The Care Manager/"Designated Care Manager" is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living/long term care and reminiscence neighborhoods. The Care Manager provides hands on care and physical and emotional support as outlined in each resident's Individualized Service Plan (ISP) while maintaining a safe and comfortable home like environment. - Observe, report, and document symptoms and conditions of residents for changes in condition such as skin, behavior, alertness, weight, diet, and participation in activities.

- Inform supervisor of any resident changes in condition.

- Greet guests, family members, residents, and team members.
- Answer, direct and/or respond to phone calls in a timely, courteous, and professional manner.
- Assist with continence management and dispose of all continence products properly to ensure sanitation of resident suite and community restrooms using standard care procedures.
- Communicate with families and is a resource as needed.
- Help residents maintain independence and physical safety and promote dignity of each resident adhering to the standards of Resident Rights and Sunrise Principles of Service.
- Participate/lead and assist residents with activities of daily living (ADLs) and Invite, Encourage, and Assist (IEA) residents to participate in activities as indicated on the ISP.

- Practice safety procedures at all times including Personal Protective Equipment (PPE), fire extinguishers, Safety Data Sheets (SDS), and Lockout Tagout procedures.
- Report all accidents/incidents immediately.
- Reports all unsafe and hazardous conditions/equipment immediately.
- Ensure any cords, carts, equipment, and other hazards are always kept out of the way, not blocking exits, and in compliance with fire codes.
- Understand and practice the proper method of attending to and disposing of, and the possibility of exposure to, blood borne pathogens, bodily fluids, infectious waste, sharp sticks, and hazardous materials.
- Report occupational exposures to blood, body fluids, infectious materials, sharp sticks, and hazardous chemicals immediately.
- Ensure oxygen tanks are stored safely, exchanged when empty and monitored to make sure liters of oxygen are at prescribed levels.
- Maintain and clean resident's room as assigned including making beds, emptying trash, putting clothes away appropriately, spot sweeping, and mopping as needed.
- Wash resident's laundry as noted in the ISP and as needed.
- Wash and fold dining room linens and napkins.
- Complete assigned housekeeping tasks.
- Maintain common areas in a clean and tidy manner at all times.

Join our team as a Medical Assistant at MOOV Health & Wellness, where we are focused on outpacing aging through a physician-guided, personalized approach to wellness. In this role, you will serve as a concierge for clients as well as being primarily responsible for supporting expert clinical care delivery for patients. The Opportunity
- Triage, vital, and room patients.
- Facilitate patient flow and ensure smooth running of the back office.
- Prepare patients for physician/advanced provider evaluation. Collect clinical intake information, which includes medical history, medications, capturing vital signs and allergies, and gathering information relative to the presenting illness or injury.
- Assist physician/advanced provider in evaluation and treatment of the patient.
- Record patient interview, history, and other relevant information into electronic medical record (EMR). Scan all patient clinical encounter documents into EMR and label appropriately per protocol as needed.
- Administer medications/injections under the supervision of the physician or advanced provider.
- Perform ordered procedures – EKG, visual acuity, height, weight, injections, venipuncture specimen collection, tray set-ups, assist physician/advanced provider as needed during procedures.
- Collect and perform Point-of-Care testing, including influenza, Strep A, Covid-19, urine dipstick, urine pregnancy, mononucleosis, RSV, etc.
- Review visit summaries and home care instructions with patients including prescriptions provided and work or school notes as well as follow-up care.
- Provide after-visit follow-up phone calls to all patients and relay any diagnostic testing results.
- Provide navigational care for patients including coordinating referrals, following up on medications, ordering from nearby pharmacies.
- Provide accurate, legal, and ethical documentation at all times.
- Comply with federal and local laws in ensuring patient privacy and confidentiality.
- Maintain safe, secure, and healthy work environment by establishing and following standards and procedures.

Required Experience and Competencies
- High school diploma or GED required.
- Current Medical Assistant certification from an accredited institution unless otherwise specified by the individual State’s requirements that allow on-the-job training in lieu of certification required.
- Proof of BLS certification is required for on-the-job training in lieu of certification required.
- Healthcare experience in an urgent care or primary care setting preferred.

* Ensures equipment is fully operational before each use by checking calibration and operating deficiencies.
* Completes all studies in RIS; ensures images are oriented, marked, labeled and notated correctly before sending to PACS; prints CDs as needed.
* Digitizes comparative studies when necessary and completes preliminary sheets when applicable.
* Maintains documentation required to meet ACR Accreditation standards.
* Demonstrates knowledge in completing and documenting ACR/MQSA QC tests including Phantom, Visual Checklist, Repeat Analysis, Compression Pressure, Compression Thickness, Flat Field Test, Calibration and DICOM Printer Test.
Operations
* Restocks linens and supplies and maintains Mammography Department in clean and orderly condition.
* Transports and positions patients and assists them on and off the procedure table.
* Performs patient charging activities on the day services are rendered.
* Applies shielding.
* Prepares and maintains patient follow-up data.
* Willingly provides knowledge and assistance to others as needed and as time allows.
* Performs clerical and support duties to ensure continuity and quality of services.
* Keeps supervisor informed and promptly reports problems or potential concerns.
* Maintains individual productivity equal to other coworkers.
* Familiar with Mammography Tracking System.
* Maintains Joint Commission and regulatory standards.
* Familiar with FDA/MQSA EQUIP program.
* Any other duties as assigned.
Required Minimum Education
* Graduate of an AMA-approved School of Radiology Technology - Required
* Currently ARRT registered - Required
Required Minimum License(s) and Certification(s)
(All required upon hire unless otherwise stated)
* ARRT Mammography
* ARRT Radiography
* Basic Life Support (BLS) through American Heart Association
Required Minimum Experience
* Minimum 1 year experience as a Rad Tech - Required
* Minimum 1 year experience in Mammography - Preferred
