Key Responsibilities

Qualifications

Associate’s degree in nursing required bachelor’s degree in nursing preferred.  For candidates hired with an associate degree, employment with MedStar Washington Hospital Center is contingent upon enrollment into an accredited BSN program within one (1) year of employment and continuing matriculation in the program to achieve a BSN within 5 years of the program start date. Job Summary

You will conduct bedside assessments, identify high-risk medical and social needs, collaborate with hospital care teams, and coordinate timely, effective home health referrals. This role is essential to preventing avoidable rehospitalizations while delivering a compassionate, patient-centered experience. Essential Functions

Clinical Assessment & Care Coordination

• Conduct onsite hospital bedside assessments within 24 hours of referral.
• Integrate evidence-based clinical guidelines to develop patient-centered transition plans.
• Engage with patients, caregivers, case managers, physicians, and inpatient teams to gather key information for discharge planning.
• Identify high-risk medical and social determinants of health needs and communicate them to the care team.
• Schedule a follow-up primary care appointment within 3 days post-discharge.
• Complete follow-up phone calls within 48 hours of discharge and document CTN Follow-Up Coordination notes in HCHB.
• Support strategies to reduce home health rehospitalizations through proactive communication and interventions.

Documentation & EMR Responsibilities

• Document CTN coordination notes to support admitting home health clinicians.
• Complete workflow tasks and assignments specific to the CTN role in the EMR.
• Receive and enter verbal orders in HCHB from licensed practitioners and ensure physician approval.
• Follow up on pending referrals to support timely home health admissions.

Interdisciplinary Collaboration

• Participate in care coordination with agency staff, contractors, patients, and referral partners.
• Communicate effectively with all providers involved in a patient's plan of care.
• Educate patients and caregivers on engagement with the VitalCaring Connection (VCC) for virtual and telephonic care.
• Prepare for and participate in case conferences with other healthcare team members.

Professional Standards

Qualifications

Education & Licensure

• Graduate of an accredited nursing program (RN, LVN/LPN) or an accredited Physical Therapy program (PT).

• May require completion of HHS Computer-Based Training depending on license category.

Experience & Skills

• Minimum of two years of clinical experience as an RN, PT, LVN, or LPN.

As a Sr. Principal Systems Engineer - Missile Guidance, Navigation, & Control Engineer, you will play a critical role in helping with the evaluation, design, implementation, and integration of embedded software algorithms. You will collaborate with a team of engineers in an integrated product team that will be responsible for tactical embedded software, firmware, and integrated flight software 6 Degrees-of-Freedom simulations to achieve overall system performance requirements.  This position is an onsite role located in Tucson, AZ. 

What You Will Do

• Development, integration, and testing of embedded tactical missile flight code

• Definition and execution of system level performance studies and analyses to inform the development and integration of new capabilities, refinement of existing capability, or to address obsolescence

• Work within an agile framework to lead development of backlogs, plans, and metrics

• Provide experience and guidance to help the program train new engineers and execute efficiently and effectively

POSITION SUMMARY/RESPONSIBILITIES

As part of the Baby Bexar Healthy Families Community-Based Perinatal Health Initiative, this position will serve as liaison between health system providers and community based resources in order to improve maternal health outcomes for University Health patients. This position will work with a clinical care team and a team of social service agencies to assist and guide patients through prenatal, delivery and postpartum services. This position will provide screening services, education, support, and patient care coordination. Candidate will ensure appropriate information is disseminated, patient is navigated to medical appointments, and follow-up with health care needs. All functions and behaviors are applicable to the clinical and community settings. Additional activities will include patient tracking, data entry, and reporting.

EDUCATION/EXPERIENCE

Bachelor's degree in social sciences, health education or related field of study is required. Four years of experience in the area of health education, public health, social work and/or case management may be considered in lieu of the Bachelor's degree. Community Health Worker certification preferred. Knowledge and experience working with programs of similar scope and focus preferred. Spanish fluency is preferred.
