About You

POSITION PURPOSE
The dedicated Charge Nurse is a registered nurse responsible for providing shift-to-shift leadership for the practice and delivery of safe, effective, efficient, ethical, and culturally competent care to the unit patient population. This pivotal role bridges direct patient care with administrative responsibilities, ensuring optimal patient outcomes through effective leadership, resource allocation, and collaboration with the interdisciplinary healthcare team. The Charge Nurse ensures adherence to established policies, promotes a collaborative work environment, and supports staff development while maintaining patient safety and satisfaction.
ESSENTIAL FUNCTIONS
- Develops and maintains departmental daily staffing plan detailing appropriate employee classifications and qualifications consistent with the department's operational plan, projected volumes, patient/acuity and other relevant indicators. Assures compliance with staffing plan.
- Oversees daily operations, staff management, and coordination of interdepartmental activities within the unit or department during the assigned shift.
- Responsible for resolving clinical issues, and serving as a liaison between nurses, physicians, and administrative teams.
- Provides orientation/education for staff and students to ensure assigned patients have a positive care experience.
- Leads staff in the delivery of safe and effective patient care.
- Delivers nursing care by assessing, planning, implementing, and evaluating patient care in collaboration with the physician.
- Provides input on staff interviewing and hiring decisions as delegated by the unit leader.
- Maintains knowledge of documentation requirements needed for patient records.

- Leads and promotes evidence-based practice and research among unit leaders and staff.
- Leads quality improvement activities including performance improvement processes to facilitate clinical practice changes.
- Conduct follow-up on incident reports as directed by the Unit leader and participate in ongoing audits of nursing-sensitive indicators to ensure regulatory compliance. Responsibilities include, but are not limited to, chart reviews, patient safety audits, shift-based crash cart checks, and other quality assurance activities as identified by the facility.
- Functions independently with minimal supervision from Nursing Leadership, guided by the Nurse Practice Act, MercyOne Central IA Market and Trinity Health policies, established nursing standards, and sound professional judgment.
- Plans unit and nursing resource utilization based on established admission, discharge and transfer criteria, initiating appropriate intervention and collaboration to optimize patient care and unit/department operations.
- Serves as a group leader and fulfills an assistant supervisory or lead role, providing guidance and support to team members. Additionally, it contributes to Human Resources functions by offering input and recommendations on personnel matters.

MINIMUM QUALIFICATIONS

• Obtains physician orders and utilizes data collected during the admission assessment; agency teaching guidelines and appropriate nursing skills to implement and follow an established plan of care.
1.2 12% Effectively and efficiently manages the care of new admissions and coordinates care with interdisciplinary team.
• Plans admission process the day before admission when possible by calling patient and gathering necessary supplies and paperwork.
• Conferences with case manager and other members of interdisciplinary team regarding the status of patient and provides appropriate paperwork to follow the plan of care.
• Make appropriate referrals to other disciplines when indicated and provides information related to patient’s status.
• Appropriately informs the physician (Attending and Hospice) and other members of interdisciplinary team of any changes in patient’s condition, safety issues, or other complications that might impede the plan of care.

JOB SPECIFICATIONS
1. Education: Graduate of an accredited or approved school of nursing, either an AD, Diploma, or BSN program.
2. Licensure / Certifications: Must have current Registered Nurse license in NC, NC drivers license with an automobile that is insured in accordance with state or organization, and a current American Heart Association CPR certification.
3. Experience: Minimum of one years’ clinical experience required; hospice experience preferred.
4. Essential Technical/Motor Skills: Working knowledge of Word, E-mail, typing of 30 wpm, basic technical or medical knowledge of home health billing requirements. Advanced customer service skills to respond with simple answers, etc.
5. Interpersonal Skills: Ability to work independently as well as effectively as part of an interdisciplinary team.
6. Essential Physical Requirements: Ability to transfer and/or maneuver objects weighing at least 50 pounds in the assessment and implementation of patient care. Position Purpose:
Join us in providing primary medical management in collaboration with our esteemed PACE Organization (PO) Physicians. As a Nurse Practitioner, you'll conduct assessments, develop care plans, and ensure participants receive comprehensive care while fostering a warm and supportive environment.
What You Will Do:
Collaborate with PO physicians, performing assessments and implementing care plans.
Participate in discharge planning and provide acute illness treatment.
Conduct home visits when necessary.
Assist in formulating clinical policies and procedures.
Participate in provider on-call rotation.
Minimum Qualifications:
Master's degree in Nursing with five (5) to seven (7) years of experience required preferably in long-term care. Must possess a current, unencumbered, active license to practice in the state of PO. CDS Certification and DEA Registration required. Board Certification by ANCC or AANP.
Certification as a Nurse Practitioner with prescriptive authority required. Current BLS required.
A minimum of one (1) year working with the frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
Current BLS certification.
Ability to travel within the Center's geographic region.
