This job provides respiratory care treatment, oxygen services, diagnostic testing and ventilation support services to both in-patients and out-patients ranging in age from neonatal to geriatric. The RT-CRT set-ups, monitors and weans Oxygen Therapy and Artificial ventilation. The RT-CRT draws, analyzes and reports Arterial Blood Gases and EKGs. The RT-CRT delivers intermittent therapy and assists with changing of artificial airways. The RT-CRT provides services as ordered by a physician and in accordance with established policies and procedures.

Position Description: The SBHC Nurse Practitioner is responsible for providing primary healthcare, as outlined by Jefferson Parish Schools and the Office of Public Health.

  • Elicits a comprehensive health history and performs a physical assessment, sports or comprehensive physical

  • reporting or referring to a medical doctor any abnormal findings.
  • The nurse practitioner is directly accountable to the medical director in matters that pertain to medical

This job works under the supervision of the Medical Director or approved staff physician and in association with the PCCs, staff nurses, and discharge coordinator in the planning, implementation and evaluation of medical and nursing care; and assumes responsibility for primary and urgent medical management, emergency stabilization of all critically ill patients, teaching of residents and medical students, staff development and education, patient education and discharge planning.

EDUCATION AND EXPERIENCE CRITERIA:
  • MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required
  • A minimum of 2 years’ clinical experience required; 3 years preferred.
  • Strongly prefer one (1) years’ previous experience as Medical Director or equivalent with a Medicare or Medicaid patient population
  • Board eligibility is required.
  • Board Certified in Internal Medicine, Family Medicine, Geriatrics or similar is preferred, but hires may have other sub-specialty training and board certification.
  • Current, active license to practice medicine in State of employment.
  • High performing physician with a proven track record of clinical leadership experience.
  • Must have completed all internal physician training and have attained partnership.
  • Experience with population risk management or complex chronic disease care management.
  • Experience working with interdisciplinary teams in quality improvement and/or medical/healthcare leadership activities preferred.
  • Preferred to be an existing high performing PCP partner and/or Medical Director within the ChenMed core model, with a proven ability to manage a panel of >400 patients with outstanding clinical, customer service and cost outcomes. Preferred to have been with the organization >2 years, be a recognized leader amongst peers, and can lead teams in quality and performance improvement initiatives.
  • If specialty, has demonstrated leadership within his/her specialty and delivered superior outcomes, with a proven ability to deliver primary care in our model.

PAY RANGE:

$238,832 - $341,189 Salary
