The Pediatric NP Provider Hospice/Palliative Care has two primary responsibilities.
Provide NP-appropriate service to patients enrolled in hospice and may also assist in staffing pediatric palliative care consultation services that function as a program of MJHS Hospice and Palliative Care
Work in the community environment, and may, on occasion, provide consultation in the long-term care environment or hospital.
In all settings, provide in-person visits or telehealth visits, as appropriate.
Providing comfort care consistent with best practices, including history-taking, appropriate examination, formulation and care planning, treatment recommendation or implementation, communication, and documentation
In the role as hospice NP, you will coordinate with the team physician and manager and will be responsible for face-to-face evaluations for hospice eligibility, and the assessment and management of hospice patients in need of exigent medical care
Contribute to the training of the hospice team, providing support for the interdisciplinary team, as needed
Help build a teaching curriculum on pediatric aspects of hospice care that will be of use to our hospice workers who are engaged in pediatric care through cross coverage for when young adolescents are incorporated into adult hospice teams
In providing both direct care and support to other clinicians, the NP will use an evidence-based approach to management and to the use of materials and policies/procedures
Other activities related to quality improvement projects may also be required, as appropriate
• Master’s degree in nursing and graduate of an approved Nurse Practitioner Program required.

The Nurse Practitioner (NP) provider is one of a group of MDs and NPs who staff palliative care consultation services that function as programs of MJHS Hospice and Palliative Care. The NP Provider may work in a nursing home, hospital or community environment. Specialist skills in symptom management, communication including goal setting discussions, and hospice eligibility review, and may work independently or as part of a treatment team. Initial patient encounters are in response to consultation requests, and the initial consultation may be followed by one or more visits, as appropriate to the referral questions and status of the patient and family. The care plan advanced by the NP Provider aims to prevent or mitigate illness burden for the patient and family. Each encounter comports with best practices of specialist palliative care, including history-taking, appropriate examination, formulation and care planning, treatment recommendation or implementation, communication, and documentation. Other responsibilities include developing quality initiatives, educating staff, and supporting hospice operations through participation in the provider on call rotation and performance of face to face encounters as needed.

  • Implement Specially Designed Instruction (SDI) that follows the gradual release model to explicitly address individual student's unique needs.
  • Draw on content-area expertise to infuse skill-building into content and adhere to the gradual release model with fidelity
  • Complete Task Instructional Plan.
  • Participate in IEP meetings
  • Use a variety of data sources to track and discuss student progress, setting goals for continued growth.
  • Regularly review student data through an electronic management system.
  • Collaborate with various teams to write teacher reports.
  • Manage student behavior using restorative practices and school-wide PBIS.
  • Implement students behavior plans
  • Observe behaviors within the classroom and develop classroom routines/structures and modify expectations to meet the behavioral needs of individual students
  • Communicate with families and stakeholders to support student success
  • Attend a 3-week summer orientation and professional development series.
  • Fulfill broader school roles, including mentoring and committee involvement.


Requirements:
  • Bachelor’s degree required; Master’s Degree preferred
  • New York State Certification
  • Both novice and experienced teachers may apply
  • Experience in a Transfer School preferred
  • Google Applications proficiency preferred
  • Preferably knowledgeable in the use of SESIS
  • Ability to create multi-tiered classroom structures and behavior plans to address multiple disabilities (ED, LD, OHI, etc)
  • Demonstrated ability to “multi-task” and deliver high quality work
  • Commitment to the UD Team mission and core values of Teamwork, Leadership, and Communication
  • Commitment to the use of restorative practices and a strength-based, youth development approach to student issues
  • Ability to function well as part of a team and work independently
  • Must be team-oriented, with a strong work ethic, excellent communication skills, a passion for serving at-risk youth, and a sense of humor

COMPENSATION & BENEFITS:

Compensation: $70,969 to $114,450 annually based on years of experience and education.
