Annual total cash compensation for this role assumes full-time employment (40 weekly hours) at full productivity and generally follows the range above. Total cash compensation includes earnings from per visit point pay and hourly pay and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. This role receives two types of compensation depending on the work being performed. When conducting visits, you will be paid per visit point rate compensation. Your per visit pay will be calculated by multiplying your per visit point rate by the productivity points you accrue for various types of visits. Each type of visit is assigned a certain number of productivity points that is inclusive of "direct" and "indirect" patient care activities. Visits are assigned based on patient and business needs. As the Home Health Patient Care Manager, you are responsible for the overall supervision and coordination of clinical services. Coordinates and supervises an interdisciplinary team of staff to assure the continuity of high quality care to home health patients assigned to the team's area in accordance with physician prescribed plan of care, and all applicable state and federal laws and regulations.
Primary Responsibilities:

* Provides clinical services within the scope of practice, as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team
* Receives referrals, ensures appropriate clinician assignments, evaluate patient orders, and plot start of care visits
* Coordinates determination of patient home health benefits, medical necessity, and ongoing insurance approvals
* Ensures patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance with physician orders
* Reviews assessments and plans of care daily, per assigned workflow, and consults clinicians with recommendations, as appropriate
* Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

Required Qualifications:

* Current unrestricted RN licensure in state of practice
* Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation

State Specific Requirements

* CA: One year prior professional nursing experience.LA: At a minimum, one year of clinical experience.

Preferred Qualifications:

* Current CPR certification or ability to complete within 90 days of hire
* Home care experience

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $72,800 to $130,000 annually based on full-time employment. We comply with all minimum wage laws as applicable.

The Per Diem RN in Medical Imaging is responsible for providing direct patient care by applying special clinical nursing knowledge and skills related to the Medical Imaging setting. He or she will strive to ensure the safety and comfort of patients and families according to legal, organizational and professional standards.

Required Qualifications:

* Graduate of an approved RN nursing program
* Current BLS through American Heart Association
* 3+ years of RN experience
* Competent in the use of equipment and related supplies for selected patient population. CPR, oxygen administration and intravenous therapy. CRT experience
* Proven excellent verbal, written and interpersonal communication skills.
* Willing or ability of minimum 8 shift per month
* Willing or ability of 4 shifts per year before or after clinic holidays, with 2 shifts related to summer holidays and 2 shifts related to winter holidays.
* Preferred Qualifications:

* Bachelor's degree in nursing
* Specialty nursing certification applicable to medical imaging
* ACLS
* Experience in a medical imaging setting/center
* Windows computer experience, RIS experience
