Job Description:
HSL provides enhanced housing with services to seniors living in its four affordable housing locations and at partner housing sites, with the goal of improving quality of life and supporting independent living. At its core, enhanced housing with services is a proactive approach wherein resident services staff regularly and actively reach out to each individual resident to engage with them around their health and wellness, identify areas of need/risk and provide intensive, individualized case management and support as needed and desired by the resident.
The Nurse Care Manager is a key member of the housing team working to support residents in living independently and safely for as long as possible by developing meaningful relationships with residents and providing support in a holistic way.
The Nurse Care Manager collaborates with team members to engage residents in wellness assessments and health education, connect them to needed services, support in managing health concerns, coordinate care for residents returning from hospital or rehabilitation stays, document all resident care tasks, and partner with community providers to promote overall well-being. This position is to work on-site at an affordable housing community in Chelsea. The position requires fluency in Spanish as the majority of the resident population is Spanish-speaking.

Position Responsibilities:
Partner with the wellness coordinator and the resident services team to provide comprehensive case management services to residents.
Provide regular preventative outreach to all residents to check in on their needs and overall health, and develop trusting relationships with residents and their families.
Conduct wellness assessments of residents to determine risk and needs. Actively follow up on all identified needs, including finding resources, making referrals,s and ensuring residents are actively engaged in services.
Evaluate resident medical concerns and support residents with decision-making regarding next steps, e.g., calling PCP, going to urgent care, going to the ED, or seeing a specialist.
Coordinate with primary care physicians and specialists, hospitals, mental health, and other community providers. Ensure effective communication around changes in status, transitions, and service utilization.
Active follow-up on all hospitalizations, rehab stays, and emergency room visits. Work with families, hospitals, rehabs, HSL Home Care, and/or VNA, ASAPs, and other providers to ensure safe discharges and ongoing services.
Follow up regularly with at-risk residents to support adherence to health and wellness-related activities, medication, and treatment plans.
Conduct and/or coordinate group and individual education sessions on health and wellness, including medication management.
Track residents with special needs, such as dementia and mental health, and make appropriate referrals.
Utilize collected data to identify, plan, schedule, and implement focused programs, such as falls prevention.
Support and educate housing staff members about common medical conditions and how to identify and communicate status changes.
Participate in resident services team meetings, provider meetings, and individual family meetings.

Qualifications:
RN and 1 year of experience or LPN and 3 years of experience required.
3 years of experience in aging services preferred, home health experience, and dementia care a plus.
Excellent triage and critical thinking skills are required, as well as the ability to handle difficult situations.
Must have compassion for and a desire to work with a senior population.
Excellent organizational and interpersonal skills, including the ability to manage multiple projects simultaneously, work efficiently, and proactively as part of a team.

Job Responsibilities
Interpret and implement medical prescriptions for speech-language pathology services, ensuring appropriateness for the home care environment
Conduct thorough assessments in the patient's home using standardized tools to evaluate communication, cognitive-linguistic function, voice, fluency, and swallowing abilities
Develop and implement individualized, evidence-based treatment plans that reflect both short- and long-term patient goals, with attention to the home environment and available resources
Adjust treatment strategies based on patient progress, caregiver input, and observed response to therapy, ensuring goals remain functional and achievable in the home setting
Document all assessments, interventions, patient progress, and caregiver interactions using electronic or paper records in compliance with agency policies and regulatory standards
Ensure documentation supports reimbursement criteria and meets all required timelines and confidentiality regulations, including HIPAA and agency-specific guidelines
Provide education and training to patients and caregivers on communication strategies, compensatory techniques, and safe swallowing practices within the context of daily home routines
Establish strong rapport and effective communication with patients, caregivers, and interdisciplinary team members to support coordinated care
Participate in case conferences, team meetings, and care planning discussions to align therapy goals with overall care plans and discharge readiness
Collaborate with care managers and vendors to recommend and coordinate the provision of durable medical equipment or assistive technology when indicated
Maintain professional licensure through ongoing continuing education, and stay current with clinical best practices relevant to home-based speech-language pathology
Assist in mentoring peers or evaluating competencies of other clinicians as needed, contributing to team development and quality of care
Position Qualifications
Minimum Required:
Master's degree in Speech-Language Pathology
Certificate of Clinical Competence (CCC) from ASHA, or current candidate in a Clinical Fellowship Year (CFY)
Current New York State license as a Speech-Language Pathologist
Current CPR certification

Previous clinical experience
Experience working with dysphagia and neurogenic communication disorders

