Position Overview

We are seeking an experienced and compassionate Pediatric Nurse Practitioner to join a highly respected hospice and palliative care program supporting pediatric patients and their families. This role has two primary focuses: providing NP-level hospice care to enrolled pediatric patients and supporting a specialist pediatric palliative care consultation service.



The vast majority of visits will take place in the patient’s home, with occasional visits to hospitalized pediatric patients as needed. Some visits may also be conducted via telehealth based on clinical appropriateness and patient needs.



This role is ideal for a clinician with strong skills in advanced symptom management, family-centered communication, goals-of-care discussions, hospice eligibility review, and interdisciplinary teamwork.



Key Responsibilities

• Provide comprehensive palliative and hospice assessments, exams, care plans, and interventions

• Conduct face-to-face hospice eligibility evaluations and reassessments

• Manage urgent and emergent medical needs for pediatric hospice patients

• Collaborate closely with physicians and interdisciplinary team members

• Lead or participate in family meetings and care goal discussions

• Support and educate hospice team members providing pediatric care

• Contribute to pediatric hospice education initiatives and curriculum development

• Participate in quality improvement projects as needed

• Maintain timely, accurate, compliant documentation

• Provide responsive and professional communication with colleagues and referral sources



Qualifications

• Master’s Degree in Nursing and completion of an accredited Nurse Practitioner Program required

• New York State RN and Nurse Practitioner licensure required

• Minimum 1 year of NP experience preferred

• Prior hospice and/or palliative care experience preferred

• Hospice & Palliative Care Certification strongly encouraged by year two of employment

• CPR-BLS required

Valid NYS Driver’s License required; car required

• Strong assessment and independent problem-solving abilities

• Excellent communication, listening, and clinical judgment skills

• Ability to work collaboratively within an interdisciplinary care model

• Ability to work compassionately with families of diverse cultural and socioeconomic backgrounds

Compensation & Incentives

Salary Range:  $115,000 – $140,000

Compensation is based on education, experience, nursing certifications, bilingual skills, and departmental needs

The care plan advanced by the NP Provider aims to prevent or mitigate illness burden for the patient and family.

Each encounter comports with best practices of specialist palliative care, including history-taking, appropriate examination, formulation and care planning, treatment recommendation or implementation, communication, and documentation.

  • In the role as hospice NP, you will coordinate with the team physician and manager and will be responsible for face-to-face evaluations for hospice eligibility, and the assessment and management of hospice patients in need of exigent medical care
  • Contribute to the training of the hospice team, providing support for the interdisciplinary team, as needed
  • Help build a teaching curriculum on pediatric aspects of hospice care that will be of use to our hospice workers who are engaged in pediatric care through cross coverage for when young adolescents are incorporated into adult hospice teams
  • In providing both direct care and support to other clinicians, the NP will use an evidence-based approach to management and to the use of materials and policies/procedures
  • Other activities related to quality improvement projects may also be required, as appropriate

• Master’s degree in nursing and graduate of an approved Nurse Practitioner Program required.
• Minimum of 1-year clinical experience as a NP provider preferred.
• Prior experience working as a member of a specialist-level palliative care team or working in a hospice environment preferred.

We are currently seeking an experienced:

Palliative Care RN

Per Diem

(Harnett County and surrounding areas)

JOB SUMMARY:

- Review referrals
- Provides expert, individualized coordination of care to patients and families
- Completes a comprehensive physical and psychosocial assessment of the patient and family, to identify need and areas for intervention
- Ensures appropriate referrals to specialists when indicated and support adherence to treatment plan, including physician visits for evaluation and on-going management.
- Assists patients in accessing appropriate services and resources in the community to improve their overall health and well-being.
- Assist Nurse Practitioners in day to day program tasks and tracking palliative referrals.
- Alleviate pain, fatigue, nausea, and other distressing symptoms.
- Accurately document patient assessments, interventions, responses, needs, goals, and teaching in the EHR.

JOB REQUIREMENTS:

- Requires current RN licensure within state of branch location and three years of clinical nursing experience, preferably in; Home Care, Hospice or Skilled Nursing facility setting.
- Successfully completed nursing education from an approved school of nursing.
- Must be able to independently organize work, communicate patient needs, and provide patient education and coordinate patient visit schedule(s).
- Must accept travel and working conditions as assigned.
- Must possess effective communication skills (oral and written), as well as, CPR certification. Minimum of twelve hours of continuing education in a twelve-month period.
- Must have a valid driver's license.
