Core Responsibilities:

1.     Oversees nursing activities in the Med/Surg and Swing Bed areas ensuring the following:

a.      The Mission, Vision, and Values of CMC are incorporated into all aspects of patient care and nursing

b.     Nurses understand and focus their care around Virginia Henderson’s theory of nursing practice, and that nurses and C.N.A.’s practice a Team Nursing Model of patient care.

c.      Current standards of nursing practice are followed at CMC and the Acute Care Manager is actively involved in reviewing nursing standard updates and incorporating them into CMC’s practice

d.     Coordinates daily scheduling and staffing of the Med/Surg and Swing Bed units, assuring the patient care delivered supports the mission and values of CMC. 

e.      Assesses the needs of the patient population, plans for those needs, intervenes based on the assessment and planning process and evaluates the outcomes and applies the clinical discipline of professional nursing through the nursing process

f.       Assures Nurses perform technical tasks and procedures safely and accurately

g.     Assists with the organization of ongoing communication among all health care providers and staff. Develops collaborative relationships to enhance quality of service

h.     Maintains effective communications

i.       Provides for optimal patient/resident safety

j.       Respects patient's rights to privacy and dignity

2.     Accountability/Responsibilities include the following:

a.     Maintains 24/7 accountability for the Emergency, Med/Surg and Swing Bed Units and recognizes need to respond to the facility (during off hours) in person as needed for increased census, acuity or mentoring. 

b.     Coverage for CNO when out of the office due to illness, PTO, or other absence. 

c.      Must be physically and mentally able to work extended hours when necessary.

d.     Attends meetings as required and relays information back to the Med/Surg and Swing Bed nursing teams 

e.     Reports situations to the CNO as appropriate

f.       Acute Care Mentorship

• Emergency Services including Trauma, STEMI, Stroke, and etc.
•  Hourly Rounding
• Acuity rating/staff ratios
• Bedside Report
• Bedside Charting
• Updated Communication Boards
• Weights/Vital signs per order
• edside care/Shift coverage as needed for call-ins, increased census, etc.

g.     Monthly

• Policy/Procedure review/update
• Schedule
• KPI for med scanning
• Quality Measures (Pain scores, med scanning, etc.)
• Annual evaluations of RN’s, C.N.A.s and Techs
• Regularly scheduled (at least quarterly) Staff Meetings.

3.     Demonstrates professionalism and professional nursing practice through:

a.      Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies

The Lead Physical Therapist reports to the Rehab Manager or Rehab Director. Under general supervision, the Lead Physical Therapist develops, supervises and provides optimal patient care services as referred by physician for Physical Therapy, based on population-specific needs utilizing a developmental and patient/family directed approach. This role provides daily operational supervision to assigned staff, Physical Therapists, Volunteers, Therapy Technicians, and Physical Therapy Assistants. This position functions as a mentor and expert in specific clinical area and possesses advanced clinical skills. Provides clinical education experiences for physical therapy students as assigned. The Lead Therapist may work in both outpatient and inpatient services, all units throughout the hospital based on assigned caseload.

Education:

• Bachelor’s, Master’s or Doctorate degree in Physical Therapy from an accredited program.

Experience:

The health system operates two inpatient facilities with 80 beds and 95 beds respectively, providing comprehensive hospital medicine services to the local community. Both locations maintain modern medical equipment and utilize Epic EMR systems to support quality patient care and clinical documentation. The facilities operate with collaborative hospitalist teams that provide mutual support during high admission periods to ensure optimal patient care delivery.

The hospitalist will provide comprehensive night shift coverage from 7 PM to 7 AM at two health system locations, managing adult inpatient medical cases with census ranging from 12-15 patients per day plus intermittent admissions and discharges. The clinician must be board certified or board eligible, hold an active Virginia license, and maintain BLS, ACLS, and DEA certifications while being open to working at both an 80-bed and 95-bed facility. Responsibilities include conducting physical examinations, ordering laboratory and diagnostic tests, writing orders, managing admissions and discharges, diagnosing injuries and illnesses, prescribing medications, providing patient education and counseling, and managing common conditions such as infectious illnesses, respiratory issues, flu, and dehydration using Epic EMR documentation.
