The Interventional Radiology (IR) Nurse provides specialized nursing care to patients undergoing minimally invasive procedures in the IR lab. This role involves pre-procedure assessment, intra-procedure monitoring, sedation administration, and post-procedure recovery management. The IR Nurse works closely with radiologists, technologists, and other healthcare professionals to ensure patient safety, comfort, and optimal procedural outcomes.
Essential Work Functions:- Perform comprehensive pre-procedure assessments, including reviewing medical history and lab results
- Educate patients and families about IR procedures, sedation, and post-procedure care
- Administer and monitor moderate sedation, ensuring patient comfort and safety
- Monitor hemodynamic status, cardiac rhythms, and oxygenation levels during procedures
- Assist physicians during procedures, ensuring proper positioning, sterile technique, and instrument handling
- Recognize and respond to complications, escalating care when necessary
- Provide post-procedure monitoring and recovery care, including pain management and patient discharge education
- Maintain accurate documentation of patient assessments, medications, and procedural details
- Collaborate with IR technologists, radiologists, and other healthcare team members to deliver coordinated care
- Adhere to hospital safety protocols, infection control guidelines, and regulatory standards
- Perform other duties as assigned within the scope of practice

We are seeking a Per Diem Speech Therapist (SLP) to support our Santa Clara County patients. This role is ideal for an SLP who wants control over their schedule, manageable territories, and a well-run home health structure behind them.

Your Role
- Provide skilled speech therapy services in the home per physician orders and plan of care
- Evaluate, treat, and monitor patients with communication, cognitive, and swallowing disorders
- Educate patients and caregivers
- Communicate findings and progress with physicians and the interdisciplinary team
- Complete documentation in Kinnser with support from AI-assisted charting tools
- Participate as a collaborative member of the care team

- Current California Speech Therapist license
- Home health experience preferred (training available for strong SLPs new to home health)

Responsibilities
- Interprets, reviews and evaluates medication orders for optimal dose, dosage form, frequency, concentration, duration, drug-drug interactions, drug-disease state interaction, drug allergy interaction, and potential cross allergenicity, taking into account the patient's special or age related needs, to ensure optimal drug therapy, high quality patient care and timely dispensing of accurate and appropriate medications.
- Dispenses medications by reviewing patient specific bulk medications and pyxis fills and refills for accuracy of the right medication, dose, dose form, strength, size, quantity to ensure accurate, timely and safe pharmaceutical care.
- Participates in process improvement teams within the department and organization to build strong relationships with staff members, demonstrates GFH core values and enhances financial viability.
Education/Experience:
- Bachelor of Science Degree in Pharmacy or Doctorate of Pharmacy from a pharmacy program accredited by the Accreditation Council for Pharmacy Education (ACPE)
- Hospital Pharmacy experience preferred.
Licenses/Certificates/Registrations
- Current, active license to practice pharmacy New York State; or eligible to reciprocate licensure from another state
- Pharmacy Registration in New York State
Skills/Abilities:
- The pharmacist needed to following:
- Communication and language skills to read analyze and interpret medication orders and information, the ability to respond appropriately to emergent situation, staff, physicians, and patient inquiries or complaints and the ability to effectively communicate important information to
- management peers and other health professionals.
- Ability to perform complex Pharmaceutical calculations and an understanding of statistical concepts such as frequency, distribution, standard deviation and variances.
- The pharmacist needs the ability to solve clinical problems related to pharmaceutical care and drug therapy.
- Knowledge of current pharmacy principles and practices
- Knowledge of computer order entry
- Knowledge of federal and state laws governing pharmacy practices
- Knowledge of regulatory requirements and standards
- Ability to work independently within established policies and procedures
- Ability to accomplish established program goals and objectives
- Ability to exercise judgment, tact, and diplomacy
