POSITION SUMMARY/RESPONSIBILITIES:
Prepares pharmacy prior authorization requests for review by nurse/pharmacist/physician reviewers. Completes pharmacy authorization requests and issues review outcome notifications to Community First Health Plans (CFHP) members and providers. Follows clinical criteria and instructions to approve prior authorization requests based on established criteria, plan policies and procedures. Performs non-medical research including eligibility verification and benefits verification. Reviews medication utilization reports and identifies trends through advanced data analytics. Produces reports to comply with regulatory and accreditation standards.
EDUCATION/EXPERIENCE
Graduation from high school/GED required. Bachelor's degree preferred. Five years of pharmacy experience required. Some coursework at the college level is preferred. Excellent customer service, phone etiquette, attention to detail, and typing skills is required. Basic knowledge of Microsoft Office and internet required. Experience in health insurance/pharmacy insurance or managed care organization preferred. Prior authorization processing experience desired. Experience in a PBM or managed care call center desired. Knowledge of InterQual screening criteria as well as DRG, ICD-10 and CPT coding is preferred.
LICENSURE/CERTIFICATION
Texas State Board of Pharmacy registration required. Certification as a Pharmacy Technician (CPhT) is required.

Two years of CT experience is required. Must meet and maintain the clinical ladder requirements for this specialty.

POSITION SUMMARY/RESPONSIBILITIES
Functions proficiently in all technical aspects of the Cardiac Catheterization Laboratory. Assists the cardiology team in performing diagnostic and interventional cardiac catheterizations and other related procedures. Performs a variety of duties including operation of radiographic equipment and imaging equipment, electrophysiologic and hemodynamic recorder and computer.

EDUCATION/EXPERIENCE
A high school diploma is required. An associate's degree in cardiovascular technology or completion of a program for radiology technology is required. Registered Cardiovascular Invasive Specialist (RCIS) is preferred.

LICENSURE/CERTIFICATION
Registered Cardiovascular Invasive Specialist (RCIS) or ARRT by recognized credential body is required. For the Adult Cath Lab technologists - Current BLS and ACLS certification is required. RCIS is preferred. For the Pediatric Cath Lab technologists - Current BLS, ACLS and PALS certification is required. ACLS must be obtained within 30 days of hire. RCIS is preferred.

POSITION SUMMARY/RESPONSIBILITIES
Provides clinical and distributive services for Neonatal through Adult patients in accordance with JCAHO, OSHA, Medicare/Medicaid standards and Texas State Board of Pharmacy Rules and Regulations. Dispenses and labels drugs or devices, selects drugs and reviews drug utilization, stores prescription drugs and devices and maintains drug records in a pharmacy. Advises or consults when necessary or required by law about therapeutic value, content, hazard, or use of drugs and devices, or offering or performing the services and transactions necessary to operate a pharmacy. Implements decisions set forth by the P&T Committee as written in the Hospital Formulary and complies with Health System policies and procedures with regard to the dispensing, maintaining and storing of drugs.
EDUCATION/EXPERIENCE
Pharmacy licensure in the State of Texas is required. Two years experience in a hospital pharmacy preferred.
