ABOUT THE ROLE

(Full Time Overnights 10:30pm to 7:00am) Sunday, Monday, Thursday and Friday

As a Charge Nurse at Belmont Village, you will administer medications, support wellness initiatives, help ensure regulatory compliance and assist with quality monitoring. You will communicate with healthcare practitioners, ancillary providers, pharmacies, family members, and team members as needed ensuring timely clinical interventions and follow-up. You will lead and provide direction to Caregivers and Medication Technicians where applicable. The ideal candidate is reliable, takes pride in their work, and demonstrates empathy. They excel in a fast-paced, physically active position, collaborate effectively, and embrace a growth mindset, prioritizing exceptional hospitality and helping seniors to thrive as they age.

YOUR TYPICAL RESPONSIBILITIES

  • Follow up on physicians orders, coordinate services with ancillary providers and report to physicians and families any changes of condition or other pertinent clinical care information

  • Provide medication administration to an assigned group of residents according to physicians orders and report any adverse reactions or clinical observations documenting as necessary

  • Ensure resident care and service plans are followed by overseeing tasks of Medication Technicians and care providers

  • Make appropriate situational decisions and manage conflict in a positive and constructive manner

  • Update and review resident medical records, clinical documentation, and incident reports

MINIMUM QUALIFICATIONS

  • Active LVN or LPN license in the State in which this community is located

  • CPR Certified or ability and willingness to obtain within 30 days of hire

  • Minimum one year of medication administration experience preferred

  • Prior supervisory experience preferred

  • Must be able to communicate clearly in verbal and written English

  • Intermediate computer and mobile device skills

  • Professional, pleasant and team oriented attitude

  • Consistent and reliable attendance is essential for all Belmont Village employees to ensure the successful operation of the community

Patient Care

  • Document timely, accurate, and relevant clinical notes reflecting patient condition, care provided, and pain/symptom outcomes.
  • Communicate with physicians regarding patient needs and changes in condition; obtain and carry out required orders.
  • Coordinate care with community partners and interdisciplinary team members to ensure continuity and quality.
  • Support emotional needs of patients and families; maintain a safe environment and uphold hospice philosophy.
  • Attend interdisciplinary group (IDG) meetings to support coordinated care planning.

Additional Responsibilities



  • Participate in on-call rotations per agency policy.
  • Ensure arrangements for medical equipment and necessary services.
  • Supervise ancillary personnel and delegate responsibilities appropriately.
  • Maintain professional growth through continuing education and in-services.
  • Fulfill all accepted case assignments.
  • Participate in quality assessment and performance improvement activities.

Qualifications



  • Graduate of an accredited school of nursing with 1–2 years of recent acute care experience and 1–2 years of recent hospice experience.
  • Current RN licensure in the state of practice and current CPR certification.
  • Bachelor’s degree preferred, with one year of hospice experience ideal.
  • Must be a licensed driver with a reliable, insured automobile.

  • Use and prepare equipment and supplies safely and effectively, consistently adhering to aseptic technique.
  • Deliver appropriate pain and symptom management; monitor patient response to treatments and medications.

Communication & Coordination



  • Document all care accurately and timely in alignment with the plan of care.
  • Participate in care coordination and promptly report needs requiring involvement from other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the RN and/or Clinical Care Manager.

Additional Responsibilities



  • Participate in ongoing personal and professional development.
  • Contribute to quality assessment and performance improvement (QAPI) activities.
  • Perform other duties as assigned by the Registered Nurse.

Qualifications



  • Graduate of an accredited practical nursing or vocational nursing program.
  • Minimum of three years of nursing experience preferred; home health, community-based, or medical-surgical experience ideal.
  • Current state LPN/LVN licensure.
  • Current CPR certification.

Essential Functions


Patient Care



  • Complete initial and ongoing comprehensive assessments of patients and families to determine hospice needs, including full physical assessments and history review.
  • Provide nursing care using all elements of the nursing process in accordance with the professional practice act.
  • Assess and evaluate patient status, initiate and update individualized plans of care, and revise as needs and conditions change.
  • Use health assessment data to determine nursing diagnoses and establish palliative, patient-centered goals.
  • Implement preventive and rehabilitative nursing procedures; administer medications and treatments as prescribed.
  • Counsel and educate patients and caregivers on care needs, symptom management, medications, diet, and safety.
  • Assist patients with activities of daily living and encourage independence and comfort whenever possible.
  • Serve as Case Manager when assigned, coordinating all aspects of care for the patient and family.

Communication & Coordination



  • Document timely, accurate, and relevant clinical notes reflecting patient condition, care provided, and pain/symptom outcomes.
  • Communicate with physicians regarding patient needs and changes in condition; obtain and carry out required orders.
  • Coordinate care with community partners and interdisciplinary team members to ensure continuity and quality.
  • Support emotional needs of patients and families; maintain a safe environment and uphold hospice philosophy.
  • Attend interdisciplinary group (IDG) meetings to support coordinated care planning.

