- Collaborate with Customer Experience and Operations to ensure marketing aligns with the in-home experience.

What You Bring
- 10+ years of progressive marketing experience, including 5+ years in B2C direct response or demand generation โ ideally within home remodeling, home services, or a related industry.
- Proven ability to scale lead-generation programs that drive high-quality homeowner leads and revenue, including both offline and online marketing programs.
- Deep expertise in paid media, lifecycle marketing, and brand storytelling.
- Past successful track record with print media and direct mail programs.
- Hands-on experience with platforms like Google Ads, Meta, YouTube, Salesforce, SFMC, and attribution modeling tools.

As a Case Manager (CCSII), you will:
- Work collaboratively with clients and their families to identify needs, goals, and preferences
- Provide information about community resources and support services

- Assist clients in accessing service delivery systems, resources, and supports
- Monitor and evaluate services to ensure they meet clients' needs and preferences
- Carry a caseload of at least twenty-five (25) persons served, complete and document a minimum of thirty (30) billable units a day (or one hundred (150) units per week) within Long Term Services and Support System (LTSS) Maryland for activities completed on behalf of eligible persons.
- Manage and complete all assigned Person-Centered-Plans (PCP) and Supports and Services Planning Tools (SSPT).
- Pair up with CCS I teams and Program Supervisors to provide support.
- Act as a backup in the absence of CCS I and PCP Manager/Scheduler.
- Assist with continuous improvement projects such as training and/or initiatives.
- Attends weekly supervision with the Program Support Supervisor.

Interpret and implement medical prescriptions for speech-language pathology services, ensuring appropriateness for the home care environment
Conduct thorough assessments in the patient's home using standardized tools to evaluate communication, cognitive-linguistic function, voice, fluency, and swallowing abilities
Develop and implement individualized, evidence-based treatment plans that reflect both short- and long-term patient goals, with attention to the home environment and available resources
Adjust treatment strategies based on patient progress, caregiver input, and observed response to therapy, ensuring goals remain functional and achievable in the home setting
Document all assessments, interventions, patient progress, and caregiver interactions using electronic or paper records in compliance with agency policies and regulatory standards
Ensure documentation supports reimbursement criteria and meets all required timelines and confidentiality regulations, including HIPAA and agency-specific guidelines
Provide education and training to patients and caregivers on communication strategies, compensatory techniques, and safe swallowing practices within the context of daily home routines

PPCH has numerous host homes located throughout the metro area. Each host home is unique in its area of specialty, general layout and physical location.
What are the responsibilities of a Host Home Provider?
The Host Home Provider's main focus is to provide a safe and supportive environment for the member(s) in services in their home. Each Host Home Provider is responsible for ensuring the health and safety of the individual living in their home, while ensuring they will have plenty of opportunities for socialization and access to community resources.
What type of training does a Host Home Provider receive?
PPCH provides specialized trainings to all providers. These trainings include CPR/First Aid, Medication Administration, Customer Rights & many other areas. All host homes are monitored closely by PPCH to ensure that the member in services is receiving the highest level of care.

Position Summary:
The Service Technician is responsible for servicing and maintaining previously installed Leaf Home systems at customersโ homes based on established service ticket requests. This role provides outstanding customer service in the course of service repairs, annual maintenance, inspections, etc.

Description of Responsibilities
The Pharmacist is responsible for regulatory pharmacy compliance, patient safety, clinical patient management and communication with patient, caregivers, and physicians. The Pharmacist coordinates and supervises in the delivery of pharmacy services to patients at home. This includes compounding, quality improvement, dispensing, drug information and clinical monitoring activities.

Availability Requirements (Per Diem):
This is a per diem position with scheduling based on operational needs and employee availability. Candidates must have flexibility to work varied shifts within the following hours of operation:
- Monday โ Friday: Between 8:30 AM and 9:00 PM
- Saturday & Sunday: Between 9:30 AM and 6:00 PM
Specific shifts and hours will be assigned based on business needs and mutual availability. Per diem employees may not have a guaranteed number of hours.
Training Period
During the initial training period, employees should anticipate a full-time schedule for approximately one (1) to two (2) months. The duration may vary based on individual progress and demonstrated proficiency in the role.
Responsibilities Include the Following:
1. The Pharmacist receives physician orders (written and verbal), and evaluates the appropriateness of these orders for dosage, drug choice, potential drug interactions, route of administration and stability.
2. Ensure that pharmaceuticals are accurately compounded and dispensed in accordance with pharmacy protocols and physician orders.
3. Available to patients for drug counseling or for drug consulting and resolves concerns and complaints.
4. Orders appropriate laboratory test(s) to monitor the effectiveness of therapy and to minimize the potential of toxic levels of drug. Also notifies the physician of all abnormal laboratory test results, and will document this communication in the patientโs progress notes. The communication can be either or both by telephone or facsimile transmission (as long as the fax clearly identifies the abnormal results).
5. Communicate with medical professionals with regard to patient status and response to medication, and when necessary to resolve patient issues.
6. Contacts the physician to discuss alternatives in cases where drug interactions or stability makes the orders inappropriate. Discuss alternatives in cases where the appropriateness of dosage, drug choice or route of administration will not, in the Pharmacistโs judgment, be in the patientโs best interest.
7. The Clinical Pharmacist works with the office staff to arrange delivery times, coordinate care, quantify delivery amounts and arrange for and set up the infusion pump (if applicable).
8. Assume on-call responsibilities in accordance with pharmacy guidelines.
Minimum Qualifications:
1. Organizational skills sufficient to maintain consistently accurate records.
2. Ability to evaluate options and to make efficient decisions.
3. Current knowledge of all applicable state and federal pharmacy laws, rules and regulations.
4. Computer skills that include proficiency with spreadsheets, word processing, and efficient use of the internet and e-mail.
5. CPR+ experience is a plus.
- Ability to use good judgment and work with minimal direction.
- Must have good organizational skills and be able to work across interdisciplinary boarders.
- Must possess excellent oral and written communication skills, with the ability to express technical issues in โlaymanโ terms.
Education and/or Experience:
- Must be a graduate of an accredited school of pharmacy. Doctor of Pharmacy degree is preferred.
- Additional training in an accredited Pharmacy Residency program is of benefit.
- A minimum of 3 years experience in a hospital or home health infusion service providing direct patient care is strongly preferred.
- Prior experience working with managed care environments is a benefit.
- Member of a professional society is recommended (i.e., ASHP, ASPEN, APHA).

- E-filing and maintenance of healthcare records such as:
- - Referral/Restart/Rx/J&P- Lab Results- Authorizations- Delivery Tickets- Compounding Records- Patient Records- Billing Correspondences- Claims/Invoices- Nursing/Home Health
- Providing clerical support to management and other departments
- Retrieve information from files as requested.
- Transmit documents as requested by fax, email or other means of transmission.
- Document all items removed from patient files, including information about who received the items, the date and time received, the items received, and the date and time the items were returned to the file.
- Observe HIPAA guidelines carefully, and maintain current knowledge of best practices with regard to patient privacy issues.
- Sort incoming mail, and distribute or file as appropriate.
- Participate in surveys conducted by authorized inspection agencies.
- Participate in the pharmacyโs Performance Improvement program as requested by the
- Performance Improvement Coordinator.
- Participate in pharmacy committees when requested.
- Participate in in-service education programs provided by the pharmacy.
- Report any misconduct, suspicious or unethical activities to the Compliance Officer.

Minimum Qualifications:

What Youโll Do: As a Flight Logistics & Client Services Coordinator, you will serve as the central point of communication for charter operations.
