What You Will Do

  • Deliver expert palliative care services in collaboration with interdisciplinary teams.

  • Provide clinical oversight and consultation for patients and families.

  • Develop, coordinate, and evaluate palliative care programs to ensure continuous quality improvement.

  • Track patient outcomes and implement improvement plans when necessary.

  • Exercise sound clinical judgment and support transitions to hospice care when appropriate.

Minimum Qualifications

  • Graduate of an accredited nursing program.

  • Current RN licensure in the state of practice.

  • CPR certification required; Hospice/Palliative Care certification preferred.

  • Minimum of one year of home care or hospice experience with strong clinical knowledge.

  • Proficient computer skills and excellent interpersonal, organizational, and critical thinking abilities.

  • Ability to work independently and collaboratively within a team.