Additional Responsibilities



  • Participate in on-call rotations per agency policy.
  • Ensure arrangements for medical equipment and necessary services.
  • Supervise ancillary personnel and delegate responsibilities appropriately.
  • Maintain professional growth through continuing education and in-services.
  • Fulfill all accepted case assignments.
  • Participate in quality assessment and performance improvement activities.

Qualifications



  • Graduate of an accredited school of nursing with 1–2 years of recent acute care experience
  • Current RN licensure in the state of practice and current CPR certification.

How you will change lives

As a Patient Care Technician (PCT) at US Renal Care, you will be an integral part of a cross-functional team providing care and delegated activities of treatment to patients. The PCT will work under the supervision of a Registered Nurse (RN) or Charge Nurse (CN) for patients with renal failure.


What you'll be doing

Patient care. You will work directly with patients to provide safe, comfortable, and high-quality dialysis treatment. Responsibilities include gathering patient stats before and after treatment, initiating treatment, monitoring patients during treatment, terminating treatment, and collecting patient blood samples per physician orders.


Technician Duties. You will ensure quality comes first by preparing machines for hemodialysis treatments, cleaning and disinfecting machines after treatment, and conducting machine safety checks and logging results for quality control, ensuring all protocols and regulations are followed.


Safety and Quality. You will use appropriate safety measures, including personal protective equipment. Working under the supervision of a Charge Nurse, you will adhere to all company policies, procedures, and state/federal laws and regulations. You will participate in all required staff meetings and continuing education offerings.

What we're looking for


  • High school graduate or equivalent.
  • Desire to work in healthcare and care for patients.
  • Previous healthcare experience in dialysis, phlebotomy, or as a CNA is a plus.
  • Must be comfortable mixing acids and bicarbonate according to protocol.
  • Ability to distinguish all primary colors.


Other requirements



  • Successful completion of USRC training program approved by the Medical Director, including demonstrated competency; this includes successful completion of USRC training course in the theory and practice of hemodialysis within 8 weeks of hire.
  • CPR certification required within 90 days of hire or as required by the state.
  • Patient Care Technician certification must be obtained through a state certification program or national commercially available certification program within 18 months of hire date and maintained in good standing thereafter. Specific state requirements may be applicable and required.

Nursing Team Roles: The focus of care is grounded in the primary value of Mayo Clinic, in that the needs of the patient come first. Nursing provides care in collaboration with the multidisciplinary team and engages the patient and family through participation in structured/scheduled family meetings, to enhance the patient experience and provide a congruent plan of care. The unit is staffed by a multidisciplinary team composed of members from respiratory therapy, physical therapy, occupational therapy, pharmacy, nursing, social work, and nutrition. Providers consist of Pulmonary/Critical Care Fellows, Critical Care Nurse Practitioners/Physician Assistants, and Critical Care Physicians from the Department of Medicine, Division of Pulmonary and Critical Care. 
 

The registered nurse (RN) is accountable for the coordination of nursing care, including direct patient care, patient/family education and transitions of care. The RN supports professional nursing practice across practice settings and across the continuum of care to meet the needs of the patient and family. The RN will function within the Mayo Clinic Nursing Professional Practice Model, which includes accountability for planning, implementing, evaluating and communicating all phases of nursing care for assigned patients. The ANA Nursing: Scope and Standards of Practice provide a basis for the practice of the RN. The RN provides leadership through activities such as preceptor role, informal and formal leadership roles, and quality improvement efforts. The RN delegates patient care according to skill level, experience, patient acuity, fiscal accountability and availability of resources. The RN possesses excellent communication skills; is skillful in mentoring and teaching; and may participate on committees or projects. A subset of employees may be required to drive their personal vehicle as a part of the responsibility of their role.

Qualifications

Graduate of a nursing program, as recognized by the Accreditation Commission for Education in Nursing (ACEN), Commission on Collegiate Nursing Education (CCNE), National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).  One year of RN experience required.  

All entry-level associate degree registered nurses with a RN start date of April 1, 2020, and after must provide documented evidence of program completion of the baccalaureate degree in nursing from an accredited nursing program within five years from the last day of the month of the RN start date.

1 year professional RN experience required

Excellent communication skills (verbal and written). Experience working in a team environment. Computer skills required, prior experience with electronic medical record systems preferred. Ability to work flexible hours, which may include days, evenings, nights, holidays, and weekends and on-call. Ability to adapt to unpredictable situations within the work setting. Demonstrated leadership, effective communicator, and excellent critical thinking skills.
