Job Summary:

The Registered Nurse in the Centralized Monitoring Unit (CMU) plays a key role in supporting the safe and efficient monitoring of nearly 300 patients’ cardiac rhythms and hemodynamic status. This is a non-direct care position, ideal for a clinically skilled RN with experience in telemetry who enjoys working in a fast-paced, technologically advanced environment.

The RN in this role provides clinical support to a team of Central Monitor Technicians (CMTs), promotes adherence to protocols, and facilitates clear communication with inpatient care teams. This position contributes to the overall quality of care by helping ensure accurate cardiac monitoring, appropriate escalations, and consistent workflow.


Job Responsibilities:

· Support and coordinate the daily activities of Central Monitor Technicians to ensure timely and accurate telemetry monitoring.

· Serve as a clinical resource for rhythm interpretation, protocol adherence, and telemetry best practices.

· Collaborate with the CMU Nurse Educator to help provide training and reinforce department policies and procedures.

· Support smooth transitions between shifts to maintain continuity of patient monitoring.

· Participate in daily team huddles and contribute to unit-based initiatives.

· Act as a communication link between the CMU and inpatient clinical staff, promoting timely updates and coordination.

· Assist in reviewing staffing assignments and making adjustments as needed.

Job Summary
The Cardiac Catheterization lab at Hartford Hospital is comprised of four suites, two monitoring rooms, two scrub areas, a supply room and a holding area. There are approximately 5,000 diagnostic cardiac caths performed per year and of these, 1,500 become interventions. Hartford Hospital’s operating room provides 24-hour backup in the event that open-heart surgery is needed.

Care of patients undergoing a variety of Catheterization and Interventional procedures with conscious sedation/analgesia administration. Familiarity with IABP, cardiac drugs, and emergency care. Provides holistic, quality and cost-effective nursing care for patients/families and/or others for a patient population within a unit, service or other primary area of practice with an awareness of the episode of care. Our Nurses work in collaborative environments with strong interdisciplinary teams, focused on a patient centered care model in a richly diverse work space. Under the direction and general supervision of the Nurse Manager, the registered nurse utilizes evidence based practices in accordance with the State Nurse Practice Act, policies and procedures of the hospital and as directed by the medical staff. Job Summary
Our Nurses work in collaborative environments with strong interdisciplinary teams, focused on a patient centered care model in a richly diverse work space. Under the direction and general supervision of the Nurse Manager, the registered nurse utilizes evidence based practices in accordance with the State Nurse Practice Act, policies and procedures of the hospital and as directed by the medical staff. The registered nurse who through knowledge and ability uses the nursing process to develop a plan of care which includes standards of practice to meet the physical, emotional spiritual, cultural and educational needs of the patient and family.

The Cardiac Rehabilitation Nurse will have expertise in the cardiovascular specialty areas. Their role will integrate the components of clinician, teacher, consultant and researcher into direct patient care.

Responsibilities:
• Completes Initial Assessment for patients referred to the outpatient cardiac
rehabilitation program
• Identifies indications for participation, contraindications
for participation and specific monitoring needs and risk factor modification
goals for each patient.
• Completes patient education/counseling and documents treatment plan, including measureable goals, tailored to the individualized goals/needs of each patient referred to the program.
• Evaluates, monitors and documents multiple patients response to exercise therapy prior to, during and after exercise therapy. This ongoing assessment includes blood pressure, heart rate, rate of perceived exertion, telemetry/EKG, and may also include oxygen saturation, daily weight, blood sugar and ventricular assist devices
• Reports any significant participant acute conditions (e.g: dysrhythmias, blood pressure, angina symptoms, weight gain) to the supervising physician immediately. Reports ongoing, abnormal trends to physician/provider as needed (ex: elevated blood pressure; abnormally low blood pressure, episodes of hypo/hyperglycemia) via event report.
• Initiates ACLS measures and/or emergency care according to hospital policy.
• Completes tasks related to patient recruitment, insurance pre-certification, registration, and records management (Scott Care, Epic) as needed.
• Keeps current in the field of cardiac rehabilitation through journals, in-services, mock codes, conferences and staff meetings
 



