Job Summary and Qualifications
The Progressive Care Registered Nurse coordinates and delivers high quality, patient-centered care. In collaboration with medical providers, the RN provides pre-operative and post-operative nursing care. The RN serves as an advocate for patients to support an unparalleled patient experience.

EastPointe Hospital is a private, free-standing psychiatric hospital for adults located in Daphne, Ala. EastPointe's clinical and medical professionals assess and evaluate adults in psychiatric crisis. Treatment focuses on stabilizing psychiatric symptoms and preventing psychiatric commitment. Adults admitted to EastPointe will have exhibited life-threatening, destructive or disabling behavior or symptoms. Each patient completes a psychiatric diagnostic evaluation and receives 24-hour clinical care for an estimated three-to-seven-day stay. The goal is for each patient to return to home or to the least restrictive level of care.
ResponsibilitiesPrimary Job Functions:
Office professional responsibilities
- Reviews documentation for any improper corrections, missing signatures, dates, times, etc. and monitors for completeness.
- Returns incomplete documentation to the program supervisor.
- Keys billing information on a daily basis from the DSR into the CSM system appropriately.
- Files documentation in the appropriate section of the consumers' records as needed.
- Completes administrative review when needed.
- Performs basic clerical functions.
- Completes census maintenance on a daily basis accurately and completely, including the \"head in the bed check\" completed at midnight.
- Completes Compliance Checklists on a daily basis.
- Makes copies and faxes information as needed and checks fax regularly.
- Assembles charts and checks records for availability of forms.
- Notifies nurses of e-signature changes.
- Upon admission, accesses and prints medication lists and medical checklists for active consumers and places on charts.
- Notifies applicable staff of emergency situations in an appropriate and timely manner.
- Ensures the cleanliness and orderliness of the units.
- Assists with pulling and locating charts and with other requests.
- Maintains the chart tracking (charge-out) system to track the location of each record.
- Ensures all charts are accounted for and filed on the chart rack at the end of each shift and all signatures have been obtained on progress notes/orders. Restraints and Seclusion observation sheets.
- Distributes office supply orders for the unit and places supply orders with the administrative assistant as needed.
- Accesses labs on a daily basis, prints reports and places on appropriate charts for review.
- Ensures unit/nursing supplies are maintained at appropriate levels.
- Keeps Dietary informed of admissions and prints dietary tickets as needed.
- Ensures that all unit copiers are working appropriately and reports issues to IT department.
- Assist with facilitating hospital phone time in professional, organized, and quiet manner.
- Serve as the back-up for the Admission Support Specialist if necessary.
- Complete \"head in the bed checks\" in the event Admission Support Specialist is unavailable.
- Support Front Desk Support Specialist as needed.
Other duties that may be performed depending on facility
- Pulls mail.
- Runs census.
- Checks for weekend admissions.
- Checks all room assignments.
- Discharges packets for receiving facilities including to ICF units to include Psychiatric evaluation, H&P, Labs and X-rays.

- Sitting can be in frequent bursts up to 10 minutes. Extended sitting can be up to, but limited to 1 hour.
- While filing in the medical records department, the employee may bend forward squat/lunge stoop or crouch to access the levels that are at floor waist level. Forward bending while sitting or crouching to access safe in office may be needed.
- Forward reaching, reaching out to front, and overhead reaching with up to 5 pounds to 15 is performed when filing records and charts.
- Forward reaching, overhead reaching and reaching down while filing medical record that can range up to 15 pounds.
- Lifting various ranges from the floor to overhead throughout the day with up to 15 pounds (medical records and supplies).
- Frequent pushing/pulling with a force up to 25 pounds (medical record cart) and doors throughout the facility (5 pounds).
- Step ladder climbing frequently to access higher shelves for filing and stocking/retrieving supplies.
- Strong grasp/pinch grip needed to use keys in doors and to handle medical records.
- Fine manipulation/dexterity to use computer, write and handle medical records paperwork.
- Must be able to stand for long periods of time, continuously bend, stoop, kneel, crouch, reach and lift at least 10 pounds.

