Provides primary healthcare and performs selective medical services under the direction of specialty physicians.
Responsible for diagnostic and therapeutic management of patients by completing medical histories, conducting physicals, establishing diagnosis through tests, and formulating treatment plans.
Provides follow-up and health maintenance care of patients in accordance with protocols approved by a physician.

EDUCATION/EXPERIENCE

Preferred: post-graduate courses in epidemiology, infection prevention or disease control, completion of CDC or APIC sponsored infection prevention training is desirable. Supervisory and managerial experience is desirable. Working knowledge of statistics and epidemiological principles is required. Computer skills required include proficiency in the following Microsoft Office applications: Word, Excel, PowerPoint; graphics display programs such as QI Macros, as well as a level of comfort with learning and adapting to new programs
LICENSURE/CERTIFICATION

- Must be at least 21 years old and hold a valid Class A license
- 1 year of Class A experience within the last 5 years or 6 months of Class A experience within the last year
- No DUI within 5 years or 10 years if CDL holder at time of DUI
- No more than 1 preventable accident in the past 2 years, no major accident within 5 years
- No more than 2 moving violations in the past 2 years
- Must be able to pass a DOT physical and drug tests
