Our Physical Therapists examine and treat patients with physical impairments through the use of physical modalities. The goal is to assist persons who are physically challenged to improve mobility and function, independent self-care, other skills necessary for functioning in daily living. We have Specialty Programs that include Balance and Fall Prevention, Spine Safety, Total Hip and Knee Replacement Program, and our Total Shoulder Replacement Program. He/she administers skilled care to clients requiring intermittent professional services and teaches the family and other members of the health care team. In addition to performing visits and completing coordination of client care, the Registered Nurse RN Home Health is the point of contact for all disciplines involved with providing care to patients and oversees the frequency of visits for the episode. The Registered Nurse RN consults as needed with the physician and the office giving details about patient care. Patient Assessment & Monitoring
- Perform comprehensive assessments for pre- and post-transplant patients.
- Monitor vital signs, graft function, fluid/electrolyte balance, lab values, and indicators of organ rejection or infection.
- Recognize and respond to early symptoms of transplant complications, such as rejection, sepsis, or medication toxicity.
Transplant-Specific Care
- Administer immunosuppressive therapies, antirejection medications, and other transplant-specific treatments.
- Provide wound care, infection prevention, and management of surgical drains as applicable.
- Support coordination of biopsies, imaging, dialysis, and other procedures related to organ function.
Critical Interventions
- Respond promptly to emergent situations including acute rejection episodes, respiratory distress, or hemodynamic instability.
- Operate specialized equipment such as infusion pumps, telemetry monitors, and devices used in managing immunosuppressive drug therapy.
Collaboration & Communication
- Partner closely with transplant surgeons, nephrologists/hepatologists, pharmacists, social workers, dietitians, and transplant coordinators to ensure seamless care.
- Provide extensive teaching to patients and families regarding medications, lifestyle modifications, post-transplant surveillance, and long-term follow-up expectations.

EP 6.5- Inpatient post-op recovery unit. The complex patient population is primarily colorectal and comprises a multitude of other services including plastic surgery, urology, trauma, emergency general surgery and orthopedics, as well as medicine.
EP 6.4- Surgical unit that has mixed acuity including both step down and floor level beds. The unit has the capacity to see a diverse population of surgical, orthopedic, neurology, neurosurgery, plastics, general surgery and trauma patients
Saint Raphael Campus (SRC):
Main 6- Specializing in bariatric, minimally invasive, and colo-rectal surgery. There is also exposure to medicine and general surgery overflow.
SLA 2- Surgical telemetry unit caring for both medicine and surgical populations including podiatry, vascular and general medicine overflow.
V3S- Diverse clinical population including urology, general surgery, OMF, ENT, plastics, vascular, and medicine overflow.

Patient Assessment & Monitoring
- Perform comprehensive assessments for pre- and post-transplant patients.
- Monitor vital signs, graft function, fluid/electrolyte balance, lab values, and indicators of organ rejection or infection.
- Recognize and respond to early symptoms of transplant complications, such as rejection, sepsis, or medication toxicity.
Transplant-Specific Care
- Administer immunosuppressive therapies, antirejection medications, and other transplant-specific treatments.
- Provide wound care, infection prevention, and management of surgical drains as applicable.
- Support coordination of biopsies, imaging, dialysis, and other procedures related to organ function.
Critical Interventions
- Respond promptly to emergent situations including acute rejection episodes, respiratory distress, or hemodynamic instability.
- Operate specialized equipment such as infusion pumps, telemetry monitors, and devices used in managing immunosuppressive drug therapy.
Collaboration & Communication
- Partner closely with transplant surgeons, nephrologists/hepatologists, pharmacists, social workers, dietitians, and transplant coordinators to ensure seamless care.
- Provide extensive teaching to patients and families regarding medications, lifestyle modifications, post-transplant surveillance, and long-term follow-up expectations.
- Participate in multidisciplinary rounds and contribute to individualized care planning.
