Wellstar Advanced Heart Failure & Palliative Care is a hospital outpatient department of Kennestone Hospital. Our goals are to improve clinical outcomes and quality of life for patients with heart failure while improving patient understanding and self-management of their disease process. Our care is based upon current evidence and best practice, and teamwork and collaboration are at the core of our care model. Our clinic operates Monday-Friday and is closed on weekends and major holidays. The clinical nurse position provides direct patient care for heart failure/LVAD/palliative patients, including rooming, vital signs, physical assessment, phlebotomy, medication administration, EKG, 6-minute-walk tests, and patient education. The position also involves telephone/EPIC inbasket triage, working with social workers and nurse navigators to facilitate patient care coordination, and delegating/supervising care provided by non-licensed staff.
Job Summary:
The RN Clinical Nurse is a proactive member of an interdisciplinary team of licensed and unlicensed care givers who ensure that patients, families and significant others receive individualized high quality, safe patient care. They practice in a clinical environment that is administered by Nurse Managers and other leaders and is supported through the Wellstar Shared Governance Model. The framework for practice is steered by the Wellstar Professional Practice Model and evidence-based practice and research. RN competencies are derived from these models and supported by the Wellstar Values.It is expected that all RN Clinical Nurses are licensed, knowledgeable and uphold the practice of nursing as outlined by the Georgia Professional Nurse Practice Act and implements the Scope of Practice and Code of Ethics Standards put forth by the American Nurses Association. As a member of the patient services team, it is expected that the individual upholds the voice of the patient, system policies and procedures while supporting service excellence goals.

Our ideal candidate has the ability to collaborate with hospital administration, physician leaders, and nursing staff to organize and support our advanced heart failure program. Our goal is to be able to deliver the best of care, which includes “end stage” therapies right here in Boise. We are currently utilizing ECMO, and LVAD therapies are being considered to treat end stage disease.
Schedule: Monday - Friday 4 10 hr shifts
Responsibilities include:
Coordinate and manage ongoing care for Left Ventricular Assist Device (LVAD) patients adult (18 through 64 years) and geriatric (65 years and above) throughout the care continuum.
Provide supportive care to temporary and durable MCS patients; functioning in an expanded role using the nursing process and integrating acts of complex assessment and comprehensive care management.
Record physical findings and participate in formulating care plans, based on the patient's condition, to maintain a complete and accurate patient medical record in the physician office and hospital.
Provide leadership, clinical consultation, and education related to the management and care of complex cardiac patients and MCS devices for staff, physicians, and other disciplines with contact or care responsibilities for this population.
Act as a liaison between patients, families, hospital staff, and referring physicians.
Participate in community outreach and educational lectures.
Manage Mechanical Circulatory Support (MCS) program protocols and documentation, including ELSO registry data.
Maintenance and tracking of outcome and quality standards.
Perform other related duties as assigned

Continuity Planning – Identifies discharge needs, coordinates planning, and problem solves continuity issues to ensure seamless continuum of patient care; collaborates with hospital based case managers to ensure effective care continuum and discharge plans are met. Assures appropriate clinical information is provided to outside agencies
Collaboration – collaborates with the multi-disciplinary team, including physicians and other healthcare disciplines, and outside agencies regarding patient stay management and care coordination issues. Initiates appropriate consults, e.g., physician and other ancillary services.
Minimum Qualifications:
Education: Graduate from school of nursing; BSN or Master’s Degree preferred
Licensure / Certification: Current license to practice as a registered nurse in the State of Ohio

Seeking BE/BC Non-Invasive Cardiologist to lead our Heart Failure Program for a well-established cardiology service in Springfield, Missouri.
The physician will join a growing team currently with six interventional and non-interventional cardiologists, two electrophysiologists, three CT Surgeons, and four vascular surgeons.
Our Cardiology outpatient and heart failure clinics are attached via a skywalk to Hospital.
Cardiology is supported with 17 Cardiology APPs providing outpatient support and 24/7 inpatient coverage.
This group works from the main hospital of a hospital system.
No traveling required.
Invasive Cardiologists perform over 4,700 cath lab procedures annually, including a robust structural program.
This position covers their outpatient/heart failure clinic and supports our extensive cardiology program, performing and interpreting non-invasive cardiac imaging, including echocardiography and Nuclear Medicine.
