We are seeking a dedicated and skilled Vascular Access Nurse to join our specialized team. In this role, you will be responsible for assessing, inserting, and managing various vascular access devices, including PICC lines, midlines, and ultrasound-guided peripheral IVs. Your expertise will ensure optimal patient outcomes and contribute to the overall quality of care provided.

Work closely with interdisciplinary teams, including physicians, nurses, and case managers, to coordinate patient care and ensure optimal outcomes.

Proficiency in ultrasound-guided procedures, strong critical thinking abilities, and excellent communication skills.

ยทย ย ย ย ย ย ย ย  Identifies and initiates appropriate nursing interventions

ยทย ย ย ย ย ย ย ย  Provides care appropriate to condition and age of the patient

ยทย ย ย ย ย ย ย ย  Performs timely and appropriate documentation relating to medical necessity in the medical record

ยทย ย ย ย ย ย ย ย  Responsible for completion and revision of the Interdisciplinary Care Plan for each patient

ยทย ย ย ย ย ย ย ย  Performs timely and accurate QI assessments

ย Assesses patients for the need for a mid-to-long-term vascular access device (VAD) to provide a reliable device for therapy utilizing evidence-based guidelines for VAD placement and care.

ยทย ย ย ย ย ย ย ย  Assesses patients with difficult intravenous access (DIVA) for appropriate VAD need

ยทย ย ย ย ย ย ย ย  Assesses patient information via electronic chart reviews for indications, pertinent history, and lab values and medication administration record

ยทย ย ย ย ย ย ย ย  Verifies indications for VAD placement, coordinate with physicians and provide recommendations for selection of appropriate vascular access device

ยทย ย ย ย ย ย ย ย  Coordinates discharge planning and post-discharge follow-up for VAD's, in collaboration with inpatient hospitalists

ยทย ย ย ย ย ย ย ย  Provides vascular access device placement for patients in outpatient settings as appropriate

ยทย ย ย ย ย ย ย ย  Places and assists in maintaining the appropriate vascular access device throughout all hospital services except Pediatric patients; or refer Interventional Radiology when appropriate

ยทย ย ย ย ย ย ย ย  Obtains informed patient consent for PICC placement

ยทย ย ย ย ย ย ย ย  Performs ultrasound-guided placement of vascular access devices under sterile or surgical aseptic technique as appropriate

ยทย ย ย ย ย ย ย ย  Administers local anesthesia(lidocaine) during ultrasound-guided procedures as appropriate

ยทย ย ย ย ย ย ย ย  Utilizes PICC tip confirmation system to confirm placement of PICC line

ยทย ย ย ย ย ย ย ย  Follows up on Chest X-Ray to confirm PICC placement of PICC line

ยทย ย ย ย ย ย ย ย  Documents placement of vascular access devices in the electronic health record

ยทย ย ย ย ย ย ย ย  Addresses and assists medical and nursing colleagues in troubleshooting complications of vascular access devices

ยทย ย ย ย ย ย ย ย  Follows up on complications encountered during the PICC placement and provide recommendations for unsuccessful PICC placement

ยทย ย ย ย ย ย ย ย  Declots VADs or assist staff as a resource in declotting

ยทย ย ย ย ย ย ย ย  Provides patient reaching on Vascular access device care and maintenance

ยทย ย ย ย ย ย ย ย  Collects VAD data and report on number of procedures performed independently, number of complications and rate of central line-associated blood stream infections

ยทย ย ย ย ย ย ย ย  Participates in the development of internal quality improvement process as assigned

ยทย ย ย ย ย ย ย ย  Maintains knowledge and competency level related to infusion therapy

ยทย ย ย ย ย ย ย ย  Evaluates new VAD products introduced to the hospital and assists in training staff as appropriate

Job Summary

Conduct Clinical Pastoral Education (CPE) programs under the standards of the Association for Clinical Pastoral Education (ACPE). The CPE Certified Educator programs under the standards of the Association for Clinical Pastoral Education (ACPE). Three main functions of the CPE Certified Educator are maintaining accreditation and standards of the ACPE, education and supervision of CPE students and supporting an interdisciplinary approach to the provision of spiritual care.

Our accreditation standards are held by reviewing, updating, and developing CPE program policies per Sanford Health, updating student handbooks, ensuring completeness and accuracy of all Center Accreditation materials, preparing for ACPE accreditation reviews along with facilitating CPE Professional Advisory Group meetings to inform members of educational activities and receiving suggestions for maintaining standards of excellence in the program.

The CPE Certified Educator will educate and supervise the CPE students. This position will be responsible for recruiting, interviewing, and selecting candidates for CPE internships and supervisory education in accordance with the admission requirements along with providing direct clinical supervision of students including the development of student-centered learning curriculum. Mentorship and educating students in the certified educator candidate program will be required along with the coordination of student spiritual support with staff chaplains by managing the student's self-directed learning in group settings. The CPE Certified Educator will also facilitate student orientation and training according to Sanford Health, Spiritual Care Department and ACPE.

While focusing on accreditation and education, the CPE Certified Educator will provide feedback and recognition along with timely and preemptive corrective intervention as needed. The CPE Certified Educator will be required to participate in various team meetings, interdisciplinary team rounds, and participates in professional leadership development; including areas related to chaplaincy and CPE professional organizations along with initiating own professional continued education while maintaining certification within ACPE.

Qualifications

Masterโ€™s level theological degree from an accredited seminary.

Must have a theological, spiritual and psychological understanding of spiritual and human dynamics; meaning making, and existential distress, and knowledge of rituals. One to two years of experience providing spiritual care in an acute care or health care setting is required.

Certified Educator certification required. National faculty preferred. Must maintain certification as a CPE Educator with the Association for Clinical Pastoral Education.

Ordination, endorsement or commissioned by faith tradition/denomination/church authority. Board Certified Chaplain with the Association of Professional Chaplains or certification eligible required; must obtain certification within two years of hire.

Position Overview:

The purpose of this position is to serve in the Registered Nurse and Director of Nursing capacity.   • Observe and record signs, symptoms and behaviors including the physiological status of patients
  • Presents proposed revision of interventions and desired outcomes to the physician
  • Maintain patient's privacy and confidentiality of information and records at all times
  • Follow physicianโ€™s orders in the delivery of nursing care
  • Collect laboratory samples and perform lab testing in accordance with established policies and procedures, as well as COLA and CLIA regulations
  • Adhere to required infection control, isolation procedures and aseptic techniques in the delivery of patient care

Job Summary and Qualifications

The Clinical Nurse Coordinator (CNC) ensures and delivers high quality, patient-centered care and charge nurse duties in the unit/department during the designated shift.   • Collaborates with a multidisciplinary team to coordinate, evaluate, customize, and deliver high quality, patient-centered care.

  • โ€‹Resolves and/or channels employee grievances, disputes, or concerns according to appropriate procedures. Escalates issues as needed.
  • โ€‹Supports physician partnerships.
  • โ€‹Represents the organization positively within the community; participates in company-supported community/patient events and in-services.
  • โ€‹Serves as the charge nurse for the unit, scheduling staff and managing all practical aspects of staff labor in accordance with patient care needs and established productivity guidelines.
  • โ€‹Supports proper inventory control and assists with managing supplies and equipment.
  • โ€‹Performs other duties as assigned.
