Job Summary
Prepares, assemble baked goods, cooks, seasons and portions food for patients/residents, staff, and visitors of the designated facility, preparing and serving food within guidelines of menu and dietary requirements. Adheres to food quality standards of appearance, taste, temperature and sanitation. Performs washing and cleaning duties to insure sanitation and cleanliness in cooking and serving areas. Works with many internal customers, requiring above average communication as well as excellent team work skills.
Displays ability to cook, function and thrive in a dynamic and changing culinary environment. Follows standardized recipes and Hazardous Analysis and Critical Control Point (HACCP) guidelines to prepare, store and use food and food products. Documents HACCP steps, temperatures and outcomes to assure the safest food possible while following production sheets for daily and weekly menus. Operates equipment such as, but not limited to, mixers, slicers, kettles, combination ovens, food grinders, food processors, ovens, stovetops, induction burners, grills, deep fat fryers, knives, dishwashers, garbage disposals, refrigerators and freezers.
Possesses ability to work independently when necessary. Possesses an understanding of food safety and sanitation processes. Exhibits understanding of basic math to appropriately measure alongside displaying proficiency in reading and understanding recipes. Demonstrates ability to be on feet for the majority of the day and walk far distances throughout the hospital.
Qualifications
High school diploma or equivalent preferred.
Possesses ability to read, write and follow oral and written directions. Ability to perform basic math. Minimum of six months of applicable experience preferred. On-the-job training will be provided.
Depending upon location of hire, Serv Safe may be required to obtain within a designated time frame. Additional endorsements to Serv Safe may be required depending upon location. Qualifications
High school graduate or general equivalency diploma (GED) preferred, but not required.
Minimum age of 16. Prior clinical or nursing assistant experience in long-term care preferred.
Required Certified Nursing Assistant (CNA) certification with the State Board of Nursing, or state certified CNA within four months of date into the position by being enrolled in a training program or in the process of obtaining certification through Endorsement. CNA training and certification required and provided by facility.
Satisfactorily completes yearly department specific competency testing and maintains a record of continuing education credits for each year. A minimum of 12 hours continuing education are required annually. Maintains all department specific required certifications.
When applicable, for applicable states, certified and registered with the Unlicensed Assistive Person (UAP) Registry.
Dependent on facility and required responsibilities, this role may classify as a Sanford Category II Driver requiring a motor

The Skilled Nursing Facility (SNF) at Champlain Valley Physicians Hospital is located on our 5th floor and accommodates 34 residents. Our facility serves a diverse range of patients, spanning from middle-aged adults to geriatric residents. We maintain a balance between long-term care and subacute care, striving for a ratio of 14 short-stay patients to 20 long-term residents. Our short-stay patients typically stay with us for 1 to 6 weeks on average, receiving specialized care for conditions such as sepsis, post-orthopedic surgery recovery, and wound care. For our subacute population, we provide a spectrum of skilled nursing and therapy services, including wound care with advanced techniques like wound vacuums, antibiotic therapy, and comprehensive physical, occupational, and speech therapy. Our long-term residents exhibit a wide range of needs, from those requiring minimal assistance with daily activities to others who rely completely on our care and support. Working with us offers a unique experience where healthcare professionals form meaningful relationships with patients. The Opportunity
- Up to $100k sign-on bonus for qualified candidates.
- Communicate and champion Vituity’s purpose, mission, vision, values, culture of brilliance, and strategy, ensuring widespread understanding and alignment.
- Assure compliance with all Vituity requirements and policies and communicate those requirements to all Vituity providers.
- Monitor current and future healthcare and economic trends. Assess their potential impact on the practice and local geographic region.
- Strategize and execute a comprehensive annual practice management plan that sets clear goals and objectives, continuously surpassing expectations and delivering value to patients, clients, providers, and the local community.
- Spearhead the local site's administrative management team, meticulously selecting team members and aligning their responsibilities to drive the achievement of practice goals.
- Demonstrate unwavering mastery of Vituity policies and fiduciary obligations, ensuring strict adherence from all providers.
- Exemplify Vituity's Partnership Principles by fostering open communication and transparent decision-making, such as conducting annual reviews of administrative stipends and scheduling preferences.
- Demonstrate a comprehensive understanding of hospital expectations and rigorously uphold compliance with all contract terms.
- Monitor operational and quality metrics and implement Vituity initiatives and operational programs to continually improve performance.
- Continuously assess and improve operational processes, leveraging technology and best practices to streamline workflows and increase efficiency.
- Monitor site financial performance and identify and create new areas for growth and revenue.
- Develop an expertise and understanding of the yearly budget, financial performance measures and monitoring systems, and billing and reimbursement issues / systems.
- Improve patient census and billing practice statistics to optimize reimbursement for the practice.
- Maintain awareness and interactions with payers such as significant IPA’s, Medical Groups, Foundations, ACO’s associated with the hospital / health system.
- Execute efficient recruitment, onboarding, and training processes for new providers, ensuring the practice is staffed with highly qualified professionals.
- Provide learning and development opportunities and mentoring to providers and staff to enhance their clinical acumen, leadership skills and overall professional growth.
- Evaluate the performance of physicians and PA / NPs in compliance with Vituity policies and guidelines.
- Monitor physician competencies with progressive improvement using appropriate metrics. Counsel, suspend, or remove staff from the schedule as necessary in compliance with Vituity polices / guidelines.
- Actively participate in contract negotiations in conjunction with the Regional Director.
- Develop, foster, and maintain productive and collaborative working relationships with hospital leadership, nursing staff and other healthcare team members.
- Ensure practice is appropriately represented and demonstrates their value through leadership roles (as applicable) and/or participation with hospital management, medical staff leadership, Medical Executive, Medical Staff and other hospital committees, and within the local community.
Required Experience and Competencies
- Licensed physician as a Medical Doctor (M.D.) or Doctor of Osteopathic Medicine (D.O.) degree from an accredited medical school and completion of residency through an accredited residency program required.
- Maintain membership and privileges on Hospital’s medical staff and comply with and abide by the bylaws, rules and regulations, and the policies and procedures of Hospital’s medical staff where services are being provided required.
- Physician Partnership status required.
- Five (5) years or more experience in a leadership role required.