Responsibilities

Department Overview
The workers of Electric Operations ensure the delivery of safe and reliable electric service to our customers. Electric Operations is responsible for every aspect of PG&E's electric distribution and transmission operations, including planning, engineering, maintenance and construction, asset management, business planning, restoration and emergency response.
Position Summary
This classification is represented by IBEW Local 1245, which has over 100 years of experience representing employees at PG&E. This job is subject to collective bargaining.
A coworker who has the qualifications of a Cable Splicer, is familiar with the underground system and has the experience and ability to analyze cable and equipment troubles quickly and to restore services promptly by replacing fuses, switching, disconnecting defective cables or apparatus, making repairs, etcetera. May be required to perform other underground work such us preparing cable racks, pulling in and racking cables, maintaining equipment in the underground system, etc. Shall install and maintain services, meters, and equipment including conduit and wiring up to the meter on customers' premises and shall handle trouble in such installations. Drives a truck and operates the associated equipment as assigned. Background of apprenticeship and experience must be such as to qualify an employee to perform these duties with skill and efficiency. Shall be familiar with Company's standards, procedures and safety regulations.
This position may be subject to Joint Apprentice Training Committee (JATC) review and approval. Provisional hires will be required to participate in targeted training to achieve journey-level knowledge of electrical technician roles and responsibilities.
This position is in the Bay Area region, and headquarters in Oakland subject to change based on business needs.
This position is a DOT (Department of Transportation) covered classification and is subject to random drug and/or alcohol screening.
PG&E is providing the hourly rate that the company in good faith believes it might pay for this position at the time of the job posting. This position is subject to collectively bargained hourly wage rates and increases. The starting wage rate for this position is $67.58 per hour. This wage rate is specific to the locality of the job. The wage rate is based on multiple factors, including, but not limited to, specific skills, education, licenses or certifications, experience, market value, geographic location, collective bargaining agreements, and internal equity.
Job Responsibilities
* Perform all necessary duties with skill and efficiency by performing maintenance on underground transmission and distribution lines.
* Perform work in connection with the construction and maintenance of underground manholes, vaults, splice boxes, duct lines and similar structures.
* Installs and maintains services, meters, and equipment including conduit and wiring up to the meter on customers' premises
* Maintains equipment in the underground system
* Prepares cable racks
* Pulls and racks underground cables and wires into and out of pipes, conduits and duct lines.
* Splices cables of any kind or voltage for the connection of cable, transformers, junction boxes, and other equipment in the underground or overhead systems or stations.
* Ensure effective customer and internal communication.
* Restore service to the public by installing / repairing services and all types of meters.
* Analyze cable and equipment trouble quickly and restore services promptly by replacing fuses, switching, disconnecting defective cable on apparatus efficiently.
* Identify the location of and extent to which underground structures such as man holes and vaults have been damaged and perform minor repairs safely on underground enclosures.
* Isolate Network grid secondary's and Network Protectors (NP's).
* Install and remove Recording Volt Meters (RVM's) for power quality research.
* Physically demanding work that may include lifting, climbing, as well as going into confined spaces.
* Drive trucks, vans, and equipment and operate the associated equipment as assigned.
* May work with crew or independently. May oversee a crew and its equipment engaged in pulling underground cables and wires of all sizes, types, lengths and number of conductors into and out of pipes, conduits and duct lines and other necessary work in conjunction with pulling of cables.
* Support and maintain a working relationship with other departments which include, Distribution Operators, Service Op drive erectors, Engineering, Estimating and Construction to improve processes and ensure a high level of customer service.
* Working knowledge of applicable safety rules, programs and safe work practices.
* Expected to learn and follow safety and work procedures.
* Working knowledge of available mobile technical software.
Qualifications
Minimum:
* CA CDL Class A Permit, or equivalent, subject to 150% driver/vehicle ratio
* Knowledge of procurement strategies and processes and relevant regulatory framework as needed to perform at the job level
* Must be at least 18 years of age
* Must possess a High School diploma or GED
* Completion of formal union sanctioned Cable Splicer apprenticeship
* Must possess an IBEW journey- level card for a Cable Splicer or State or JATC certification
* Must be able to lift 90 lbs
* Able to wear company provided Personal Protective Equipment (PPE)
* Able to work and drive in all types of weather and road conditions
* Able to work extended hours, nights, weekends and holidays
* Able to travel to job sites as necessary
* Ability to work shift schedule (evening)
* Ability to complete and obtain a 40 hour Switchman Training Certification/Card
* Must pass Post-Offer Physical Assessment as a requirement of the pre-employment process
* If hired as Provisional, must pass the provisional training plan within one year from hire date

Job highlights
The visit, which lasts about 45 minutes, allows for time to connect one-on-one and answer health questions. It includes a medication and medical history review, a physical evaluation, and, if ordered by the person's health plan, additional diagnostic tests (i.e., diabetic eye exams, spirometry, etc.). You'll use an iPad and an intuitive clinical workflow for each evaluation and are not required to prescribe medicine, order lab tests or alter people's current treatment regimen.