Qualifications

Qualifications:

  • Current Connecticut State RN License
    • Current BLS/ACLS
    • BSN required; MSN preferred
    • Previous Cardiac Rehab experience preferred with knowledge of exercise therapy
    • Previous experience in a cardiovascular clinical area e.g.: Coronary Care, Cardiac Rehab, Congestive Heart Failure-Two years of recent and relevant experience preferred
    • Meets the skills criteria for Cardiac Rehab Nurse Competencies.
    • Capable of meeting the physical and emotional demands of the position
    • Desire to work within a comprehensive health care team

The Greater Manchester Region has approximately 2,500 employees. The Nursing Professional Development (NPD) practitioner is a Register Nurse with NPD practice judgement and expertise who influence professional role competence and growth of learners. The NPD practitioner is responsible for supporting learners within the context and framework of the Nursing Professional Development, Scope and Standards practice as approved by the American Nurses Association. This position works in collaboration with nursing leadership to foster and influence competency management, clinical nursing education, professional role development, policy standardization, collaborative partnerships, scientific inquiry, transition to practice and onboarding/orientation of new Hartford HealthCare colleagues. The practitioner is an essential member of the nursing leadership team who promotes the alignment of regional NPD and practice excellence with the HHC system as a whole.

 

Knowledge, Skills and Ability Requirements 

 

·        Demonstrates ability to develop, deliver and evaluate educational programs.

·        Working knowledge of high reliability and LEAN process improvement strategies.

·        Demonstrated leadership skills and a commitment to education as a methodology for promoting continuous quality improvement in the delivery of patient care and services.

·        Knowledge of the principles/theories of adult learning.

·        Knowledge of unit based orientation plans.

·        Strong skills in the process of educational needs assessments.

·        Excellent communication skills, both verbal and written.

·        Ability to manage multiple priorities in a fast-paced environment.

·        Must be able to work independently as well as part of a collaborative team.

·        Proficient in Microsoft Word, PowerPoint and strong skills in excel.

Key areas of responsibility

Learning Facilitator: Uses the educational design process and adult learning principles to bridge the knowledge, skills and/or practice gaps identified through learning needs assessment and evaluation of outcomes.

 

Collaborative Partner: Collaborates with members of the nursing leadership team to assess learning needs, develops programs, monitors clinical competency and supports action plans. Represents nursing in the development of inter-professional learning activities.

 

Role Development: Promotes professional nursing practice and quality patient care according to the ANA standards of practice. Endorses the theory-based approach through the continuum of practice from novice to expert. 

 

Mentor: Advances the profession of nursing and the NPD specialty by contributing to the professional development of others and supporting the ongoing professional learning as individuals develop across practice, professional and educational settings.

 

Competency Management: Utilizes tools to capture knowledge, skill, ability judgment and performance during the situational or dynamic phases of practice (i.e. initial and ongoing competencies).

 

Orientation: Develops, facilitates, conducts and evaluates programs during the onboarding phase and when changes in roles, responsibilities and practice occur.

Change Agent: Works to transform processes at micro, meso and/or macro system levels.  Uses change management strategies and theories to drive desired outcomes.

Champion of Scientific Inquiry Promotes a spirit of inquiry, the generation and dissemination of new knowledge and the use of evidence to advance NPD practice, guide clinical practice and improve the quality of care for the healthcare consumer/partner.

Qualifications

EDUCATION/CERTIFICATION/EXPERIENCE:

  • BSN

  • Minimum of 3 years of active clinical nursing experience.

  •  

    At least 1 year of experience in an acute care environment or area of specialty for ambulatory/ancillary environments. 

     

  • Experience as a preceptor. 

     

  • Licensed Registered Nurse from the CT DPH obtained prior to hire date.

  •  NPD or Clinical Specialty Certification within two years of hire hire/transfer

  • BLS required
