Position Purpose:
Join us in providing primary medical management in collaboration with our esteemed PACE Organization (PO) Physicians. As a Nurse Practitioner, you'll conduct assessments, develop care plans, and ensure participants receive comprehensive care while fostering a warm and supportive environment.
What You Will Do:
Collaborate with PO physicians, performing assessments and implementing care plans.
Participate in discharge planning and provide acute illness treatment.
Conduct home visits when necessary.
Assist in formulating clinical policies and procedures.
Participate in provider on-call rotation.
Minimum Qualifications:
Master's degree in Nursing with five (5) to seven (7) years of experience required preferably in long-term care. Must possess a current, unencumbered, active license to practice in the state of PO. CDS Certification and DEA Registration required. Board Certification by ANCC or AANP.
Certification as a Nurse Practitioner with prescriptive authority required. Current BLS required.
A minimum of one (1) year working with the frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
Current BLS certification.
Ability to travel within the Center's geographic region.

ESSENTIAL FUNCTIONS

- Recognizes and responds appropriately to urgent/emergency patient needs and initiates emergency procedures.

The Behavioral Health Nurse is an integral member of Behavioral Health Service Line and works collaboratively with the treatment team members and providers in the various inpatient and outpatient programs offered at Mercy. The Behavioral Health Nurse works closely with community partners to ensure successful transition of care and promotes education to the public on Behavioral Health-related topics through various forums.

Provides competent patient care to the above indicated patient populations by obtaining and appropriately interpreting data, making appropriate decisions based on demonstrated knowledge of age-specific growth and development, and appropriately adapting procedures and treatments.
Effectively communicates pertinent patient changes/conditions to colleagues and to multidisciplinary caregivers.
Hours/Schedule:
Full Time 36hrs a week
Night 12hr shift 7pm - 7:30am
Every third weekend, every other holiday
Minimum Qualifications:
Current RN Licensure in the state of Iowa, or qualified multistate licensure required.
Bachelor of Science in Nursing preferred
Ability to be non-judgmental regarding living and health related decisions.