About the Facility
- California Medical Facility (CMF) California Medical Facility (CMF) was established in 1955 by the California Legislature to provide a centrally located facility to meet the medical, psychiatric, and dental health care needs of male felons incarcerated within the California Department of Corrections and Rehabilitation (CDCR).
CMF is a medium security medical facility located in the city of Vacaville in Solano County.
Vacaville is located 37 miles southwest of Sacramento and 65 miles northeast of San Francisco.
The majority of inmate-patients at CMF reside in an outpatient setting and receive health care in an outpatient clinic.
Patients who require additional nursing care may be housed in the Outpatient Housing Unit (OHU).
Patients who require 24-hour nursing care may be housed in a licensed medical correctional treatment center (CTC).
CMF also operates a 400-bed inpatient psychiatric CTC, a 50 bed mental health crisis bed CTC, and the first licensed prison hospice in the United States.
Health care services provided at CMF include primary care, chronic care, and specialty care clinics; radiology, occupational and physical therapy, pharmacy services, laboratory, respiratory, inpatient and outpatient mental health treatment, end-of-life/palliative care services, services for the visually and/or hearing impaired, and services for those with other acquired or developmental disabilities.

Must be able to provide clinical face-to-face interviews at cell fronts in inmate housing units.

Document all care provided to the patient-inmate/ward, including face-to-face contacts and IDTT meetings.
Monitor inmate/ward progress using Subjective, Objective, Assessment, Plan and Education (SOAPE) formatted progress notes.
Effectively applies community standard of practice for psychiatric treatment modalities and ethical standards.
Perform mental status exam (MSE) in face-to-face clinical interviews.
Identify the relevant signs and symptoms of medical/psychological disorders in order to comprehensively diagnose, assess and manage the presenting symptomatology.
Examine prior mental health records, institutional chromos, probation reports, court records and other available documents to assist in the formulation of patient's diagnosis and aid in the development of the inmate's/ward's treatment plans.
Review, examine and diagnose psychiatric patients of all ages and patient records and files in mental health program to determine progress, effectiveness and/or appropriateness of treatment services.
Consult, as necessary, with supervising psychiatrist on unusual, complex, or serious cases, or present such cases to a clinical conference for advice or decision.
Provide clinical consultation to psychiatric staff on unusual, complex or serious problems and cases where technical expertise is needed.
Prescribe medication as deemed appropriate for treatment and utilize the CDCR Correctional Formulary or obtain non-formulary approval.
Interpret laboratory results and make clinically appropriate referrals to medical services, as appropriate.
Determine type of psychiatric and general medical treatment needed.
Prescribe changes in treatment when indicated.

We provide care for cardiac patients who have undergone a cardiac intervention from the cath lab which includes stent placement, pacemaker placement, and diagnostic angiograms.
We also care for the patients who have chosen to undergo Watchman and Mitraclip procedures.
We manage patients with congestive heart failure, NSTEMI, STEMI, pre-CABG, and peripheral vascular occlusions.
We do have patients with cardiac drips and also take medical-surgical overflow patients from the ER.

Run errands for the unit as required.
Assist Licensed personnel in providing a safe and comfortable environment at all times; replenish patient room with water, linen, and towels, as appropriate.
Perform unit-specific patient care tasks in accordance with the skills checklist and documents on the flowsheet.
Identify obvious changes in the patient's condition and take responsibility for reporting those changes promptly to the nurse in charge.
Assist with the ordering and maintenance of stock items according to established inventory when needed.
Keeps unit supplies in an organized and orderly fashion.
Respond appropriately to emergencies.
Communicate effectively with other team members during and between shifts.
Responds to phone calls and patient call lights.
Appropriately adapt assigned patient assessment, treatment, and/or care methods to accommodate the unique physical, psychosocial, cultural, age-specific, and other developmental needs of each patient served.

- Hamilton County Supportive Housing Team Leader Experience / Knowledge: Minimum of two years of related work experience Education / License: High School Diploma and 10 years' experience in direct care or a Bachelor's Degree in health related field of counseling, psychology, social work, sociology or a behavioral science field with course work or experience in the area of cultural diversity, human development, etiology and treatment of mental illness, alcohol and drug abuse, physical and sexual abuse, suicide, and intellectual and developmental disabilities.
