Job Description:
Our Nurses work in collaborative environments with strong interdisciplinary teams, focused on a patient centered care model in a richly diverse work space. Job Summary:
The Connecticut Orthopedic Institute and Ayer Neuroscience Institute are highly dedicated to improving the lives of our patients. The Nurse Navigator serves as a patient advocate, liaison, and advisor/educator to guide patients throughout the continuum of care for their surgical experience beginning in the pre-operative phase up to a full year post-operatively to mitigate risks and promote positive outcomes. This position builds relationships with patients, families and the internal clinical team to improve patient outcomes, satisfaction and transitions through the continuum of care.
Job Responsibilities:
· Serves as a member of the multidisciplinary care team, functioning as a patient advocate, educator, key point of contact and liaison between the patient and all other appropriate services.

Clinical Supervisor- Hospice

In general, most of the time will be spent in the following activities:

Assures the development of personnel qualifications and policies
Identifies and facilitates professional development needs and competency of staff through home visits with staff to supervise and evaluate clinical performance; identifies and follows through with development plan
Maintains compliance with regulatory agency policies and procedures by providing staff guidance in making clinical and case management decisions that are cost effective and focused on clinical outcomes
Accountable for team performance in achieving desired clinical and operational performance measures
Utilizes clinical software to identify opportunities for education, most appropriate utilization of services, oversee clinical progression of patients and trend team outcomes and performance.
Assures that clinical and functional outcomes are achieved at the highest level possible
Resolves patient care problems collaboratively with internal/external customers – collaborates with peers across the regions
Oversees the daily patient assignments for clinical staff based on client needs and staff competency to maintain team productivity standards; LEAN Daily Management process (Visual Boards)
Coordinates referrals and ensures timeliness of SOCs and timeliness of adjunct service delivery
Ensures that patient needs are continually assessed
Ensures that an individualized plan of care is developed, implemented and updated as needed for all patients.

The Float Pool at Hartford Hospital, also known as the STAR Team, is comprised of experienced, highly skilled Nursing professionals. Registered Nurses on the STAR Team have a variety of skills, allowing them to transition from one specialty to another with confidence and ease. Our team is broken down into two groups, Med/Surg and Critical Care. The Med/Surg team floats between approximately 18 units (including the Step Down units) while our Critical Care RNs primarily float between the six ICUs at Hartford Hospital (MICU, SICU, Cardiac ICU, Neuro ICU and Cardiothoracic ICU). The Surgical-Trauma Intensive Care Unit is a 14-bed unit. As a Level 1 Trauma Center, the mainstream patient population includes motor vehicle trauma, orthopedic injuries, vascular surgical, abdominal surgical, liver and kidney transplant, and extensive esophagectomy patients. Under the supervision of the rounding Surgical-Trauma Intensivist, the unit has a team of Physician Assistants, Advanced Practice Registered Nurses, Registered Nurses and surgical house staff physicians consisting of interns and residents.

The Nursing Manager is a Registered Nurse (RN) who has responsibility for the supervision of patient care, clinical personnel, and unit operations. This position supervises a work group to ensure that a high quality, cost-effective patient and family centered care is provided in support of the hospital's strategic plan, and the function's goals and objectives.
This position works collaboratively with functional leadership and is responsible for the oversight of daily operations of the assigned department including off shift and weekend supervision of nursing and non-licensed staff. This position manages resources, quality improvement, payroll, creating and maintaining colleague schedules and monitors patient throughput to facilitate efficient clinical operations. In addition, this position is responsible for a safe and therapeutic unit environment, which includes working in the department as staff RN to assist as needed.

Job Description
· Perform comprehensive assessments to establish an individualized care plan.
· Facilitate communication between the patient and the interdisciplinary team.
· Develop and maintain a therapeutic relationship by providing education and promoting self-advocacy and independence.
· Utilize state-of-the-art technology to document skilled services provided.
Qualifications
- Registered Nurse with Valid CT RN license
- One year of RN experience; Homecare experience preferred.
- Valid driver’s license and reliable transportation required.
- Computer proficiency required.
