What you’ll do:

The Advanced Practice Provider (APP) will provide care within the Cardiac Surgery Department. The APP assumes the management of cardiac surgery patients, functioning in an independent and collaborative role with the surgeons and other health care team members. Ensures that care delivered to patients is provided according to the mission, strategic plans and all internal/external regulations, policies, and procedures. Practices within the Illinois Department of Professional Regulation Nurse Practice Act . Supports the established care models. 

  • Manage caseload of patients undergoing Cardiovascular and Thoracic surgery procedures including complex aortic surgery, lung and heart transplantation, ventricular assist device, and ECMO. 

  • Exercise autonomous decision making in the assessment, diagnosis, and initiation of medical treatments. 

  • The scope of practice includes daily management of patients in CVICU and telemetry floor, discharge planning, patient education, family education, staff education, and quality improvement projects. 

  • Responsible for planning, organizing, and directing patient care efforts in collaboration with medical and surgical physicians and other team members 

  • Evaluates the health status, acute episodes of illness, and health care needs of individuals through emphasis on comprehensive history, physical exam, and psychosocial assessment and accurately presents and documents in medical record. 

  • Designs and evaluates individualized treatment plan. Consults with other members of the health care team on emergent problems for rapid disposition/resolution. 

  • Coordinates care for optimal continuity of care across the health care continuum. 

  • Facilitates patient/family participation in health care by identifying learning needs and providing specialized education to enhance decision making. 

  • Exhibits professionalism by performing research, education, and leadership in clinical practice.

Pay Range: $109,304 - $180,336 per year
