educating participants on Advance Directives and participate in discussions about Do Not Resuscitate (DNR) orders when appropriate.
- Supervise medical students and residents during their PACE rotation.
Requirements:
- Master's degree in clinical nursing from an accredited Nurse Practitioner program.
- 3-5 years of nursing experience, with Nurse Practitioner experience preferred.
- NJ: Each nurse practitioner shall be so certified by the New Jersey State Board of Nursing.
- At least one year of experience working with a frail or elderly population preferred (training provided if not).
- Reliable transportation, valid driver's license, and required auto insurance.
- Ability to work autonomously while reporting to the PCP and Center Director.
- Strong clinical assessment skills, organizational skills, and communication skills.
- Ability to work within an Interdisciplinary Team (IDT) and understand the physical, mental, and social needs of frail older adults.
- Ability to meet the physical demands of providing care in a dynamic, patient-centered environment.

The Physical Therapy Assistant Provides clinical physical therapy treatments for program participants on acute, restorative and maintenance levels as needed. Follows the treatment plan as per Registered Physical Therapist assessment and documented care plan.
QUALIFICATIONS:
- A graduate from a physical therapist assistant curriculum approved by the American Physical Therapy Association.
- Currently licensed as a Physical Therapist Assistant in the state of Colorado.

- Maintain proper documentation according to state, federal PACE regulations.
- Maintain proper records according to PACE policy and procedures.
- Perform and record, if required, as specified by the planned treatment program, non-evaluative on-going data: Measurements of vital signs, range of motion, strength and linear increments; Types of assistive mobility devices used, and the distances covered; Results of tests and evaluations performed by the registered physical therapist.
- Respond to psych-social support needs of the participant and/or the participant’s family.
- Communicate appropriately and effectively with all personnel and to document according to professional standards.
- Keep timely records, meet regularly with the Registered Physical Therapist and attend care conferences as requested.
- Participate in patient care conferences and other rehabilitation related meetings as appropriate.
- Communicate with Registered Physical Therapist and IDT members regarding patient progress, problems and plans.
- Participate in in-service training programs for all staff in the facility.
- Instruct participant and family members and nursing staff in restorative and therapeutic maintenance programs.
- Abides by regulations pertaining to PACE as established by Federal and State Agencies and the Volunteers of America Health Facilities policies.
- Attends required in-services & completes assigned online modules.
- Perform other duties as assigned.

- Assesses the need for adaptive Durable Medical Equipment (DME), specialized shoes and prosthesis followed by recommendations to Interdisciplinary Team. Receives authorization prior to purchases. Assesses all newly issued orthotic shoes, braces and prosthetics for appropriateness and proper fit.
- Assists as liaison with Senior Community Care nursing home and assisted living staff regarding physical therapy needs of participants who are their residents.

Job Summary / Responsibilities
1. Scrubs on all types of operating procedures; handles surgical instruments, sutures & sponges; anticipates the needs of the surgeon, assisting as necessary. *WORK SCHEDULE:*
Normal work hours and clinical hours for this position are Monday through Friday dayshift; 8:00am to 5pm. The role requires 3-4 clinical half-day sessions per week and 3 half days dedicated to scholarly work unless otherwise directed by program director.

Job Summary and QualificationsThe Occupational Therapist provides high-quality, patient-centered care by managing the complete process of occupational therapeutic care of an assigned group of patients, in coordination with the physician and the patient care team. - You will observe and report patient’s reaction to treatment and changes in patient’s condition.

Job Summary and QualificationsThe CT Technologist performs computerized tomography diagnostic procedures as outlined by the radiologist. Operates diagnostic equipment, verifies images and prepares room with necessary equipment and supplies for the procedure. Technologist is responsible for assisting in the primary mission of the medical imaging department by ensuring that proper diagnostic information is provided through the production of high-quality CT scans. Other duties include, but are not limited to; clerical, scheduling, diagnostic imaging, transportation, ordering supplies, inventory, cleaning and anything deemed necessary by the medical imaging management team. Assist in diagnostic work area as needed. Job may include direct supervision of students at the discretion of the area supervisor. Employee must have the same licensure or have completed the same training to satisfy the requirements of the discipline of the student. Direct supervision means the primary caregiver is readily available to the student throughout the shift.

Licensure/Certification/Registration:
- ARRT required upon hire
- SCRQSA required upon hire
- BLS – Healthcare provider on hire or within 30 days of hire from American Heart Association (AHA) or American Red Cross (ARC)
- CT registry preferred, but not required, within 6 months of departmental orientation completion date, at which point SCRQSA ARRT will be obtained.
Education:
