Schedule: Full-time hours Mon - Friday, Weekend on call every 8-10 weeks and 1 week night, every other week
Position Overview:
- Ensures the timely and adequate delivery of hospice services to the terminally ill patient and their family, operating within the plan of care as established by the hospice team and attending physician.
- Assesses the total needs of the patient/family during regularly scheduled and after hour home visits; Documents: assessment, identified problems, nursing interventions, goals, and outcomes of interventions.
- Coordinates total patient/family hospice care under the supervision of the Director and with the interdisciplinary team, the attending physician, and other providers; documents such coordination.
- Collaborates with the interdisciplinary team in the development, review and revision of the clinical component of patient/family plan of care, including exchange of information, review of problems, assessing effectiveness of interventions and documenting outcomes.
- Ensures continuity of care between patient/family, team members, ancillary providers, long term and inpatient care facilities, and the attending physician.
- Communicates patient/family/caregiver needs, ongoing nursing assessment, interventions, goals and outcomes through the interdisciplinary process.
- Provides and documents continuing education of the hospice concept of care to patients and their families/caregivers.
- Supervises the delivery of patient care provided by Hospice Aides, Licensed Practical Nurses; ensuring compliance with the established plan of care and completing required documentation of supervision.
- Facilitates the delivery of hospice services to patients residing in long term care facilities, documenting joint coordination of care with facility professional staff, and attending facility care plan meetings as appropriate.
- Participates in on-call rotation for delivery of care after office hours, on weekends, and holidays.
- Provides ongoing monitoring of patient appropriateness for hospice services and completes required documentation for certification and recertification.
- Interfaces with the patient/family/caregiver, the hospice team, other health care providers in a respectful, professional and courteous manner.
- Maintains compliance with Medicare Conditions of Participation, Joint Commission standards, Hospice and Professional Standards of Nursing Practice, and agency specific policies and procedures.
- Supports community education of hospice for growth and development of the program.
- Dispenses medical supplies in a cost effective manner, as dictated by the plan of care and patient needs.
- Participates in quality improvement, utilization review, and infection control activities as requested.
- Attends appropriate inservices, and participates in continuing education.
- Ensures accurate and current patient chart information by timely and consistent documentation.
- Maintains a professional, well-groomed appearance, adhering to the agency dress code.
- Maintains sound privacy and security practices and prevents privacy or security breaches. If breach occurs, takes corrective action.
Qualifications:
- Current license as a Registered Nurse in the State of Alabama
- Minimum of one (1) year medical/surgical experience. Hospice/Home Health experience preferred.

Essential Functions:
This Program Cost Control Analyst position will, under minimal supervision, interface with the program managers, functional management, and cost account managers (CAMs) to provide financial and administrative support and analysis to meet program requirements.
The Program Cost Control Analyst will have experience with Earned Value Management and performance measurement baseline concepts and be able to apply them to multiple contracts for control accounts within an engineering Integrated Product Team (IPT). This position will be responsible for leading and training other analysts if full level 1 Earned Value contracts.
Earned Value Management (EVM) tasks include:
Establishing Work Breakdown Structure for execution of budgeted cost of work scheduled
Assessing and maintaining of objective performance criteria – developing, maintaining, analyzing and justification of estimates to complete
Supporting internal and external reporting requirements for variance analysis and budgeting baseline
Maintaining a solid and accurate cost and schedule integration with business partners and program teams
Analyzing funding, cost risk analysis/assessment and visibility reports - Preparing government cost performance reports and preparation and/or review of Performance measurement variance analysis
Additional responsibilities will include:
Perform analysis & prepare reports in order to ensure that contracts are within negotiated and agreed-upon parameters and government cost control guidelines. Be responsible for supporting the preparation and coordination of the monthly and quarterly financial forecasting and reporting processes. Provide internal reporting requirements to include incorporation of forecast, identification of staffing issues related to baseline/ETC, and execution of corrective action or updates using program reporting tools. This position will interface with Business Managers and support booking rate files and profitability updates. Good leadership skills & the ability to work with the Program Office while leading a smaller team are essential.

About the Unit:
The laboratory at Mount Carmel Dublin will provide a wide range of laboratory procedures that will aid clinicians in diagnosing, treating, and managing patients. There will be comprehensive testing in the specialties of hematology, chemistry, coagulation, urinalysis, blood gases, molecular, and blood banking. Located on the 1st floor will be an Outpatient Lab Service Center that will provide convenient hours for patients to have their blood drawn. About the job:
The Medical Technologist performs laboratory testing. Has knowledge of the clinical significance of results, monitors and understands operation of instrumentation, and reports results accurately and
rapidly.
What you'll do:
- Performs all clinical testing in assigned department.
- Records and reports test results accurately and promptly.
- Performs and interprets quality control testing following established guidelines.
- Maintains a high level of quality assurance.
- Practices established safety procedures, including Universal Precautions and proper use of safety equipment.
- Able to recognize a problem by following prescribed strategies and make necessary corrections.
- Where no present criteria for decisions are available, will consult with supervisor/manager. In absence of supervisor/manager, will act to determine final decision.
- Coordinates and evaluates new reagents, procedures and equipment as requested by supervisor/manager.
- Performs routine scheduled and specialized maintenance of laboratory equipment.
What we're looking for:
- Education: Baccalaureate or Associate Degree in Medical Technology or related field.
- Licensure / Certification: Certification from American Society of Clinical Pathologists, equivalent; eligible for certification.
- If applicable, must maintain active certification.
- Experience: Preferred, but not required.

What you will do:
As an RT you will work at assessing, treating and caring for patients of all ages who suffer from pulmonary (breathing) disorders. In addition to analyzing and performing diagnostic testing, respiratory therapists perform relevant exams as well as set up and utilize cardiopulmonary equipment and machines needed to assist or monitor the patients breathing.
Responsibilities:
- Gather data to be able to accurately assess a patient's current health status
- Assess the patient according to the patient's physical or age related needs
- Observe, record and report symptoms, reactions and progress of patients
- Administer and record prescribed medications, treatments and diagnostic procedures
- Responsible for airway management including performing endotracheal intubations and extubations, pulmonary hygiene, and tracheostomy care and emergency life support procedures
- Provides extensive health education to patients and to the general population through community services
What you will need:
- Associates Degree in Respiratory Therapy
- Current NY State RT License
- ACLS and BLS certificates
Pay Range: $34.00 - $48.00
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
