We have an exciting opportunity to join our team as a Temporary/Per Diem Medical Assistant - In House Temporary Program (Full Time Coverage, As Needed).
In this role, the successful candidate The Medical Assistant is responsible for assisting physicians and/or Advanced Care Practitioners and nurses in providing patient care. This role is responsible for validating patient identifiers, preparing patients and rooms for examination, measuring and recording vital signs, drawing blood, and collecting other lab samples. The Medial Assistant is also responsible for administrative tasks, such as, scheduling appointments, answering phone calls, and performing data entry.

Clinical & Administrative Responsibilities 60;
• Completes necessary intake which includes obtaining patient vital signs and other tests within defined scope of practice as needed. 60; 60;
• Follows guidelines for validating patient medical record when accessing and updating information, including patient name and date of birth. 60;
• Assists with maintaining patient flow to and from exam rooms and treatment areas. Assists in escorting patients and specimens throughout the facility.

• Communicates the functionality and purpose of MyChart to patients during intake
• Ensures the appropriate translation services or equipment is in place prior to the start of visit 60;
• Assists with blood/specimen collection and ensures proper handling and processing of patient specimens as as per organizational policy & procedure. 60;
• Performs Point of Care testing such as: EKG, Glucose Testing, and Pregnancy Test, Urinalysis, following organizational policy & procedure, and with documentation of any required competency in place.
• Preps the appropriate patient encounter ensuring that all relevant information for visit is up-to-date including bloodwork, radiology images, medications, and past medical history. 60;
• Reviews and imports any “Prepare for your Visit”, 60;“Outside Information” and patient history prior to provider encounter
• Works with clinical team to ensure logbooks related to equipment, specimen collection and other clinical safety measures are followed per FGP standards.
• Supports best practice workflows for MIPS by updating medical records with patient vital signs and Histories (Immunization, Medical & Surgical). 60;May also be required to scan and/or upload diagnostic reports according to their description and link them to their associated order in Epic.
• Maintains patient privacy as it relates to HIPPA standards.
• Handles appropriate patient calls and documents outcome of all patient communications as needed. Refers all clinical questions/concerns to RN/Provider.
• Assists with inbasket message management including any necessary tasks related to patient medical advice requests as per best practices
• Utilizes EPIC functionality to review and update patient records as well as maintain communication with peers and supervisors related to patient care
• Assists physicians with procedure and/or set-up according to the specialty where necessary 60;
• Stocks exam/treatment rooms and/or supply closets with medical supplies, linen and medical equipment as needed. Effectively communicates as supplies need to be replenished.
• Cleans and sterilizes designated equipment after use, , following organizational policy & guidelines, and with documentation of required competency in place.
• Supports FGP Clinical Quality Management by facilitating appropriate documentation and maintenance of clinical supplies and equipment.
• Reviews appropriate downtime procedures as it relates to patient visit 60;
• Completes annual competencies such as Fire Safety, Hazardous Waste, Medsled, HIPPA compliance, etc. 60;
• Partners with administrative and clinical personnel to complete prior authorization, medication refills and referrals related to patient continuity of care.
• Assists with prescription refill request by locating requested prescription in EMR, identifying last patient visit, and tasking RN, LPN, or Provider to evaluate, set up and/or process for renewal
• Accompanies physician into exam room and transcribes history, including history of present illness, past medical and surgical history, family and social histories, medications and allergies, physical exam, laboratory and radiologic orders as dictated by the MD and document accurately the physician's encounter with the patient.
• List all proper diagnoses and symptoms as well as follow up instructions and prescriptions as dictated by the physician.
• Documents patient orders including laboratory tests, radiology tests, medications, etc.
• Performs other duties as assigned.

Minimum Qualifications:
To qualify you must have a High School Diploma or the equivalent. Completion of a Medical Assistant Program from an accredited institution OR upon hire hold a current medical assistant Certification from a Nationally Recognized accredited institution (such as American Association of Medical Assistant(AAMA); American Medical Technologies (AMT); National Center for Competency Testing (NCCT); National Healthcare Association (NHA)). Competencies: Working knowledge of English is evident in verbal, reading and writing abilities; other language an asset; demonstrated ability in computer skills. Ability to complete multiple tasks efficiently and thrive in a team work environment which pursues a positive patient care experience.
Preferred Qualifications:
MA experience preferred. EKG, phlebotomy and venipuncture experience preferred.

In conjunction with the Laboratory Director and/or Laboratory Manager, this Job develops and implements new procedures, instrumentation, quality control, and reagent use.
This Job is also responsible for the development and maintenance of policies and procedures that meet the standards of regulatory agencies and implement service quality improvements.

