What You Get To Do:

  • Assist with all aspects of Polyschool training events.
  • Establish and organize the products and quantities for items required to perform Polyschool events.
  • Ensure all needed material, supplies, tools, and any other products necessary are ordered and delivered to the appropriate location within the required time frames.
  • Work with, supervise and instruct additional Technical staff needed to support Polyschool events.
  • Prepare all demonstration decks/mock-ups.
  • Execute hands-on demonstrations for Polyschool audience with the assistance of Technical staff member(s).
  • Ensure all used materials are disposed of appropriately and all supplies, materials and equipment are packed and returned to an appropriate location.
  • Assist Sales Team in any related sales or promotional activities as directed by the Manager.
  • Travel to other territories to assist as needed.
  • Represent Polyglass at appropriate industry functions and events, such as association events and conferences, and provide feedback and information on market and creative trends.
  • Understand all local code regulations and industry practices governing roofing activities.
  • Understand customer base in a territory as far as key individuals, contractor capabilities, and competitor systems used.
  • Plan ahead and effectively communicate activities to all functional groups and Managers.
  • Follow with and abide by all regulations, policies, work procedures, safety rules, and instructions.
  • Work with the Polyschool Training Manager to constantly update current and work on new training presentations.
  • Actively assist in continuous improvement activities and offer positive suggestions for improvements and innovations.
  • Professionally answer queries providing Polyglass solutions.
  • Work proactively to promote teamwork at all levels of the Company.
  • Manage resources to execute assigned programs.
  • Be a self-starter and work independently..
  • Ability to travel as required of the job


What We Look For:

  • High School diploma required
  • This position requires the ability to work with a personal laptop in a Windows environment.

Summary:

The CVOR registered nurse is responsible for providing direct and indirect nursing care to assigned patients, as well as assisting in monitoring and supporting the patient throughout the surgical procedure. They collaborate closely with the entire surgical team to ensure the optimization of patient outcomes by confirming all necessary surgical supplies and equipment are sterile and functional, assisting with safe patient positioning and draping, and coordinating immediate post-surgical continued patient care.

Essential Work Functions:
  • Maintain proficiency in Cardiovascular/Cardiothoracic surgical procedures, including open and closed heart surgeries
  • Prepare the Cardiovascular operating room for the patient and surgical procedure
  • Assemble equipment, supplies, medications, and implants, ensuring equipment functions and supplies are available
  • Collaborate with surgeons, anesthesiologists, perfusionists, and the surgical team to deliver coordinated care
  • Utilize data from surgical preference cards for the surgical procedure and surgeon
  • Position patients safely using approved devices to prevent injury and enable optimal surgical exposure
  • Actively participate in Universal Protocol and timeout to confirm patient identity, procedure, and surgical site
  • Handle and document specimens accurately to ensure diagnostic integrity
  • Provide post-surgical care coordination, including patient handoff to the recovery team
  • Assess, plan, and evaluate the nursing care of the patient before, during, and after a surgical procedure until patient handoff to the next phase of care
  • Ensure that sterile procedures are followed throughout the course of surgery
  • Respond effectively to emergencies, such as cardiac arrest, during surgery
  • Perform other duties as assigned within the scope of the position
  • Adhere to hospital safety protocols, infection control guidelines, and regulatory standards
Required Essential Skills:
  • Critical thinking, service excellence, and good interpersonal communication skills
  • The ability to read, write, and communicate in the English language
  • Ability to read/comprehend written instructions, follow verbal instructions, and proficiency in PC skills

Job Summary and Qualifications

The primary purpose of the Lead Case Manager is to ensure that primary operations of the Case Management/Utilization Review Department function efficiently. This position maintains a caseload or equivalent assignment as volume dictates while providing a first line resource to Case Managers, Utilization Review, and Medical Social Workers. This position routinely dialogs with the Manager and/or Director to ensure seamless front line communication.

  • Performs a comprehensive assessment of psychosocial and medical needs of assigned patients, in collaboration with the assigned Treatment Coordinator.
  • Acts as coordinator of patient/family/caregiver education and monitors documentation of education by team members. Promotes participation of the patient/family/caregiver in team discussions related to plans, goals and status through Family Conferences and other interactions.
  • Ensures implementation of an individual treatment plan that supports patient strengths, abilities, needs, and preferences. Directs activities of the Patient Care Conference. Coordinates team activities in the implementation of patient treatment plan and re-assesses treatment plan after a change in patient condition. Facilitates the involvement of the patient throughout the rehabilitation process.
  • Documents findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments. Communicates information of social, financial or discharge needs and preferences of the patient/family/caregiver. Uses financial information available in decision making about the provision of services for the patient.
  • Ensures communication with external and internal sources. Interacts with patients, team members and other stakeholders.
  • Provides thorough verbal and/or printed information to help patients/families/caregivers make informed decisions about post-acute care options, while addressing goals of care and treatment preferences. Provides printed tools explaining how to access additional details from website.
  • Adheres to hospital policy when making post discharge referrals, documenting the process in the patient record. Provides Patient Choice letter and full list of post-acute providers in patient's designated area.
  • Provides or makes appropriate referral for individual, family, group, or sexual counseling as needed. Makes appropriate peer support referrals within the community prior to discharge.
  • Facilitates discharge and arrangement of follow-up services. Facilitates implementation of discharge/transition recommendations. Identifies and utilizes appropriate community resources to meet patient discharge and continuity of care needs.
  • Participates in quality improvement activities. Assists in the collection and analysis of utilization data, identification of improvement areas, and improvement plan implementation.
  • Assumes accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.
  • Maintains a patient-first philosophy and engages in service recovery when necessary.
  • Supports the development and implementation of strategies to elevate the patient experience.
