Position Overview:

The Assistant Clinical Manager – Home Health, will evaluate nursing activities to ensure patient care, staff relations, and efficiency of service within the team. Act as a resource and support for clinical staff and facilitate collaborative teamwork on the unit and with ancillary departments. Oversee the Quality Program for the clinic and ensure that all standards are met, and keep procedural manuals and other necessary equipment up to date. This individual will serve as a backup to the Clinical Manager and will manage a caseload as needed for client coverage.

Essential Job Functions:

- Maintains clinical record documentation according to agency policy and procedures and state/federal regulation
- Ensures compliance with regulations and is available for federal and state licensure surveys
- Participates in Performance Improvement activities including but not limited to clinical record review, quality indicator monitoring, and quality management
- OASIS and/or coding experience is strongly desired
- Acts as a Preceptor to coordinate orientation and training for all RN Clinical new hires
- Provides ongoing training and educational opportunities for Clinical staff
- Serves as a senior member to all staff regarding quality improvement and documentation
- Oversees clinical orientation duties including documentation and quality improvement for all new health related employee
- Carry a caseload of patients, understand and perform the admission process plan of care
- Supervise RN, LPN, and HHA field staff as indicated.

Requirements:

- Active RN licensure in the state of the application
- Bachelor’s degree required

Under the general direction of the Director of Safety & Environmental Health, or designee, the Occupational Safety & Environmental Health Coordinator is responsible to lead initiatives related to incident response and reporting of workplace violence and staff occupational injuries. Conduct routine safety rounds in all facilities where CHS employees are located while identifying potential environmental safety hazards to the Department of Corrections (DOC) for mitigation. The Occupational Safety & Environmental Health Coordinator will engage with staff to identify safety gaps or environmental health concerns, provide on-going support and follow-up for staff involved in workplace violence incidents and/or who have suffered occupational injuries. This position serves as a central resource for safety education, incident response, and environmental risk mitigation across all CHS sites and services.

Key Responsibilities

  • Respond on-site to serious incidents of workplace violence, occupational injury, or environmental hazards, to provide support, gather factual information and develop a detailed report of findings.
  • Conduct post-incident well-being checks for employees who were involved in a workplace violence incident or who may have sustained a severe work-related injury.
  • Assist in maintaining workplace violence, occupational injury, and other safety-related databases and logs.
  • Compile workplace violence and staff injury reports for distribution to required entities.

Since 2016, Correctional Health Services (CHS) has been the direct provider of health care in the New York City jails. Position Overview

Under direct supervision of a licensed mental health professional, with latitude for independent judgment and decision making, performs professional work of ordinary difficulty in the mental health discipline. The following are typical tasks for Assignment Level I:

Examples of Typical Tasks:

  1. Provides mental health counseling and psycho-education services to clients with less complex cases.
  2. Participates in the interviewing and case history taking process by utilizing appropriate assessment and screening tools.
  3. Consults with relevant mental health professionals regarding the diagnoses, prognoses, and recommendations for disposition on individual cases; contributes to the formulation of client comprehensive treatment plans.
  4. Coordinates with appropriate personnel and/or departments to counsel and assist clients with the completion of applications and documentation for educational, vocational, and entitlement programs.
  5. Acts as a resource to clients seeking additional support on a broad variety of available services within the healthcare facility and the community.
  6. Prepares and maintains accurate and complete records and files of patient contact information which includes consumer interactions, clinical case histories, phone contacts, agency interactions, and other related information.
  7. Participates in appropriate internal and/or external training conferences.

1. Valid license and current registration to practice as a Mental Health Counselor issued by the NYSED and two (2) years of full-time, paid, professional experience providing direct client care in a behavioral health setting.

Assignment Level III

1. Valid license and current registration to practice as a Mental Health Counselor issued by the NYSED; and three (3) additional years of post-licensure experience.

DUTIES AND RESPONSIBILITIES

Financial and Business Systems Management

  • Understands and drives business metrics and performance measurements to ensure effectiveness, high performance and compliance.

  • Demonstrates the ability to improve the financial performance and profitability of the account

  • Understands the contractual agreement and recognizes ways to maximize opportunities

  • Demonstrates the ability to positively move the metrics for forecasting, productivity, claims, customer service, and turnover

Compensation: Towne Park is committed to offering competitive, fair, and commensurate compensation. Actual compensation will be based on a candidate’s job-related skills, experience, education or training, and location. The annual base pay range for this position is $70,304.00.

Additional Compensation: Employees may be eligible to receive annual incentive bonuses depending on their job classification and the policy guidelines.Β

Benefits: Employees are eligible to enroll in medical, dental, and vision insurance, accident insurance, critical illness insurance, hospital indemnity insurance, and telemedicine benefits.
