to join our team in our Central Pennsylvania Senior Living office. This office services our geriatric clients on a per visit basis in senior living communities throughout Dauphin and Cumberland County.

One year prior clinical experience as a licensed Practical Nurse is required.

As a home care nurse, you will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to receive the medical care required to remain at home.

Licensed Practical Nurse (LPN) Responsibilities:

- Making home visits to clients living in designated territories.
- Performing assigned duties, including administration of medication, wound care, treatments, and procedures.
- Monitoring clients' conditions; reporting changes to Clinical Manager and Client Services Manager.
- Following up with, executing, and properly documenting doctors' orders.
- Performing client assessments as necessary.
- Case management and coordination.
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing electronic medical records on a state-of-the-art touch pad tablet.

Qualifications of a Licensed Practical Nurse (LPN):

- A current license as a Licensed Practice Nurse in the state of Pennsylvania.
- A minimum of one year of recent, verifiable experience as an LPN.
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
- Prior home care experience strongly preferred, but not required.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
- Ability to travel to cases as assigned.

Job Summary and Qualifications 

As a Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) at HCA Healthcare, you’ll play a vital role in delivering compassionate, personalized care to patients in the comfort of their homes. In collaboration with the Registered Nurse and care team, you’ll help manage medical needs and promote their independence – caring for patients like family, one home visit at a time. 

Your responsibilities will include:

  • Providing nursing care in the home, including wound care, medication administration, and support during procedures 
  • Documenting assessments, interventions, and outcomes clearly and thoroughly to support safe, continuous care 
  • Monitoring patient status and alerting the care team of any changes to ensure timely, coordinated responses 
  • Educating patients and families to support medication adherence, strengthen understanding of health conditions, and promote greater independence at home 
  • Collaborating closely with RNs, providers, therapists, and care coordinators to deliver high-quality, personalized care in the home setting
 What qualifications you will need: 
  • Graduate of an accredited LPN/LVN program

  • Minimum of one year experience as an LPN/LVN in a clinical care setting
  • Home care experience preferred
  • Must be licensed as an LPN/LVN in good standing in the state(s) where they will be providing services
  • Meets all additional requirements for hire including state specific requirements
  • Current BCLS prior to delivering patient care
  • Reliable transportation and proof of valid automobile liability insurance

Position Purpose:

Join Trinity Health At Home as a Home Care Occupational Therapist ( OT ) and make a meaningful impact by delivering compassionate, high-quality therapy services to clients in their homes. You’ll help individuals regain independence and improve their quality of life through personalized care plans and therapeutic interventions.

What You Will Do:

  • Assess clients’ functional status and develop individualized treatment plans in collaboration with physicians.

  • Deliver preventive, therapeutic, and rehabilitative services in accordance with the plan of care.

  • Educate clients and families on safety, health promotion, and use of prosthetic/orthotic devices.

  • Supervise Certified Occupational Therapy Assistants (COTAs) and Home Health Aides (HHAs).

  • Coordinate care with interdisciplinary teams and participate in case conferences.

  • Document all care activities accurately using point-of-care systems.

Minimum Qualifications:

  • Graduate of an accredited Occupational Therapy program.

  • NBCOT certification and state licensure/registration.

  • Minimum one year of experience or completion of a 6–8 week preceptorship.

  • Valid driver’s license and reliable transportation.

  • Home health experience preferred.

  • Commitment to ethical standards and continuous learning.

Position Purpose


Trinity Health At Home is seeking a compassionate, organized, and mission‑driven Home Care Coordinator to support patients transitioning from the acute care setting to home-based services. In this vital role, you will facilitate timely and efficient referrals to home care and hospice, educate clinical teams on available services, and collaborate across the healthcare continuum to ensure patients receive the highest standard of care. This position also contributes to business development and new program initiatives that support our commitment to exceptional patient-centered service.

What You Will Do
  • Serve as the primary liaison between hospital care teams, patients, and Trinity Health At Home to coordinate safe and seamless discharges.

  • Maintain daily communication with case managers, discharge planners, and acute care units to follow up on referrals promptly.

  • Conduct initial assessments of home care needs, review hospital medical records, and prepare documentation according to agency policies.

  • Coordinate services based on admission criteria, patient preferences, and clinical needs, securing physician orders in accordance with regulatory requirements.

  • Provide education to patients, families, physicians, and healthcare staff regarding home care, hospice, and community resources.

  • Facilitate communication across physicians, hospital units, intake teams, and other care providers to support continuity of care.

  • Participate in quality improvement activities, business development efforts, and new program implementation.

  • Maintain confidentiality, ethical practice, accurate documentation, and adherence to compliance requirements.

Minimum Qualifications
  • Bachelor’s degree or equivalent experience required.

  • Clinical education and current state clinical license preferred.

  • Minimum 2 years’ experience in acute care, customer service, sales/marketing, or a related field.

  • Strong verbal and written communication skills.
