Responsibilities
We are currently seeking an enthusiastic and professional Full Time Home Health LPN.
This role is responsible for providing assistance with the primary caseload for our skilled home health care patients.
This position provides skilled professional nursing care to patients, their families and caregivers as prescribed by the physician and in compliance with the Home Health Medicare Conditions of Participation, Ohio Home Health licensure laws and agency policies and procedures.
Qualifications
Education : Graduate of approved nursing program
Licensure/Certification: Valid Ohio Licensed Practical Nurse (LPN) license, CPR
Experience: A minimum of 2 years' direct patient care experience in hospice or home care
Since this is a community-based ministry, all LPN's are required to be licensed drivers with automobiles in good working conditions and have automobile insurance in accordance with Ohio laws, they must also have a clean driving record for insurability purposes.
This position will serve the county where you reside.

The Banner Sun Health Research Institute is seeking a full-time neuropsychologist to join our multidisciplinary Memory and Movement Disorders clinical team.

The Banner Sun Health Research Institute (BSHRI) is a sub-specialty center of excellence at the forefront of clinical and research efforts in the areas of healthy aging, Alzheimer's disease and related neurodegenerative disorders, and Parkinson's disease and other movement disorders. The Neuropsychology Center at BSHRI includes 2 neuropsychologists and 6 full-time psychometrists, as well as an active training program with postdoctoral fellows and predoctoral practicum students. The Neuropsychology Center team provides clinical evaluation and consultation services and supports BSHRIs robust industry- and grant-funded research programs and outreach efforts. Our team of dementia experts at BSHRI includes neurologists, nurse practitioners, social workers, scientists, and researchers. This is an excellent opportunity for a provider who enjoys working in a collaborative, state-of-the-art environment to provide patient-centered care.

The Specialty Care Neuropsychologist will conduct comprehensive outpatient neuropsychological evaluations of older adults, primarily those receiving care in our Cleo Roberts Memory and Movement Disorders Center. Full psychometry support is provided for test administration and scoring. Common referral reasons include diagnosis of mild cognitive impairment and dementia syndromes, differential between neurodegenerative disorders, pre-deep brain stimulation evaluations, and assessment of psychiatric comorbidities. Consultation takes place at weekly interdisciplinary case conferences and among neuropsychologists. The Neuropsychologist is also engaged in the education and training of psychology graduate students, psychometrists, and other staff across departments. Opportunities exist for research involvement and scholarly productivity, based upon experience and interest.

Position Details As a Level 1 Trauma Center we see any and all trauma patients in the surrounding area.

We primarily see blunt force trauma patients (falls, motorcycle/motor vehicle accidents) as well as the occasional penetrating trauma (gun shot, stabbings, or impalements).

In the Surgical ICU we see any patient requiring ICU level of care after a surgical procedure; the exception is cardiac surgery and neurosurgery.

Trauma ICU is a 14 bed unit consisting of all private rooms and the Surgical ICU is an 8 bed unit consisting of all private rooms.

Both units are in different locations, but as a member of our team you can work on either unit helping to expand your clinical knowledge base with critically injured and critically ill patients.

Schedule is .9 FTE (36 hours)/Straight Nights.

Every other weekend/Every other holiday rotation.

We primarily see blunt force trauma patients (falls, motorcycle/motor vehicle accidents) as well as the occasional penetrating trauma (gun shot, stabbings, or impalements).

In the Surgical ICU we see any patient requiring ICU level of care after a surgical procedure; the exception is cardiac surgery and neurosurgery.

Job Summary

Job Duties

- Provides direct patient care and clinical support for physicians and other providers in the outpatient setting.
- Functions as an integral member of the health care team under the direction of the provider.
- Performs clinical duties such as preparing patients, collecting and reviewing pertinent information, performing technical duties, documenting using the electronic medical record, and supporting a safe environment for patients.
- Provides administrative and clerical support for the clinic/department including patient scheduling, filing reports, and following up on documentation, phone calls, and billing inquiries.
- Obtains and documents, using the rooming tool in the electronic medical record, the patient’s reason for visit, weight, vital signs, visit screening information, medication review, and pertinent information for provider.
- Sets up examination and treatment rooms between cases.

- Documents in the appropriate sections to create a complete and accurate medical record.
- Uses the electronic medical record and scheduling system as designated.
- Facilitates timely appointments for patients. Completes referrals and pre-authorizations as needed.
- Collects co-payments.
- Under the direction of the provider and/or site lead, assists with orientation of new personnel.
- Executes and documents ongoing patient communication, as directed by provider, prioritizes communications to the provider, responds to patient questions and requests to ensure efficient use of provider time and timely response to patient needs.
