Responsibilities:The Nursing Practice and Quality Specialist serves as a clinical and operational expert in advancing nursing practice, patient safety, and quality outcomes. Guided by the NAHQ Healthcare Quality Competency Framework, this role supports the design, implementation, and evaluation of nursing practice standards, policies, and quality improvement initiatives that foster excellence in patient care, regulatory compliance, and continuous learning.

Performance and Process Improvement
- Lead and facilitate interdisciplinary quality improvement (QI) projects using the PDSA methodology.
- Will represent a multi hospital view of system wide standardized processes and practices
- Analyze clinical processes, identify performance gaps, and implement improvement plans that enhance outcomes and efficiency.
- Monitor progress through data collection, dashboards, and feedback loops to ensure sustainability of improvements.
- Collaborates with providers, nurses and other clinical disciplines in the implementation, compliance, and evaluation of evidence-based clinical standards.
- Leads and participates in taskforces, committees or councils related to process improvement and provision of quality, evidence-based care.
- Uses Lean tools and principles to improve quality of workflow and patient care.

- Collaborate with leaders and frontline staff to implement corrective action plans and sustain compliance.
Health Data Analytics and Reporting
- Partner with data analysts to transform complex data into actionable insights for nurse leaders, providers, and staff.
- Support data-driven decision-making through accurate, timely performance measurement and reporting.
Population Health and Care Coordination
- Integrate population health principles into nursing practice to reduce health disparities and improve outcomes across the continuum.
- Contribute to initiatives that improve patient experience and outcomes across care settings.
Professional Practice Development
- Mentor and educate nurses on quality science, evidence-based practice, and outcomes measurement.
- Assists in developing and revising standards of care and practice to reflect national and regional standards, local practice trends, and current research
- Collaborates with nursing leadership, Legacy Research Institute, Nursing Professional Development Specialists and nursing staff in identifying opportunities for nursing clinical inquiry.
- Maintains currency in clinical practice and knowledge of professional standards of practice through participation in continuing education, community and professional activities, and committee membership.
Education:
- An academic degree in nursing is required (BSN or MSN/MN); master’s degree required.
Experience:
- Four years relevant clinical experience at a proficient to expert level of practice required with demonstrated involvement in quality or performance improvement.
- Preferred: Experience with regulatory/accreditation readiness, patient safety, and data analytics.

Qualifications
A degree in Respiratory Care from an accredited respiratory care program is required; minimum associate degree or equivalent as defined by the National Board for Respiratory Care (NBRC).

What You Will Do
Monitor and follow patient progress and report health changes to supervising registered professional nurse through detailed observations, interviews, and physical/psychosocial care within LPN scope of practice.
Deliver personalized nursing and care management by constantly evaluating the evolving patient needs and adjusting the plan of care.
Carry out quality-driven and care coordination by communicating within the VNS Health interdisciplinary team and across providers and other resources to deliver the plan of care effectively and efficiently.
Practice in the community, traveling to patients’ homes or care facilities, using effective time management and agility to manage weekly caseload and other duties.
Use agility and empathy to deliver continuity of patient care including coordinating supplies and providing education.
Use technology, including a tablet computer, phone, and other systems to coordinate and maintain accurate and up-to-date electronic medical records (EMRs) for patients in the community.
Provide empowering education and support to patients and their families to help them understand and navigate end of life treatment.
Qualifications
Licenses and Certifications:
License and current registration to practice as an LPN in New York State required
Valid driver's license may be required as determined by operational/regional needs
Education:
High School Diploma or completion of LPN Program required
Work Experience:
Minimum of one year work experience as an LPN preferred
Bilingual skills may be required as determined by operational needs
Home care/hospice experience preferred
Pay Range per Visit:
If you are applying to the per diem per visit version of this job, the hiring range is as follows: $50.68- $54.07 per visit.

Therapeutic Certified Recreational Therapist
Part Time Position
Range: $25 - $32 per hour

- Provide therapeutic recreation-based treatment program opportunities, including individual and group therapy, in a safe, effective manner to all program participants.
- Complete assessment/evaluation in the areas of Recreational Therapy upon admission for all program participants.
- Work directly with paraprofessional staff to facilitate training and program participant growth in the areas of Recreational Therapy.
- Assist in the development of the rehabilitation plan of care to meet the needs of each program participant.
- Communicate with the program participant, family, and funders as needed regarding participant status and need of services.
- Remain current and compliant with federal, state, and accrediting body regulations and standards.
- Adhere to work practices consistent with Mentor mission, policies, and procedures.
- Adhere to code of ethics, standards of practice, and continuing education requirements of the American Therapeutic Recreation Association (ATRA).
- Procure and maintain all recreational equipment to assure safe and appropriate use by program participants and staff.
- Advocate and educate staff, program participants, and families in the areas of recreational therapy and therapeutic recreation.
- Maintain best practice methods through continued education coursework.
Qualifications:
- Bachelor’s Degree in Recreational Therapy.
- Two years of experience in a rehabilitation environment and/or work with persons with neurological and developmental deficits.
- Certification of CTRS (Certified Therapeutic Recreation Specialist).
- Knowledge of rehabilitative techniques relative to recreation for persons with diverse neurological impairment.
- Ability to interpret evaluations from various disciplines (Neuropsychology, Occupational/Speech/Physical Therapies) and understand the relevance to your practice.
- Ability to handle stressful situations in a calm, professional manner. Ability to exercise sound judgment and discretion.
- Exceptional communication skills with an ability to establish trust and rapport quickly.
- A good listener with an ability to empathize while still providing guidance.

A Hospice Registered Nurse (RN) provides compassionate end-of-life care to patients in their homes or hospice settings, focusing on comfort, pain management, and emotional support. Hospice RNs assess patient needs, coordinate care plans, and educate families to ensure patients experience dignity and peace in their final stages. They collaborate with interdisciplinary teams to provide holistic care while upholding hospice philosophy and regulatory standards.
Essential Work Functions:- Assess patient needs and develop individualized hospice care plans in collaboration with physicians and care teams
- Administer pain management interventions, medications, and treatments to ensure patient comfort
- Educate and support families and caregivers, helping them understand symptom management and end-of-life care
- Monitor changes in patient condition, adjusting interventions to maintain quality of life
- Coordinate care with hospice physicians, social workers, chaplains, and other team members to provide holistic support
- Document all care provided, symptom changes, and family interactions accurately and in compliance with hospice regulations
- Ensure adherence to hospice best practices, safety protocols, and regulatory requirements
- Offer emotional and psychological support to both patients and their loved ones during the end-of-life journey
- Communicate regularly with the hospice care team to ensure continuous, compassionate care
- Complete charting and documentation requirements while meeting visit and productivity goals
- Adhere to infection control, safety, and healthcare regulations
- Perform other duties as assigned within the scope of practice
