This role is an innovative approach to partnering with case managers and placement resources in Kansas to identify the best placement match for youth already in or newly entering state custody. You are responsible for identifying placement resources for children in state custody referred to by their case manager. As a member of the Kansas Foster Care team, you will work with 3 other team members and report to our Placement Finding Manager.



WHAT YOU WILL DO:




  • The Placement Finding Specialist receives information about youth by phone from case managers and assesses, in conjunction with the case manager, the youth's placement needs
  • Specialists then utilize available technology to conduct a search for placements that would match the youth's needs, discuss placement needs by phone with prospective placements and then provide placement options by phone and/or email to the youth's case manager
  • There is a strong emphasis on placing siblings together, maintaining youth in their community and minimizing the number of placement changes youth experience while in foster care
  • Referrals come in as either emergencies (placement must be found within four hours) or non-emergencies (anything beyond four hours). Placement Finding specialists must meet established benchmarks for each referral type

What You’ll Do

  • Work with the team to help monitor inventory levels and communicate status on a regular basis.
  • Ensure compliance with licensed guidelines and expectations.
  • Maintain outfitting tool and other merchandising tools, pin or boost products based on inventory and product launch plans

GENERAL SUMMARY AND PURPOSE:

SKILLS, KNOWLEDGE, EDUCATION AND EXPERIENCE:

A current RN license is required.

  • Multistate licenses must establish residency with the Idaho Board of Nursing within 60 days from hire.
  • All colleagues must provide licensure or proof of application in process for an Oregon RN license within 90 days of the hire date.

BSN strongly preferred. A minimum of two years of Registered Nurse experience is required. Previous leadership roles preferred. At hire, Basic Life Support certification from the American Heart Association or American Red Cross is required. (ACLS and/or PALS may be accepted in lieu of BLS) Other certifications may be required as identified in the SAHS Certification Crosswalk.

ESSENTIAL FUNCTIONS:

Coordinates with transport, PACU, and inpatient units for movement of physical beds to appropriate areas in a timely manner. Enters calls from outlying facilities into Forefront. Demonstrates ability to utilize Central Logic to search for physicians and use paging system including appropriate scripting for text paging. Communicates frequently with the emergency department to facilitate timely admissions. Prepares periodic reports regarding issues and concerns in the placement of patients. Serves on committees within and beyond Patient Care Services PRN.
