Responsibilities
Performs all responsibilities and duties required by unit as specific to the scope of service. Implements procedures and theories related to the specific area of practice. Maintains up-to-date and accurate documentation of nursing care provided to assure the integration of information for use by the healthcare team.Completes initial assessment upon patient admission and develops appropriate plan of care in accordance with unit standards and that is based on patient and family needs. Implements plan of care, nursing interventions, and patient care procedures. Modifies plan of care based upon continuous evaluation.Takes responsibility for patient and employee safety by reporting and/or correcting safety hazards and incidents.Communicates with others directly and in private when necessary to resolve issues. Compensation and Benefits
The compensation range for this position is $29.04per hour - $49.95per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked and compensation model. General Summary of Position
Serves as a member of the Case Management Team and applies RN clinical expertise and medical appropriateness to care coordination and discharge planning. Facilitates the delivery of quality cost effective patient-centered care from pre-admission through post-discharge timeframe. Ensures the care is designed to meet individualized patient outcomes. Monitors the care and services delivered to selected patient populations during the acute hospital stay promotes effective case management and utilization of resources and works to achieve optimal clinical and resource outcomes for the acute and post-hospital phases of care.
Primary Duties and Responsibilities
- Contributes to the achievement of established department goals and objectives and adheres to department policies procedures quality standards and safety standards. Complies with governmental and accreditation regulations.
- Collaborates with the multidisciplinary health care team to develop and coordinate the plan of care.
- Communicates with patient family and/or significant other health care team external case manager community resources and facility to address appropriate issues and patient/family goals.
- Demonstrates the ability to develop a plan of care that addresses needs across the continuum; have an intervention for problems identified; develop long- and short-term goals with specific time frames for resolution; identify specific services to be provided in the care plan; include the family/care-giver in the plan of care; and show life planning contingencies such as power of attorney and/or advance directives.
- Evaluates and documents the patient's response to the plan of care and achievement of outcomes. Makes recommendations for modifications to the plan of care as indicated. Adheres to all policies and procedures regarding documentation and confidentiality of information.
- Maintains knowledge of regulatory agencies' requirements necessary criteria for admission to various care settings and Medicare's/Medicaid's reimbursement methods for different levels of care.
- Manages a caseload of patients. Identifies essential resources needed to implement the plan of care.
- Manages own professional growth in the area of managed care care management other health care financial trends clinical practice and research.
- Manages patient care according to multidisciplinary plan of care and/or managed care contracts by directing decision making and identifying and managing barriers that impact on patient care outcomes.
- Participates in Performance and Service Improvement teams. Assists in program evaluation through customer service surveys LOS data analysis charge/discharge data comparison to state averages and best practice/benchmark data.
- Performs a comprehensive assessment in collaboration with interdisciplinary team to identify patient-specific problems and needs related to diagnosis treatment including psychosocial and financial concerns as well as medical.
Minimal Qualifications
Education
- Associate's degree in Nursing (ADN) required
- Bachelor's degree in Nursing (BSN) preferred
Experience
- Minimum of 2 years clinical experience in an acute care hospital setting required
- 1-2 years case management experience preferred
Licenses and Certifications
- RN - Registered Nurse - State Licensure and/or Compact State Licensure in the District of Columbia or Maryland depending on work location required
- CCM - Certified Case Manager preferred
Knowledge Skills and Abilities
- Ability to use computer to enter and retrieve data.
- Working knowledge of Microsoft Word Excel and PowerPoint applications.
- Effective verbal and written communication skills.
- Must be able to run and analyze departmental productivity reports.

Summary:
Under the direct supervision of a Registered Nurse/licensed Nurse, this Job will assist with routine and repetitive patient care activities in a nursing unit. Responsibilities:

Assist Licensed personnel in providing a safe and comfortable environment at all times; replenish patient room with water, linen, and towels, as appropriate.
Perform unit-specific patient care tasks in accordance with the skills checklist and documents on the flowsheet.
Identify obvious changes in the patient's condition and take responsibility for reporting those changes promptly to the nurse in charge.
Assist with the ordering and maintenance of stock items according to established inventory when needed.
Keeps unit supplies in an organized and orderly fashion.
Respond appropriately to emergencies.
Communicate effectively with other team members during and between shifts.
Responds to phone calls and patient call lights.
Appropriately adapt assigned patient assessment, treatment, and/or care methods to accommodate the unique physical, psychosocial, cultural, age-specific, and other developmental needs of each patient served.
Demonstrates competence to perform assigned patient care responsibilities in a manner that meets the population-specific and developmental needs of patients served by the department.

The hourly rate for this position is:
$23.20 - $40.60Benefits:
Except as otherwise required by state law, CarMax Associates are entitled to the following paid sick, vacation, and holiday time.
Associates that are considered full-time hourly or commission/incentive eligible:
- To earn up to 48 hours of sick time per year accrued on a per pay period basis and between 80 hours and 200 hours per year of vacation time after a 90 day waiting period depending on years of continuous service with the Company.
- For 8 hours of pay for each of a total of 6 paid scheduled holidays per year plus 1 floating holiday. If such an Associate does work on a scheduled holiday due to business need, they are eligible for Holiday Premium Pay.
Associates considered full-time salaried are entitled to paid time away with no specified limit as needed for sick, vacation, bereavement, jury duty, holidays, floating holiday, etc. subject to manager approval.

Under general supervision, the State Inspection Technician performs State vehicle safety inspections tests per state and/or federal regulations.

Principle Duties and Responsibilities:
- Perform state inspections using appropriate equipment and adhering to prescribed rules, standards and procedures
- Ensure that all vehicles meet CarMax Final Quality Standards.
- Provide outstanding customer service at all times.
- Maintain, repair, and clean special equipment
Job Specifications:
- Read, interpret and transcribe data in order to maintain accurate records.
- 2 Year automotive trade school degree or has minimum 2 years automotive experience
- Requires appropriate state inspection certification
- Work with and through associates of other departments within the store.
- Perform additional duties in a high-energy, fast-paced working environment as requested by supervisor
- Possess above attention to detail and demonstrate manual dexterity in both arms and hands
- Speak and listen to customers, associates and managers in person, on the phone, and/or on the radio.
- Lift objects that weigh as much as 50 lbs.
- Comply with CarMax, O.S.H.A., and EPA policies
Working Conditions:
- Combination of both indoor and outdoor environment; May include working at times in noisy and/or inclement weather conditions.
- May require walking or standing for an extended time.
