Job Summary:
The Clinical Outreach Provider (NP/PA) delivers high-quality primary care across diverse community settings, including school-based health centers (serving K?12), homeless shelters, outpatient treatment centers, and other outreach locations. This role is designed for a nurse practitioner or physician assistant with a background in family medicine or internal medicine. Pediatric or substance use management experience is preferred but not required.
The provider will offer clinical services both within Care Alliance?s brick-and-mortar clinics and through mobile medical units, ensuring access to care for underserved and vulnerable populations. The Clinical Outreach Provider will practice within an integrated, team-based model of care, collaborating with physicians, behavioral health clinicians, pharmacists, case managers, and community partners.

- Provide comprehensive primary care services across the lifespan, including preventive, acute, and chronic disease management.

- Prescribe medications, order diagnostic tests, and develop individualized care plans.

Minimum Education and Experience:
Required:
