Position Highlights: CT7B Advanced Care
- Number of beds on unit: 24
- Patient population (age, diagnosis, specifics about unit): The Advanced Care Unit is a 24-bed unit specializing in the care of acutely unstable patients that do not meet criteria for Intensive Care status but still require frequent, advanced assessments and interventions. Specific patient populations or conditions that require Advanced Care status include complex multi-trauma, uncompensated sepsis, continuous non-invasive ventilation, stroke, complicated withdrawal from alcohol or other recreational drugs, and DKA. The unit also includes an Epilepsy Monitoring Unit (EMU) where patients with seizure-like events can be safely taken off medication to purposefully induce seizure-like events and provide vital information to the Epilepsy Team about how to treat them.
- Nurse to Patient ratio: 1:3
- Department culture and resources: The Advanced Care Unit is a fast-pace, team-oriented unit. We work closely with our physician partners and ancillary team members to achieve the best outcomes for our patients. We have a robust Partnership Committee that meets monthly. We have a unit-based educator, a quality outcomes coordinator, 2 nurse supervisors and charge nurses.
- Frequent nursing skills/equipment used: Minimum Q4 hour full RN assessments. Minimum Q2 hour vital signs. Continuous cardiac monitoring by primary RN, Foley/IV/NG insertions, arterial line/central line/chest tube monitoring and care, IV pumps, feeding pumps, lift equipment. Commonly used IV drips: amiodarone, diltiazem, insulin for DKA, dobutamine, heparin, octreotide.

The Advanced Care Team Clinician provides clinical insight and education to patients, as well as patient instruction on the safe and appropriate use of home respiratory and other equipment. In addition, the ACT Clinician maintains knowledge of equipment and services, remains compliant with all applicable rules and regulations and supports the continuum of care for patients.
ESSENTIAL FUNCTIONS:
- Educates patients/family/caregivers which may include explaining diagnosis,personal care, symptom recognition, equipment uses and care and other relevant information via phone call, Telehealth Visits and in person as needed.
- Completes daily tasks of general follow-up with patients, NIV follow ups and phone calls to the patient/family/caregiver as needed.
- Completes NIV Setups and NIV home visits as needed, participates in on-call as needed.
- Supports the continuum of care by creating and/or overseeing care plans, communicating with the physician or other clinicians, identifying, and resolving patient care issues whenever possible.
- Maintains current and thorough knowledge of equipment and services. May assist with quality control, maintenance of equipment, and general support of daily Clinical Outcomes Team operations
- Maintains current understanding of insurance benefits, coverage, and patient costs to adequately explain to patient and/or caregivers.
- Maintains compliance with HIPAA, The Joint Commission, State Respiratory CareBoard, State Pharmacy Board and all other applicable rules and regulations.

Unit Description:
The Skilled Nursing Facility (SNF) at Champlain Valley Physicians Hospital is located on our 5th floor and accommodates 34 residents. Our facility serves a diverse range of patients, spanning from middle-aged adults to geriatric residents. We maintain a balance between long-term care and subacute care, striving for a ratio of 14 short-stay patients to 20 long-term residents. Our short-stay patients typically stay with us for 1 to 6 weeks on average, receiving specialized care for conditions such as sepsis, post-orthopedic surgery recovery, and wound care. For our subacute population, we provide a spectrum of skilled nursing and therapy services, including wound care with advanced techniques like wound vacuums, antibiotic therapy, and comprehensive physical, occupational, and speech therapy. Our long-term residents exhibit a wide range of needs, from those requiring minimal assistance with daily activities to others who rely completely on our care and support. Qualifications Required Education: Associate degree in nursing.
Preferred Education: Bachelor's degree in nursing.
Required Experience: None.
Preferred Experience: Two (2) or more years of RN experience.
Required Certification/License/Registry: Licensed as an RN in accordance with Floridaβs Nurse Practice Act.
Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification from an accredited American Heart Association (AHA) instructor.
Other specialized life support requirements designated by work area.

Uses evidenced-based practice and information technologies while maintaining focus on providing comfort, relieving pain, and alleviating anxiety.
Practices under the Department of Nursingβs umbrella and within the scope and conditions of the Florida Nurse Practice Act (Florida Statutes (FS), Title XXXII (Regulation of Professions and Occupations), Chapter 464 (Nursing), Part I).
