Job Summary:

The Nursing Professional Development (NPD) Practitioner is a Registered Nurse with NPD practice judgement and expertise who influences professional role competence and growth of learners. This position works in collaboration with nursing leadership to foster and influence competency management, clinical nursing education, professional role development, policy standardization, collaborative partnerships, scientific inquiry, transition to practice and onboarding/orientation of new Hartford HealthCare colleagues. The practitioner is an essential member of the nursing leadership team who promotes the alignment of regional NPD and practice excellence with the HHC system.

 

Job Responsibilities:

The area of coverage for this position is Labor & Delivery/Maternal Newborn.  Key areas of responsibility are:

 

·        Learning Facilitator: Uses the educational design process and adult learning principles to bridge the knowledge, skills and/or practice gaps identified through learning needs assessment and evaluation of outcomes.

·        Collaborative Partner: Collaborates with members of the nursing leadership team to assess learning needs, develops programs, monitors clinical competency and supports action plans.Represents nursing in the development of inter-professional learning activities.

·        Role Development: Promotes professional nursing practice and quality patient care according to the ANA standards of practice. Endorses the theory-based approach through the continuum of practice from novice to expert. 

·        Mentor: Advances the profession of nursing and the NPD specialty by contributing to the professional development of others and supporting the ongoing professional learning as individuals develop across practice, professional and educational settings.

·        Competency Management: Utilizes tools to capture knowledge, skill, ability judgment and performance during the situational or dynamic phases of practice (i.e. initial and ongoing competencies).

·        Orientation: Develops, facilitates, conducts and evaluates programs during the onboarding phase and when changes in roles, responsibilities and practice occur.

·        Change Agent: Works to transform processes at micro, meso and/or macro system levels.  Uses change management strategies and theories to drive desired outcomes.

·        Champion of Scientific Inquiry Promotes a spirit of inquiry, the generation and dissemination of new knowledge and the use of evidence to advance NPD practice, guide clinical practice and improve the quality of care for the healthcare consumer/partner.

Qualifications

Education:

·       BSN required and enrolled in a master’s program with degree completion within two years of hire date.

 

Licensure, Certification, Registration:

·       Licensed Registered Nurse from the CT DPH obtained prior to hire date.

·       RNC-OB or C-EFM preferred.

·       Neonatal Resuscitation preferred.

·       NPD or Clinical Specialty Certification within two years of hire hire/transfer BLS required. 

·       NPD Specialty certification preferred.

 

Work Experience:

·       Minimum of 3 years of active clinical nursing experience.

·       At least 1 year of experience in an acute care environment, Labor & Delivery or Maternal Newborn.

·       One year of relevant teaching experience preferred, nursing professional development and/or specialty area preferred.

·       Experience as a preceptor or other informal educator role.

·       Demonstrates ability to develop, deliver and evaluate educational programs.

·       Working knowledge of high reliability and LEAN process improvement strategies.

·       Demonstrated leadership skills and a commitment to education as a methodology for promoting continuous quality improvement in the delivery of patient care and services.

·       Knowledge of the principles/theories of adult learning.

For over 60 years, St. Vincent’s Special Needs Services (SVSNS) has been a trusted provider of specialized services for children and adults with multiple developmental disabilities and complex medical needs, such as cerebral palsy, acute brain injury, neuromuscular disorders and autism spectrum disorders. Each year, more than 400 children, adults and their families receive services from the organization.

Provide comprehensive nursing oversight in adult residential group homes for medically complex individuals. The Nurse Case Manager ensures optimal care and safety for residents, manages health assessments, and supports staff through training and audits. The role involves collaboration with interdisciplinary teams to coordinate care plans, monitor outcomes, and maintain compliance with regulatory and quality standards.

The Nurse Case Manager participates in a collaborative coverage model, overseeing nursing needs across multiple group homes rather than a single site. Each nurse may cover up to 3.5 homes, dedicating approximately 8–10 hours per home weekly. This structure supports individualized care and continuity, while aligning with HHC’s standards for regulatory compliance and quality improvement.

Clinical Oversight & Documentation

•             Reviews all documentation by staff, including unlicensed staff and LPNs

•             Audits Medication Administration Records (MARs), physician orders, and medication practices in partnership with HHC Quality

•             Ensures completion and follow-up of medical appointments

•             Coordinates directly with providers when needed

•             Completes nursing notes per DDS and HHC Quality recommendations

•             Conducts quarterly medication review audits

•             Serves as primary medical contact for state or regulatory inquiries related to assigned residents, clinical practices, or nursing oversight within designated homes

Resident Assessments & Care Planning

·        Performs nursing assessments on admission, weekly, and with any change in condition

·        Develops, reviews, and updates nursing care plans quarterly and as needed

·        Participates in new admission planning, including:

o   Nursing Health and Safety Assessment

o   Self-Medication Assessment

o   Fall Risk Assessment

o   Health History and Diagnoses

o   Care Plan development

Qualifications

Job Summary:

The Trauma Quality Performance Improvement Patient Safety (PIPS) Nurse is responsible for monitoring and ensuring quality care for the trauma patient.  The PIPS nurse works under the direction of the Trauma Program Manager and the Trauma Medical Director and collaborates with leadership teams to support the trauma system.  The PIPS nurse helps to coordinate all aspects of trauma care including identifying, developing and implementing standards of care through performance improvements processes and is accountable for assessing the effectiveness of the standards of care.

Responsibilities:

·        Performs primary review on all defined trauma patients for performance improvement tracking across the continuum of care.

·        Prepares and presents select cases for secondary review clearly and concisely with all relevant clinical information and opportunities for improvement. 

·        Monitors and evaluates the care for the trauma patients concurrently and retrospectively through issue identification, primary, secondary, and tertiary reviews as well as multidisciplinary reviews.

·        Assists in identifying opportunities related to the care of the trauma patient, designs and monitors indicators for the continuous quality improvement processes, assures timely evaluation and evaluates the quality and effectiveness of care to the trauma patient in an organized format that supports loop closure.

·        Works collaboratively with members of the trauma program to monitor the process and outcomes of patient care to trauma patient throughout the continuum.  Ensures quality care is being provided through monitoring and tracking and using the information to improve the knowledge and skill of trauma care providers and promote performance improvement and patient safety within an established institution structure. 

·        Participants in the preparation for all site visits and/or accreditations to ensure maintenance of requirements established by the State and the American College of Surgeons.

·        Assists in ensuring data abstraction and entry into the trauma registry database is accurate.  Provides coordination and facilitation of data collection, analysis, trending and benchmarking of data and outcomes (NTDB, TQIP, etc.) using appropriate registries. Assists with the Registry to facilitate performance improvement activities.

·        Attends trauma related performance improvement and morbidity and mortality meetings on a regular basis.

·        Manages professional growth and development by participating in trauma related educational opportunities, committees and projects at the local, state and national level.

·        Perform other related duties as required.



Qualifications

Education:

·        Bachelor’s degree in nursing.  Master’s degree in nursing, Business or Health Administration preferred.

·        2-years of experience in a critical care unit or emergency department setting.   5-years of experience in a critical care unit or emergency department setting; 2 years supervisory or project management experience preferred.

Licensure, Certification, Registration:

·        RN, BLS and ACLS certification.

·        Trauma Nursing Core Course (TNCC)/Trauma Certified Registered Nurse (TCRN) preferred.

Work Experience:

·        Basic computer knowledge to enter/retrieve data for reporting. Ability to work with an electronic medical record (EMR) and trauma registry software for data extraction and input.
