The Clinical Manager ensures the client's Plan of Care is accurate, individualized, and consistently updated based on the patient’s condition. This role requires strong Home Health leadership experience, advanced knowledge of HCHB (HomeCare HomeBase), and the ability to manage clinical staff, orders, documentation, and weekly case conferences.
Key Responsibilities
Clinical Oversight
- Ensure Plan of Care is implemented and updated per patient needs or clinical changes.
- Review and ensure accurate evaluation packets, OASIS, and 485 forms.
- Process OASIS assessments, verify SOC dates, and correct documentation discrepancies.
- Follow up on missing documentation and ensure completion by Licensed Professionals.
- Manage referral intake and ensure timely scheduling of assessment visits.
- Lead weekly Case Conferences and guide recertification/discharge decisions.
- Review and approve Case Conference coordination notes within 48 hours.
- Communicate with physicians to obtain, confirm, or update clinical orders.
Order Management
- Review, approve, or decline orders per protocol.
- Ensure modifications, corrections, and medication updates are completed accurately.
- Track unsigned orders and follow up as needed.
- Ensure frequency-related orders reflect correct scheduling.
Quality & Patient Safety Monitoring
- Review vital-sign alerts and wound-score deviations; notify physicians as appropriate.
- Enter non-admit information in HCHB and ensure Branch Director approval.
- May perform patient visits or be included in on-call rotations.
Administrative Duties
- Verify benefits and obtain authorization for non-Medicare patients.
- Follow up on billing claim audit deficiencies within 24 hours.
- Run and submit missed-visit notifications to physicians weekly.
- Serve as backup for PSC functions (rescheduling, lab report review when allowed).

The Dental Hygienist will provide comprehensive preventive and therapeutic dental care at the Naval
Hospital Bremerton Dental Department. This position focuses on patient education, oral health

• Examine patients’ oral cavities—including mouth, throat, and pharynx—and record conditions of

lesions, or periodontal disease to the supervising dentist for evaluation and treatment.
• Apply desensitizing and topical agents to aid in the prevention or treatment of caries, gingivitis,
and oral ulcerations.
• Provide oral hygiene instruction to individual patients and groups, demonstrating proper
brushing and flossing techniques, explaining the causes of tooth decay, and discussing the role
of nutrition and general health in oral care.
• Take impressions and fabricate sports/safety mouth guards and soft night guards for use in vital
bleaching or protection.
• Polish restorations and apply pit and fissure sealants as indicated.
• Assist chairside during dental procedures when directed by the Officer-in-Charge of the dental
clinic.
• Record treatment data, maintain patient documentation, and ensure instruments and
equipment are cleaned, sharpened, sterilized, and in proper working order.
• Utilize Dentrix to accurately enter patient-specific workload data, readiness status, and to
schedule patient appointments in accordance with Navy protocols.

Essential Functions & Responsibilities
- The physician serves as medical director for skilled nursing facilities (SNF) and participates in administrative and quality initiatives for the facilities he or she is responsible for;