The primary role of the Home Infusion Nurse is to provide clinical care and education to home-based patients receiving a wide range of parenteral medications dispensed from Optum Infusion Pharmacy (the "Company"). The most common route of administration is intravenous, however medications may also be given via subcutaneous infusion or injection, intramuscular injection, or intraspinal (intrathecal and epidural) infusion. Service delivery takes place in patient homes and also in ambulatory infusion suites, where available. All infusion services and medication delivery is provided in accordance with Optum Infusion Pharmacy policies, procedures and therapy-specific protocols, and based on comprehensive orders received from a physician or licensed practitioner. The Home Infusion Nurse fills an integral role in the clinical operations of the organization, working collaboratively with the multi-disciplinary team needed to prepare, deliver and seek reimbursement for the infusion therapy that is safely and effectively administered to each patient.
Schedule for this position will be Monday, Wednesday, Thursday, and Friday, 10am-8pm. This is a heavy travel position, mileage is reimbursed and drive time is part of the schedule. Patient population includes both adult and pediatric patients.
Primary Responsibilities:
- Establish and maintain positive clinical rapport with patients and their families, working closely with them in the provision of infusion therapy, education related to self-care and self-administration, and clinical monitoring of patient response to therapy
- Assess patient appropriateness for home infusion therapy, including safety of the home environment, availability of caregiver support, and willingness of patient and/or caregivers to become as independent in therapy administration and monitoring of response to therapy as possible
- Obtain and document patient's medical history at the start of care and ongoing, particularly as related to the diagnosis being treated with the prescribed infusion therapy
- Complete all required clinical documentation thoroughly and timely, including nursing plans of care (POC), plans of treatment (POT), visit notes, clinical summaries, medication profiles, discharge summaries and any updates provided to members of the patient's care team, as well as obtaining patient and/or caregiver signatures on relevant consents and required forms
- Participate in case conferences as needed to comprehensively plan patient care with the multi-disciplinary care team
- Provide skilled care to patients of varying ages and medical conditions, as outlined in the plan of care and in accordance with physician orders and the plan of treatment
- Establish, secure and maintain vascular access devices as prescribed, including (but not limited to) peripheral IV (PIV) placement, PICC and tunneled central venous catheter dressing changes, and implanted port access
- Administer medications as prescribed, performing therapy-specific clinical assessments in accordance with the Company's policies, procedures and protocols, which may include frequent vital sign monitoring, management of acute infusion reactions, monitoring of side effects, and procurement of laboratory specimens
- Communicate observations of patient response to therapy and overall physical assessment/condition with members of the care team in a timely manner, which may include the nursing supervisor, dispensing pharmacist, registered dietitian, and licensed prescriber, among others
- Establish collaborative relationships with home health agencies (HHAs) delivering services on behalf of / or in conjunction with the Company, providing agency education and support as requested by the nursing supervisor
- Maintain a comprehensive knowledge base of infusion therapies administered in the home and ambulatory infusion center, utilizing Company policies, procedures and protocols in addition to all available training resources to ensure understanding of each therapy before initiating treatment
- Maintain current nursing licensure in all states in which patients are serviced, as required by the respective State Boards of Nursing and/or Department of Health
- Comply with all State and Federal regulations and laws, including employee health and safety protections mandated by the Occupational Safety and Health Administration (OSHA), patient privacy and data security required by the Health Insurance Portability and Accountability Act (HIPAA), as well as all home infusion standards of practice
- Complete all training required upon hire and annually per policy, including cardiopulmonary resuscitation (CPR) certification
- Serve as a professional representative of the Company when meeting with and/or providing education to external customers, payers and referral sources
- Assist in staff orientation and training, serving as a clinical educator and resource as requested
- Drive personal/rental vehicle (per travel policy) in the completion of job duties, including (but not limited to) travel to the Company office, patient homes, area hospitals, physician offices, home health agency offices, and the Company's Ambulatory Infusion Suite (AIS)

Required Qualifications:
- Active and unrestricted licensure as a Registered Nurse (RN) in the state(s) of practice and others as requested

- Pediatric experience
- Current Basic Life Support (BLS) and/or CPR certification, or willing to obtain prior to starting employment
- 1+ years of experience working as an RN in a clinical care setting that contributes to proficiency in infusion therapy (such as medical surgical unit, emergency department, intensive care unit, post-anesthesia care unit, or similar setting)
- Proficient with electronic medical record documentation
- Demonstrated ability to work with an interdisciplinary team of professionals including pharmacists and technicians, physicians and other licensed prescribers, registered dietitians, home health agencies (HHAs), sales representatives, and others as needed to provide comprehensive care and support of patients and caregivers
- Demonstrated ability to and able to work independently in home or alternate-site settings

- Demonstrated ability to and able to meet attendance, overtime, after-hours on-call, and other reliability requirements of the position

Practices as a member of the interdisciplinary health care team responsible for the provision of medical nutrition therapy. Fosters continuity of the Nutrition Care Process across all phases of the continuum of care as recommended by the Academy of Nutrition and Dietetics. Performs nutrition assessment for patients identified to be at nutrition risk based on criteria outlined by the hospital and best practice standards. Calculates metabolic nutrient requirements according to the age and disease specific needs of patients. Performs nutrition focused physical exam to further evaluate risk of malnutrition as indicated. Develops a nutrition diagnosis as appropriate and consistent with the Nutrition Care Process. Creates and implements a nutrition intervention and care plan for all patients with a nutrition diagnosis. Continuously evaluates and monitors the effectiveness and outcomes of medical nutrition therapy intervention. Provides nutrition education to patients, family, and/or members of the health care team on recommended nutrition intervention(s) to accomplish nutrition goals; which may include, but is not limited to any of the following topics: specialized diet, enteral/oral medical nutrition supplements, food selections, eating behaviors, and community nutrition resources.
