What You'll Do as a Hospice RN / RN Case Manager:
Be the primary point of contact for patients and families, guiding them through every step of their hospice journey.
Deliver and document skilled, hands-on nursing care based on each patientβs individualized Plan of Careβin their home, assisted living, or wherever they call home.
Perform initial and ongoing assessments to monitor patient condition and adjust care as needed.
Collaborate with an interdisciplinary teamβincluding physicians, LPNs, CNAs, social workers, chaplains, and othersβto build and refine personalized care plans.
Provide emotional, physical, and spiritual support not just to the patient, but to their loved ones as wellβoffering education, comfort, and bereavement guidance.
Teach caregivers how to care for their loved one safely and confidently, offering both practical training and emotional reassurance.
Communicate important updates in patient condition to the care team, ensuring timely and appropriate interventions.
Maintain accurate, up-to-date clinical documentation and follow infection control and compliance standards.
Support the organization's quality improvement and scheduling initiatives to ensure care is consistent, compassionate, and efficient.
Help patients and families understand and navigate topics such as:

Qualifications β What Youβll Bring:
- Active RN licenseΒ in the state of employment (or eligible to obtain).
- 1+ year of RN experienceΒ in hospice, home health, ICU, oncology, geriatrics, or related nursing fields.
- A genuineΒ hospice heart: compassionate, empathetic, and patient-centered.
- Comfortable providing care in diverse settings, including private homes and facilities.
- Strong communication and critical thinking skills in emotionally sensitive situations.
- ValidΒ driverβs license, auto insurance, and reliable transportation.
- CPR certificationΒ required.

In this role, you will ensure that intake and referral processes are completed efficiently and effectively to meet the needs of patients, families, referral sources, and the organization. Essential Responsibilities
- Receive and respond to referrals from hospitals, home health, physicians, case managers, skilled nursing facilities, and other healthcare professionals
- Assess patient eligibility for hospice services by reviewing medical records, conducting clinical evaluations, and communicating with physicians and families
- Educate patients and families on what hospice care includes, how it works, and why it mattersβproviding clarity, reassurance, and compassion
- Complete necessary documentation and coordinate the intake and admission process, ensuring all legal, clinical, and payer requirements are met
- Serve as a liaison between referral sources and our hospice care teamβensuring seamless handoffs, clear communication, and rapid response to needs
- Make daily visits to hospitals and facilities, maintaining visibility and accessibility with discharge planners, case managers, and physicians
- Build and maintain strong relationships with referral partners and identify new outreach opportunities to increase awareness of hospice services
About You
Education
- Licensed Nurse with three to five years of hospice experience required.
- BSN with five or more years of hospice experience preferred.
Experience
- Minimum of three years of hospice experience required.
- Prior experience as a discharge planner, case manager, or educator preferred.
Licenses and Certifications

Position Profile
RN case managers have the primary responsibility to provide and coordinate hospice care for home care patients. They give care and oversee the interdisciplinary hospice team to make sure patients' physical, emotional and spiritual needs are met. RN case managers educate patients and families about the disease process, help manage and control symptoms and offer support and strength during difficult times. Care delivery primarily takes place in the patient's home or in a facility where the patient receives care.

Responsibilities
Enhabit Hospice RNs work with patients, their families, and other health care professionals during the final stages of patients' lives. Our nurses work in private homes, residential care facilities, nursing centers, and other hospice care environments. They may also supervise licensed vocational/practical nurses and nursing aides. Our nurses must be capable of compassionate communication with patients and families, have keen observation skills, high ethical standards, and knowledge of when to alert doctors and others about changes in patients' conditions.
Qualifications
* Must be a graduate of an approved school of nursing and licensed as a Registered Nurse in the state of employment
* Must possess a valid state driver's license, automobile liability insurance, and have dependable transportation
* Minimum of one year of clinical experience is strongly preferred

How You Will Contribute
The Hospice Aide provides compassionate personal care and supportive services to patients in their place of residence. This role assists individuals who are nearing end of life, helping maintain dignity, comfort, and quality of life. The Hospice Aide is an essential member of the care team and delivers care in alignment with the patientβs individualized Plan of Care.

How You Will Make an Impact
β’ Provide assistance with at least 2 of the following ADL's including personal hygiene
β’ Assist patients with medication intake and prescribed exercises
β’ Incidental household services, which are essential to patient health care
β’ Will follow the Plan of Care (Instruction Sheet) as instructed and make appropriate updates during scheduled visits
β’ Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care

The Experience and Skills You Bring
- Must be a certified nurseβs aide (CNA). State Tested Nursing Assistant (STNA) for Ohio
- Must maintain a valid driver's license, good driving record and reliable automobile
- High school graduates or GED preferred
- Demonstrate the ability to be able to work within a multi-disciplinary team setting
- Demonstrate the ability to work in field situations
Preferred Knowledge, Skills, and Experience
- Preferred candidates will have in-home and/or hospice experienceΒ
