Initial patient encounters are in response to consultation requests, and the initial consultation may be followed by one or more visits, as appropriate to the referral questions and status of the patient and family. The care plan advanced by the NP Provider aims to prevent or mitigate illness burden for the patient and family. Each encounter comports with best practices of specialist palliative care, including history-taking, appropriate examination, formulation and care planning, treatment recommendation or implementation, communication, and documentation. Other responsibilities include developing quality initiatives, educating staff, and supporting hospice operations through participation in the provider on call rotation and performance of face to face encounters as needed.

- Bachelor’s degree in process engineering, packaging technology, chemical engineering, or a related field (Master’s degree preferred).
- Minimum of 2 years’ experience in paper, packaging, or a similar industry.
- Background in data networks troubleshooting.
- Strong verbal, written, and interpersonal communication skills.
- Demonstrated ability to follow instructions and execute tasks effectively.
- Proficiency in Windows and Microsoft Office applications.
- Industry certification such as CTS preferred.
- Proven project management experience.
- Willingness to travel up to 10%.
