The Radiology Technologist performs general x-ray procedures consistent with industry standards, regulations and Meridian policy, under the direction of the Physician.
In this role, you will need to be able to work in either Edison, NJ or Old Bridge, NJ.
Responsibilties:A day in the life of a Radiology Technologist at Hackensack Meridian Health includes:
- Communication: Explains procedures to patients to inform and alleviate potential anxiety and to minimize need for additional exposure. Informs patients of follow up instructions as required. All information relayed to patients is clearly and accurately documented in the charts.
- Compliance: Consistently complies with CLIA, OSHA, and other state/government regulatory standards/ requirements and agencies. Refers regularly to the POL Policy Manual. Maintains records or logs as required by regulatory agencies.
- Customer Service: Greets patients and visitors in a prompt, courteous and helpful manner. Smiles, making patients/visitors feel welcome and important; makes direct eye contact; responds promptly to patients' needs; monitors radiology schedule or requests and works to ensure full patient satisfaction.
- Documentation: Reviews chart in advance of patient intake. Completes all required documentation according to standards established by the physician, in an accurate, legible and timely manner, and enters the date of service using MM/DD/YYYY format; Signs and dates all charts within a timely manner.
- Equipment Maintenance: Demonstrates ability to troubleshoot equipment. Maintains radiology equipment by performing scheduled cleanings/tests and documents results. Acts quickly and responsibly to ensure equipment is functioning properly.
- Financial Responsibilities: Performs x-rays in a manner that minimizes costly errors or need for utilization of additional supplies.. Ensures that equipment is serviced on time.
- Inventory Management: Monitors work area, supply inventory and equipment. Notifies Practice Manager, or designee, when supplies are low. Assists with unpacking, storing, and/or distribution of supplies upon delivery.
- Patient Satisfaction: Promotes patient satisfaction by providing excellent service by interacting in a friendly, helpful manner. Communicates potential patient satisfaction issues to the practice manager. Keeps informed regarding the practice's performance on patient satisfaction.
- Post Scan Procedures: Develops films of acceptable quality with minimum retakes. Completes proper forms and documents findings for physician review. Enters all required information into the database. Prints reports and distributes as per practice policy.
- Pre-Procedure Responsibilities: Obtains required demographic information from patient and verifies insurance coverage prior to initiating procedure. Completes pertinent patient history and accurately records data into medical record and/or radiology equipment.
- Radiology Procedures: Verifies the identity of the patient by asking the patient's full name and date of birth, which should be compared to the physician order. Assists the patient on and off the x-ray table as needed and assesses the immediate environment for potential safety concerns.
- May be involved in rooming patients including taking vitals and documentation in EPIC.
- Other duties and/or projects as assigned.
- Adheres to HMH Organizational competencies and standards of behavior.
Education, Knowledge, Skills and Abilities Required:
- High School diploma, general equivalency diploma (GED), and/or GED equivalent programs.
- Completion of an AMA approved school of Radiologic Technology.
- BLS certification.
Licenses and Certifications Required:
- NJ Registered Radiological Technologist License.
- Basic Cardiac Life Support Certificate.
Licenses and Certifications Preferred:
- American Registry of Radiologic Technologists Certification.

The Registered Nurse functions as a caregiver for a specific patient population in order to achieve desired outcomes. 1. Collects comprehensive data pertinent to the patient's health or the situation.
2. Performs age and disease specific assessment / data collection independently in a systematic manner focusing on physiologic, psychological and cognitive status.
3. Recognizes clinical and diagnostic status changes in patient status.
4. Identifies variables in patient/family education needs based on age, disease, and culturally specific learning differences.

1. Develops a plan of care that prescribes strategies and interventions to attain expected outcomes.
2. Formulates age and disease specific patient / family centered goals / outcomes based on assessment data.

1. Must have a current and valid Registered Nurse license to work at the assigned HMH location by date of hire.

The Cardiac Nurse Navigator, in collaboration with the cardiologists, APN's, and the entire healthcare team, oversees and supports the cardiac patient and their significant other(s). The Cardiac Nurse Navigator is a member of the healthcare team and is responsible for coordinating, communicating and facilitating the care of selected patients with primary cardiac care needs. The navigator is accountable for a designated case load determined by the careful daily selection of eligible patients. They assess, plan, and facilitate, with patients/families and healthcare professionals involved in the patient's care to meet treatment goals, and arrange for the appropriate next steps. Oversees Interfacility Coordination and handoff between acute & outpatient services.
Responsibilties:1. Participates in the collaboration with physicians, nursing staff, and interdisciplinary team in the assessment, planning, implementation and evaluation of care for selected patients and their families.
- All patients who are admitted for medical care will be screened for potential eligibility to the Cardiac Transitions of Care (TOC) program. All eligible patients will be enrolled.
- Meets directly with the patient/family to assess needs, based on assessment and prior evaluation from care coordinators/case management and develop an individualized needs assessment.
- Facilitates communication and coordination between members of the health care team and involves the patient/family in the decision making process, in order to minimize fragmentation of services, manage resources and remove barriers to the discharge plan of care.
- Develops a TOC plan, in collaboration with the patient/family, patient caregiver, patient support persons and healthcare team that will provide maximum benefit for each patient. In addition to aligning with patient quality metrics. Confirms the patient has a primary care provider, cardiology providers upon discharge and refers appropriately to a primary care provider and/or cardiologist if needed.
- Works collaboratively with all members of the multidisciplinary health care team and community partners for timely and appropriate transitions to the next appropriate level of care.
- Maintains current and up to date information of community resources and refers patients to those community resources which will enhance patient's life and clinical outcomes. Consults with other community agencies and committees to identify potential resources to support patients and their families. Will actively work to find community partners.
- Documents and communicates information to the Multidisciplinary Team in order to coordinate and maximize care. The Electronic Health Record will reflect the needs of the patient, any education needed based on the patient's medical history, coordination of follow-up care, and referral to community services.
- Provides patients and families with community resources and discharge care coordination options.
- Provides appropriate patient and family education regarding diagnosis, treatment, and self-care management and documents outcomes in the medical record.
- Ensures timely follow up appointments with appropriate care providers.
- Participates actively on appropriate workgroups, and/or meetings. Is a positive problem solver. Identifies and refers quality issues for review to the cardiac and transition of care team locally.
- Reassesses periodically and evaluates against care goals and the plan of care and, when indicated, the plan or goals are revised. Medical records reflect that each patient's discharge plan is re-assessed in response to changes in patient's needs and Social Determinants of Health.
- Completes all other necessary duties with attention to detail and in a timely manner.
2. Monitor readmission rates for Medicare and all payers, and implement needed performance improvement projects to improve scores in collaboration with the cardiac team.
3. Collaborates with the cardiac team to help ensure that ACC metrics and goals are met.
4. Other duties and/or projects as assigned.

Qualifications:Education, Knowledge, Skills and Abilities Required:
1. Graduate of an NLN/AACN accredited program in nursing.
2. Bachelor's Degree or equivalent years of cardiovascular experience.
3. Minimum 2 years of experience as a registered nurse.
4. Computer skills to include Google Docs and data entry.

6. Exceptional communication skills to enable communication and collaboration with physicians, patients, families, and ancillary staff.
