POSITION SUMMARY/RESPONSIBILITIES

The RN Service Coordinator-RN (RN-SC) contributes to the Long Term Services and Supports (LTSS) service coordination process by performing activities within the scope of licensure; provides supervision and direction to staff participating in Member’s cases following applicable state law and contract; develops, monitors, evaluates, and revises the Members’ care plans to meet Member’s needs, to optimize health care across the care continuum to enhance the Member's well-being, independence, integration in the community, and potential for productivity. EDUCATION/EXPERIENCE

Graduation from an accredited school of professional nursing or social work is required. BSN is preferred. Four (4) recent years of clinical experience preferred, which may include service coordination, case management, quality management, or managed care experience. Working knowledge of HMO standards, LTSS, and NCQA standards is preferred. Knowledge of Medicare and Medicaid HMO experience is preferred. Experience in meeting the needs of vulnerable populations who have chronic or complex conditions is required. Bilingual, English/Spanish, is preferred.

LICENSURE/ CERTIFICATION

A current, unrestricted license to practice professional nursing issued by the State of Texas is required. RUG Certification is required and must be obtained within 30 days of employment for all RN candidates.

Requirements

ABOUT THE OPPORTUNITY/WORK

The Early On Evaluator/Service Coordinator is responsible for coordinating, conducting and completing developmental assessments, reports and Individualized Family Service Plans as needed in accordance with IDEA regulations. Incumbent is responsible for providing outreach to families in Oakland County communities through home visits; for initiating and maintaining communication and coordination based on family needs, including schools district personnel; and for maintaining internal and external customer relations through completion of paperwork, adherence to COA standards, quality assurance measures and participation in professional development activities. The position reports to the Early Childhood Services Manager.

  • Conduct developmental assessments on referred children within identified time frames.
  • Complete reports summarizing the results of the assessment, including recommendations and case conceptualization within identified time frames.

Primary Purpose of Position

Responsible for the direction, assignment, and coordination of Turbine Floor Outage Execution to support safe, reliable, and efficient outage activities consistent with Constellation and Clean Energy Center (CEC) goals.   • Participate in various aspects of work control coordination, scheduling, and engineering meetings involving online and refuel outage activities, to ensure that work is properly assigned, planned and ready to execute.
  • Effectively communicate and address CEC and personnel issues to the appropriate levels of Outage Services & Alliances (OS&A) and CEC management. Foster open communications and collaboration between all organizations internal and external to OS&A.
  • Oversee project activities of multidisciplinary team regarding turbine generator disassembly and reassembly, coordination of turbine deck lay-down, mobilization of equipment and tooling as well as interfacing with management and specific contract personnel. Satisfy contractual obligations, meet project milestones, and optimize short-term and long-term company profit by effectively managing and facilitating resources.
  • Understand, support, and administer all OS&A processes, such as the training program, surveillance program, procedure revisions, and Corrective Action Program (CAP) to ensure effective work practices. Identify and support improvement opportunities and development, implementation, and performance monitoring of all Business Goals and objectives.
