This position is responsible for delivering specialized Home and Community-Based Services (HCBS) to eligible clients with SUD and/or SMI.
Services will be tailored to meet individual needs and adhere to guidelines outlined in the New York State HARP Adult Behavioral Health Home and Community-Based Services Provider Manual.
Minimum Qualifications: Unlicensed staff must be at least 18 years of age and have a high school diploma or equivalent, and 1-3 years of relevant experience working with individuals with SUD disorders and/or SMI or a BA degree.
Certified Peer Specialist/Certified Recovery Peer Advocate, or equivalently qualified by education in the human services field or a combination of work experience and education, with one year of education substituting for one year of experience.
Major Duties and Responsibilities : The HCBS Unlicensed Behavioral Health Provider is an active member in the FLACRA team and provides services identified and approved through HARP Care Management and Managed Care Organization (MCO) to clients with who meet eligibility criteria for HCBS services.
Under the supervision of the Licensed Program Director the HCBS Unlicensed Behavioral Health Provider will function to provide the following service types: Β· Psychosocial Rehabilitation (PSR) Β· Habilitation (HAB) Β· Family Support and Training (FST) Β· Education Support Services (ESS) Β· Empowerment Service β Peer Supports (EMP) Β· Pre-Vocational Services (PVOC) Each service type is defined in the New York State: Health and Recovery Plan (HARP) Adult Behavioral Health Home and Community Based Services (BH HCBS) Provider Manual.
Duties Serves as an active member of the clientβs treatment team, including participation in team meetings and treatment planning meetings.
Team members include but arenβt limited to the care managers/HARP coordinators.
Manage individual caseloads, including referrals, intake, prior authorization with insurance providers, input/management of service thresholds, development of individual service plans and care plan reviews/updates as well as discharge/transition planning.
Will additionally complete/coordinate care with referral source for crisis plan.
Provides services as approved by HARP and the MCO as described in the BH HCBS Provider Manual.
Serves as role model for clients by sharing own recovery process and teaching effective coping techniques and self-help strategies depending on HCBS service line.
Provides case management, resourcing, service collaboration for individual clients as extension of case management services.
Provides advocacy services on behalf of clients.
Informs supervisor, and other members of treatment team as indicated, of issues/concerns related to individual clients.
Participates in required trainings to maintain certification and HCBS requirements.
Knowledge of Crisis Intervention and Early Engagement Helps facilitate outreach and encourage engagement in appropriate treatments Provides services in the community which may include clientβs homes.
Providers transportation services as appropriate.
Other duties as assigned.

Communicates and documents assessment of patient with the interdisciplinary team. Utilizes all available resources in order to meet the needs of the patient and family.
